Serum hypoalbuminemia is an independent prognostic factor in Chronic Myelomonocytic Leukemia (CMML).

CMML is a heterogenous myelodysplastic/myeloproliferative neoplasm (MDS/MPN) sharing both diseases' molecular and clinical phenotypes. Several models are used to risk-stratify patients diagnosed with CMML. Inflammation plays a pivotal role in developing the disease or its progression and has been linked to worse outcomes. Serum albumin (SA) is an inflammatory marker and/or surrogate for co-morbidities. While the role of SA has been investigated in myelodysplastic syndrome (MDS), acute myeloid leukemia (AML), multiple myeloma, and other cancers, its prognostic value in CMML remains unclear. We

Subperiosteal abscess of the orbit with multiple abscesses in the brain and body due to Chromobacterium violaceum sepsis in an infant: a case report and review of the literature.

Chromobacterium violaceum is a rare pathogen known to cause severe, life-threatening infections in humans. Its typical clinical presentations include skin and soft tissue infections, fulminant septicemia, multiple visceral abscesses, and diarrhea. We report the case of a 2-month-old male infant who initially presented with acute fever and subsequently developed acute proptosis of the right eye, accompanied by a palpable mass in the right mastoid area. Computed tomography (CT) scans of the orbits, paranasal sinuses and brain revealed a subperiosteal abscess at the right sphenoid wing, right oto

Sudden Diplopia Two Months After a Blowout Fracture: A Case Report of Orbital Tissue Adhesion Associated With Acute Sinusitis.

Blowout fractures (BOFs) frequently result in early-onset diplopia, which often resolves spontaneously. Delayed diplopia is rare, and there are no reported cases of its sudden onset following a prolonged asymptomatic period. Persistent diplopia significantly impacts quality of life. BOF-induced late-onset diplopia is rare. To date, no cases of late-onset or acute-onset diplopia have been reported, underscoring its exceptional rarity. This report discusses a unique case of sudden diplopia two months post BOF, which improved after surgical treatment. A 57-year-old man presented with a history of

Bilateral orbital inflammation in a pediatric patient with eosinophilic granulomatosis with polyangiitis.

A 5-year-old female presented to the ophthalmology clinic with one month of left upper eyelid ptosis and three months of intermittent wheezing and coughing. MRI of the brain and orbits revealed infiltrative enhancement involving both orbits, including the retro-maxillary space, maxillary sinus walls, greater wing of the sphenoid, and possibly the left cavernous sinus. She experienced acute respiratory decompensation in the setting of pansinusitis and tree-in-bud nodular pulmonary opacifications. Laboratory workup revealed anemia, thrombocytosis, and elevated inflammatory markers. An orbitotomy

Outcomes of inflammatory markers in the diagnosis of complicated rhinosinusitis in children.

PURPOSE: Acute rhinosinusitis (ARS) is a common pediatric disorder. When complications occur, it can lead to various morbidities. Therefore, easily accessible and inexpensive markers are needed to predict the development of complications, treatment, and clinical course of acute rhinosinusitis. We aimed to investigate the predictive ability of the various inflammatory markers in pediatric complicated ARS patients. METHODS: The data of 79 pediatric patients with complicated ARS and 116 pediatric patients with uncomplicated ARS were analyzed retrospectively. The complete blood count and C-reacti

A Rare Complication of Sequential Bilateral Posterior Knee Dislocation Following Bilateral Robotic Assisted Primary Knee Replacement: Case Report.

INTRODUCTION: The International Headache Society divides headaches into primary (vascular and muscular) and secondary (inflammatory). Secondary headaches include headaches secondary to inflammation caused by injuries to the head and neck. Cervicogenic headache (CGH) is thought to be referred pain arising from irritation caused by cervical structures innervated by spinal nerves C1, C2, and C3; therefore, any structure innervated by the C1-C3 spinal nerves could be the source of a CGH. Pain from the lateral atlanto-axial joint (AAJ) is one of several causes of CGH. The pain is perceived in the u

Posterior scleritis mimicking orbital cellulitis in an early adolescent patient.

Posterior scleritis is a rare but potentially sight-threatening condition that commonly causes diagnostic confusion due to its variable and perplexing clinical symptoms and signs. We present a case of posterior scleritis presenting with signs of acute orbital inflammation mimicking orbital cellulitis in an early adolescent female. However, subsequent medical imaging revealed signs of posterior vitritis, serous retinal detachment and scleral thickening. Considering the absence of an infective source and the imaging results, the diagnosis of posterior scleritis was confirmed.

Treatment of sinusitis in children: an Italian intersociety consensus (SIPPS-SIP-SITIP-FIMP-SIAIP-SIMRI-SIM-FIMMG).

Sinusitis is an inflammation of the mucous membrane of the paranasal sinuses. Bacterial sinusitis usually occurs as a complication of viral infections of the upper respiratory tract and is a frequent cause of medical consultation. The clinical presentation of acute bacterial sinusitis can vary. It most commonly manifests as an upper respiratory tract infection (nasal congestion, postnasal drip, cough) that persists for more than 10 days without clinical improvement. Unfortunately, updated guidelines in paediatric age are not currently available. The purpose of this consensus is to provide guid

Consensus disease definitions for ophthalmic immune-related adverse events of immune checkpoint inhibitors.

Ophthalmic immune-related adverse events (Eye-irAEs) from immune checkpoint inhibitors can cause visual morbidity. The absence of standardized definitions for Eye-irAEs not only impedes the development of evidence-based treatments but also progress in translational research. The objective of this study was to develop consensus guidance for an approach to Eye-irAEs.Four ophthalmic physicians (uveitis specialists and neuro-ophthalmologists) drafted Eye-irAE consensus guidance and definitions, which were reviewed by the multidisciplinary Eye-irAE definition panel. The panel was divided into Group

Paracentral Acute Middle Maculopathy in a Pregnant Woman on Supplemental Hormones for Invitro-Fertilization.

This case study examines the rare occurrence of paracentral acute middle maculopathy (PAMM) in a first-trimester pregnant patient, highlighting the importance of promptly addressing acute neurological symptoms in expectant mothers. PAMM, a retinal vascular disorder causing ischemic damage to the middle retinal layers, is sparsely documented in literature. The patient, a 31-year-old primigravida with migraines and elevated anti-nucleic acid titer, presented with sudden painless monocular vision loss. Initially suspected as a migraine aura, persistent symptoms led to a comprehensive evaluation.

Management of orbital complications of acute rhinosinusitis: A 13-year experience.

INTRODUCTION: After the development of antibiotics and early diagnostic methods, the complications of acute rhinosinusitis (ARS) have reduced in recent years. However, in clinical practice, we still observe both orbital and intracranial complications. The aim of this study is to describe a series of patients with complications of ARS and to define what clinical and radiological characteristics may indicate the need of surgical treatment. MATERIALS: Retrospective observational study of patients diagnosed with orbital complications secondary to ARS who were hospitalized at a tertiary hospital b

Tolosa-Hunt Syndrome: A Case Report.

Tolosa-Hunt syndrome (THS) is a rare neurological condition characterized by ophthalmoplegia preceded by retroorbital pain or headache. It mainly involves cranial nerves III, IV, and VI. The majority of the time, the cause is unknown; it is believed that the cause is nonspecific granulomatous inflammation of the cavernous sinus and superior orbital fissure or rarely beyond. THS is an exclusionary diagnosis. It is diagnosed clinically according to the patient's symptoms, neuroimaging test results, and response to corticosteroid therapy. The mainstay of treatment is corticosteroid therapy. A 62-
