A fifteen year old boy and the forty two year old women were treated because of blepharochalasis. In both cases the goiter and cheilochalasis were observed. Ascher's syndrome was diagnosed on the basis of these three principal symptoms. The facial appearance and the eyelids functioning were improved by plastic surgical treatment. It also stopped the illness developing.

Methods to rejuvenate the eye region include shortening of the forehead, raising the brow, improving the upper eyelids by excision of blepharochalasis tissue, and reducing lower eyelid fat pads by a transconjunctival approach and at the same time elevating the stretched lateral canthus by an effective canthopexy. The risks and complications of these procedures are minimal, circumscribing the shortcomings of conventional blepharoplasty.

Ptosiform blepharochalasis is not only an esthetic but also a functional problem for the elderly patient. In clinical terms there is an extension of the lid skin over upper-lid margin, a relaxation of the orbicularis muscle and a large amount of periorbital fat. The operation involves resection of the excess skin by an elliptical excision, shortening of the orbicularis muscle, and resection of the fat pads from two compartments. Postoperatively there is only edema, but no pain, which is important for the elderly.

When blepharoplasty is performed by transconjunctival route, sometimes the excess of skin created by overlaxness of the lower lid does not retract to achieve the desired cosmetic result. We propose a method for controlled retraction of the eyelid by coagulating the skin using the CO2 laser. Thanks to this approach, the lower eyelid's tone recovers and the desired cosmetic appearance is achieved. We describe a 21-year-old male patient with blepharochalasis, a form of localized acquired cutis laxa. He had a 13-year history of recurrent swelling attacks of the eyelids of unknown origin leading to periocular localized cutis laxa. There are several groups of patients seeking lower lid blepharoplasty who present with the potential for unsatisfactory postoperative results, such as those presenting with atonic eyelids, patients with severe blepharochalasis alone or with large fat bags, patients with festoons, those with abnormal anatomy, and patients with deformity due to prior surgery. The anatomical structure of fascial tissue of the upper eyelid is described in this article. It is emphasized that removing redundant fascial tissue is an important procedure in the double-eyelid operation of the upper eyelid. This is especially true for patients with baggy deformities or blepharochalasis. Based on this principle, 654 cases of double eyelidplasty were performed from 1990 to 1993 with satisfactory results revealed by postoperative follow-up of 1 month to 3 years.

The blepharochalasis syndrome is an uncommon disorder seen in young people that is characterized by recurrent episodes of eyelid edema. After repeated episodes various eyelid supportive structures are stretched, giving rise to the characteristic features of thin excess lid skin, blepharoptosis, pseudoepicanthal folds, and disinserted lateral canthal tendons. Surgical repositioning and reconstruction are often required. The author describes 10 patients with this disorder and suggests a relation to angioedema, a common dermatologic condition with numerous triggering mechanisms.
