Platelet-Derived Growth Factor Isoform Imbalance Is Associated with Innate Inflammation and Corneal Epitheliopathy in Dry Eye Disease.

INTRODUCTION: Tear-deficient dry eye disease (DED), including Sjögren's syndrome and ocular graft-versus-host disease (oGVHD), is often refractory to therapy. We tested whether platelet-derived growth factor (PDGF) isoform imbalance, quantified as a PDGF-AB/BB: PDGF-AA ratio ("PDGF Ratio"), is associated with innate activation and corneal epitheliopathy. METHODS: Tear samples from 151 participants were profiled across two independent assay runs: discovery (n = 79) and replication (n = 72). Data were harmonized as within-plate rank percentiles. Plate-adjusted mediation tested whether the PDGF

Beyond overlap: a Ro52-driven glandular-pulmonary phenotype at the interface of Sjögren disease and anti-synthetase syndrome in a multicentre cohort.

BACKGROUND: While Sjögren disease (SjD) overlap is known to modulate the clinical-serologic manifestations of other connective tissue diseases, the association with anti-synthetase syndrome (ASyS) has been seldom described in Caucasian cohorts. Anti-Ro52 autoantibodies, shared by both conditions, may blur early diagnostic attribution, particularly when glandular symptoms precede myositis-spectrum features. OBJECTIVES: To characterize the clinical phenotype, serological profile, and disease trajectory of patients fulfilling classification criteria for both SjD and ASyS. METHODS: We conducted

Human hypofunctional NCF1 variant aggravates salivary gland immunopathology in Sjögren's disease by promoting switched memory B-cell recruitment and long-lived plasma cell differentiation.

OBJECTIVES: We assessed the role of a systemic lupus erythematosus causal, hypofunctional variant, neutrophil cytosolic factor 1 (NCF1)-p.Arg90His (p.R90H) substitution, in Sjögren's disease (SjD). METHODS: Association between NCF1-H90 and SjD was assessed in case-control cohorts. Peripheral blood mononuclear cells (PBMCs) and minor salivary gland (MSG) from patients with Sjögren's Syndrome type A antigen (SSA)+ SjD were assessed using cytometry by time-of-flight and single-cell RNA sequencing, respectively. RESULTS: The NCF1-H90 allele was associated with increased risk for SjD in Chinese a

Smart In Situ-Forming Ocular Therapeutics for Dry Eye Disease: Bridging Biomaterials, Nanotechnology, and Clinical Translation.

Dry eye disease (DED) is a multifactorial inflammatory condition of the ocular surface that significantly impairs quality of life through tear film instability and chronic irritation. Despite the prevalence of DED, conventional topical therapies are hindered by poor ocular bioavailability (< 7%) due to rapid precorneal clearance and restrictive anatomical barriers. Stimuli-responsive in situ forming systems have emerged as a transformative strategy to overcome these limitations by undergoing a "sol-to-gel" transition in response to physiological triggers such as temperature, pH, or ionic stren

Paraproteinemias in Sjögren disease: cryoglobulinemia and serum monoclonal gammopathy identify distinct clinical phenotypes and differential lymphoma susceptibility.

OBJECTIVE: Sjögren disease (SjD) is a systemic autoimmune disease characterized by increased risk of B-cell non-Hodgkin lymphoma. Although cryoglobulinemia and serum monoclonal component (MC) are well-recognized paraproteinemias in SjD, no large-scale study has directly compared their isolated and combined impact on phenotype and lymphoproliferative risk. METHODS: A retrospective, multicenter, cross-sectional study was conducted within the Italian GRISS registry. Patients with SjD were stratified into four groups: isolated monoclonal gammopathy (MC-alone), isolated cryoglobulinemia (CRYO-alon

Evolving treatments for Sjögren disease: current approaches and emerging targets.

Sjögren disease (SjD) is a prevalent systemic autoimmune condition characterised by exocrine gland dysfunction, systemic inflammation and heterogeneous organ involvement. Current management remains largely symptomatic, with no approved disease-modifying therapies available and substantial unmet clinical need. However, advances in understanding immunopathogenesis have accelerated the development of targeted treatments. Ianalumab, a dual-acting B cell-activating factor receptor (BAFF) receptor inhibitor with B cell-depleting activity, is the first agent to report positive phase 3 results, showin

[Current treatment, foreseeable newcomer and new treatment approaches for Sjögren's disease].

Although Sjögren's disease (SjD) is the most frequent connective tissue disease, there are still no approved medications for systemic treatment. Many disease-modifying antirheumatic drugs (DMARD) and biologicals have not shown sufficient efficacy in studies. The reasons for this are the complex immunological mechanisms and a high degree of clinical and immunological heterogeneity of the patients; however, significant progress has been made in recent years: for the first time 2 phase 3 studies (ianalumab and telitacicept) reported positive results. The current treatment recommendations are base

Chrysanthemum indicum L.-derived extracellular vesicles enhance the therapeutic efficacy of cyclosporine a against dry eye disease.

The pathological progression of dry eye disease (DED) involves a vicious cycle of oxidative stress and inflammation, posing a critical therapeutic challenge. Conventional therapies, such as cyclosporine A (CsA), are limited by poor corneal permeability and low ocular bioavailability. Here, we developed a novel, biocompatible nano-eye drop formulation using Chrysanthemum indicum L.-derived extracellular vesicles (CELNs) as a natural nanocarrier to engineer CsA-loaded CELNs (CsA@CELNs) for synergistic DED therapy. By combining CsA-mediated immunoregulation with enhanced corneal permeability and

Matching-adjusted indirect comparison of acoltremon ophthalmic solution 0.003% and cyclosporine 0.05% ophthalmic emulsion for increased tear production in patients with dry eye disease.

Aim: Dry eye disease (DED) is a chronic, multifactorial condition arising through loss of tear film and ocular surface homeostasis. Treatment aims to restore natural tear production and normalize ocular surface homeostasis. Several prescription medications are available in the US that increase tear production in patients with DED. In the absence of head-to-head trials, this study employed a matching-adjusted indirect comparison (MAIC) approach to estimate the comparative tear production efficacy of acoltremon 0.003% and cyclosporine 0.05% for the treatment of DED.
