Pentyl (2E)-3-(3-methoxyphenyl) prop-2-enoate attenuates acute respiratory syndrome induced by intestinal ischemia and reperfusion in rats.

Acute respiratory distress syndrome (ARDS) is a life-threatening inflammatory condition characterized by high morbidity and mortality, with limited effective pharmacological therapies currently available. The search for novel compounds with anti-inflammatory and antioxidant properties remains essential to improve clinical outcomes. In this context, Pentyl (2E)-3-(3-methoxyphenyl)prop-2-enoate (MAP), a phenolic cinnamic acid derivative, emerges as a promising candidate due to its potential biological activities. This study aimed to investigate the anti-inflammatory effects of MAP using an integ

Conjunctival Squamous Cell Carcinoma Masquerading as Orbital Cellulitis in a Patient With Relapsed Mixed Phenotype Acute Leukemia.

Conjunctival squamous cell carcinoma (SCC) rarely presents as orbital cellulitis, creating a significant diagnostic challenge in profoundly immunocompromised patients. We report the case of a 40-year-old man with relapsed mixed phenotype acute leukemia (MPAL) receiving prolonged leukemia-directed therapy who developed progressive unilateral periorbital swelling, pain, and imaging findings consistent with orbital cellulitis. Despite broad-spectrum antimicrobial therapy, serial computed tomography and magnetic resonance imaging demonstrated persistent superior orbital soft tissue thickening with

ENOblock attenuates intestinal ischemia/reperfusion-induced acute lung injury and improves survival.

BACKGROUND: Intestinal ischemia/reperfusion triggers systemic inflammatory responses that contribute to acute lung injury. Enolase-1 is a glycolytic enzyme increasingly implicated in its proinflammatory functions and in tissue injury. We hypothesized that an Enolase-1 inhibitor, ENOblock treatment would attenuate acute lung injury and improve survival following intestinal ischemia/reperfusion. METHODS: Adult C57BL/6 mice were subjected to superior mesenteric artery occlusion for 1 hour followed by 4 hours of reperfusion. Mice received either vehicle or ENOblock (5 mg/kg body weight) via retro

Orbital Inflammation can be visualized by black blood MRI in isolated polymyalgia rheumatica.

OBJECTIVE: Giant cell arteritis (GCA) and polymyalgia rheumatica (PMR) are considered as manifestations of a shared disease spectrum (GCA-PMR spectrum disease, GPSD). Data on orbital involvement in isolated PMR and across the spectrum remain limited. This study evaluated inflammatory intraorbital MRI findings across the GCA-PMR spectrum. METHODS: This retrospective observational study included patients with isolated PMR and combined GCA-PMR (presence of both clinical diagnoses and fulfilling classification criteria of GCA plus PMR). Patients with GCA and no signs and symptoms of PMR were not

Acute Orbital Inflammation following Neridronate Therapy: A Case Report.

INTRODUCTION: Bisphosphonates are known to occasionally induce ocular and orbital inflammation. While most reported cases involve agents such as zoledronate or alendronate, reports associated with neridronate remain exceedingly rare. Early recognition is vital as this condition can be sight-threatening but typically responds well to treatment. CASE PRESENTATION: We describe a 69-year-old man who developed fever and myalgia, followed by the abrupt onset of severe unilateral orbital inflammation 72 h after receiving an intramuscular injection of neridronic acid. Clinical examination of the left

Preseptal Cellulitis as the Presenting Sign of Systemic Inflammatory Disease Secondary to Chikungunya Virus: A Case Report.

INTRODUCTION: Preseptal cellulitis in children, a leading cause of pediatric orbital inflammation, often masquerades as more severe systemic inflammatory conditions. Chikungunya virus is an arbovirus with a wide spectrum of systemic manifestations that can lead to potentially harmful complications within the nervous, cardiovascular, and renal systems if left undetected. A diagnostic challenge resides with its rare ophthalmic presenting feature of preseptal cellulitis. Herein, we described a case of Chikungunya virus (CHIKV) masquerading as a preseptal cellulitis that led to a systemic inflamma

Painful acute esotropia as a presenting sign of presumed post-infectious orbital myositis in a child: a case report.

PURPOSE: To describe a self-limited presentation of pediatric orbital myositis and discuss its implications for management. OBSERVATIONS: A previously healthy 9-year-old boy presented with acute painful diplopia and esotropia following a recent febrile upper respiratory tract infection and presumed viral conjunctivitis. Best-corrected visual acuity was 20/40 OD and 20/40 OS. Examination demonstrated an esotropia of 30 prism diopters (PD) at corrected distance and 35 PD at corrected near, bilateral abduction limitation (- 1), and localized temporal conjunctival injection over the lateral rectu

Varicella-Zoster Virus and the Eye: Clinical Spectrum, Management, and Vaccination.

Varicella-Zoster Virus (VZV) is one of the most important pathogens in ophthalmology. Reactivation may involve the adnexa (blepharoconjunctivitis, pseudomembranous conjunctivitis), cornea (dendritic keratitis, nummular and necrotizing stromal keratitis, disciform endotheliitis, neurotrophic ulcers, mucous-plaque keratitis) and sclera (episcleritis, anterior scleritis). Uveal inflammation ranges from anterior uveitis-with iris atrophy, trabeculitis-induced glaucoma and complicated cataract-to posterior necrotizing syndromes: acute retinal necrosis in immunocompetent hosts and progressive outer

Herpetic panophthalmitis in a patient with a history of neonatal herpetic encephalitis.

Herpetic acute retinal necrosis (ARN) with orbital inflammation is a rare form of panophthalmitis. We report the case of a 15-year-old boy with hypoxic ischemic encephalopathy (HIE) secondary to neonatal herpetic encephalitis who developed unilateral ocular inflammation and proptosis. Imaging and aqueous PCR confirmed orbital inflammation and active HSV infection. Given the patient's comorbidities, aggressive surgical treatment was not pursued. Treatment aimed at protecting the contralateral eye from involvement requires life-long antiviral prophylaxis.

Panophthalmitis With Orbital Cellulitis Following Glaucoma Drainage Implant Surgery in an Adult Patient.

BACKGROUND Glaucoma drainage implants, including the Paul glaucoma implant, are widely used in the management of refractory glaucoma when medical therapy and conventional filtering surgery prove insufficient. Although associated with favorable outcomes, rare but serious postoperative complications such as endophthalmitis and panophthalmitis can occur, resulting in significant visual morbidity. CASE REPORT We describe the case of a 77-year-old man with chronic angle-closure glaucoma who developed acute panophthalmitis with orbital cellulitis 12 days following uneventful Paul glaucoma implant su

An atypical presentation of Lemierre syndrome in a child with nephrotic syndrome: a case report and literature review.

BACKGROUND: Lemierre's syndrome (LS) is a rare but potentially fatal condition characterized by septic thrombophlebitis of the internal jugular vein (IJV) following oropharyngeal infection, with an estimated incidence of 3.6 cases per million population. It primarily affects healthy adolescents and young adults; pediatric presentations, particularly in children under ten, are exceedingly rare and frequently atypical, with a heightened risk of diagnostic delay and multisystem morbidity. Although orbital cellulitis and intracranial venous extension have each been individually described in pediat

Primary Spontaneous Orbital Hemorrhage with Secondary Orbital Roof Blow-Out Fracture: A Case Report.

Background and Clinical Significance: Periorbital ecchymosis, or "raccoon eyes," often indicates a basilar skull fracture. When trauma is absent, clinicians should consider other causes, such as Valsalva maneuvers, bleeding disorders, or systemic inflammation. Spontaneous orbital hemorrhage is rare, and its appearance as a mass causing a secondary orbital roof blow-out fracture is even more unusual. This report presents a unique case of primary spontaneous orbital hemorrhage (PSOH) leading to a secondary orbital blow-out fracture; Case Presentation: A 60-year-old man with a history of ulcerati

Sustained response to rituximab in an ANCA-negative necrotizing vasculitis with initial detection of Corynebacterium kroppenstedtii.

OBJECTIVES: In this case, we present a patient with rapidly progressive unilateral destructive orbital and midface inflammation. Initial detection of Corynebacterium kroppenstedtii was followed by a consistent course of localised necrotizing small-vessel vasculitis, which was responsive to B-cell depletion. METHODS: A 79-year-old woman presented with acute left periorbital pain, tearing and purulent discharge. The workup included serial microbiological cultures, fungal and mycobacterial testing, molecular diagnostics, CT/MRI/PET-CT imaging, multiple surgical biopsies with histopathology and s

Frequency of AQP4 and MOG Antibodies in Patients With Optic Neuritis Fulfilling Minimal New Multiple Sclerosis MRI Criteria.

OBJECTIVES: Recent revisions to multiple sclerosis (MS) diagnostic criteria include the optic nerve as a site of dissemination in space, enabling this diagnosis in patients with acute optic neuritis (ON) and a single additional MS-typical location on MRI if dissemination in time (DIT) is demonstrated. We aimed to assess the frequency of non-MS diagnoses in this context. METHODS: We retrospectively analyzed consecutive patients with inaugural acute ON and at least 1 MS-typical lesion in a single brain location on baseline MRI across 3 French centers. All patients met DIT criteria and underwent

Advancing, Graduating, and Attesting Readiness of Pediatrics Residents With Concerns.

PURPOSE: Worldwide, as many as 6 million children annually sustain ocular trauma, with up to a quarter of a million children requiring hospitalization. Management of pediatric ocular trauma differs from that in adults, both in terms of initial assessment and acute intervention, with significant variation in practice between different centers. Patterns of healing and long-term outcomes are also very different for children compared to adults. In order to develop effective protocols for management, it is first necessary to understand current trends in presentation and treatment. METHODS: We cond

Comparing Screening Tools for Predicting Phoenix Criteria Sepsis and Septic Shock Among Children.

BACKGROUND AND OBJECTIVES: The Phoenix criteria for pediatric sepsis and septic shock have recently been proposed for worldwide application. The Phoenix sepsis criteria are based on organ dysfunction scoring. Although many screening tools exist, their performance in predicting Phoenix outcomes is not known. We hypothesized that the quick Pediatric Septic Shock Screening Score (qPS4) would demonstrate greater sensitivity compared with the Liverpool quick Sequential Organ Failure Assessment (LqSOFA) and a commonly used 2-stage screening tool created at Children's Hospital of Philadelphia (CHOP).
