IL-23 drives uveitis by acting on a population of tissue-resident entheseal T cells.

Recurrent acute anterior uveitis is a frequent extra-articular manifestation of the axial spondyloarthropathies (AxSpA): chronic inflammatory diseases affecting the spine, enthesis, peripheral joints, skin, and gastrointestinal tract. Pathology in AxSpA has been associated with local tissue-resident populations of IL-23 responsive lymphoid cells. Here we characterize a population of ocular T cell defined by CD3+CD4-CD8-CD69+γδTCR+IL-23R+ that reside within the anterior uvea as an ocular entheseal analogue of the mouse eye. Localized cytokine expression demonstrates that uveal IL-23R+ IL-17A-pr

Influence of Dental Intervention and Disease on Acute Invasive Fungal Rhinosinusitis Severity and Outcomes.

BackgroundAcute invasive fungal rhinosinusitis (AIFRS) is an aggressive and often fatal disease process that principally impacts immunocompromised patients. Maxillary dental trauma and infections have been associated with the development of maxillary sinus fungal balls, but the role of dental procedures/trauma in the pathogenesis of AIFRS remains poorly defined.ObjectiveThis study seeks to review a single-institutional experience with AIFRS and examine the association between dental events and AIFRS severity and outcomes.MethodsRetrospective review of 95 consecutive patients with biopsy-proven

Complex Ocular Adverse Events Following Oxaliplatin Treatment: A Case Report.

BACKGROUND Ocular adverse events related to systemic chemotherapy are relatively rare and often underrecognized in clinical practice. We describe a unique case of severe ocular toxicity associated with oxaliplatin-based chemotherapy, presenting as multiple iris cysts, anterior chamber inflammation, retinal detachment, and lens opacity, ultimately resulting in significant vision loss. CASE REPORT A 47-year-old woman with adenocarcinoma of the esophagogastric junction underwent laparoscopic radical gastrectomy on March 4, 2024. Postoperatively, she was started on adjuvant oxaliplatin-based chemo

Immune-mediated extraocular myositis in an adolescent following coronavirus disease 2019: A case report.

A wide spectrum of ocular manifestations have been documented in patients following infection with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). Nevertheless, bilateral extraocular myositis specifically associated with natural SARS-CoV-2 infection has been rarely reported. We report the case of an East Asian adolescent female who presented with right ocular discomfort and diminished visual acuity for 2 weeks. A SARS-CoV-2 infection had been confirmed via reverse transcriptase-polymerase chain reaction 10 days ago. Magnetic resonance imaging of the orbits revealed bilateral eyel

Invasive Fungal Granuloma of the Skull Base Mimicking Spindle Cell Neoplasm in a Young Immunocompetent Patient: A Diagnostic Challenge.

Skull base lesions presenting with proptosis and sinonasal involvement in young patients are commonly attributed to vascular neoplasms such as juvenile nasopharyngeal angiofibroma. However, rare infectious etiologies such as invasive fungal granulomas may present with overlapping radiological features, leading to diagnostic challenges. We report a rare case of a 19-year-old immunocompetent male presenting with progressive bilateral proptosis and a large skull base mass involving the clivus, nasal cavity, and orbits. Initial imaging suggested a vascular neoplasm, and biopsy revealed a spindle c

Occult medial orbital wall trapdoor fracture inducing recurrent rhinogenic intraorbital abscess in a pediatric patient: a case report.

BACKGROUND: In pediatric populations, rhinogenic intraorbital abscess is frequently encountered, typically resulting from the direct extension of acute sinusitis. However, recurrent rhinogenic intraorbital abscess secondary to an occult medial orbital wall trapdoor fracture is extremely rare and often overlooked due to atypical trauma history and subtle imaging features. CASE PRESENTATION: An 11-year-old male pediatric patient experienced recurrent nasal congestion and rhinorrhea with redness, swelling, and pain of the left eye. He was first diagnosed with acute sinusitis, yet conventional an

Metastatic melanoma to the orbit masquerading as idiopathic orbital inflammation: a case report.

This case highlights an exceedingly rare presentation of secondary orbital melanoma and reviews the current literature on orbital melanoma. Case presentation: A 39-year-old man was referred to our clinic after acute-onset decreased vision and left upper eyelid ptosis. Outside-hospital magnetic resonance imaging (MRI) showed an enhancing left superior orbital apex mass. The patient was initially treated for presumed idiopathic orbital inflammation without improvement. An orbital biopsy was performed, and the pathology showed secondary malignant melanoma. A computed tomography (CT) chest scan sh

Unusual Presentation of Orbital Cellulitis as an Inflammatory Pseudotumor in a Kidney Transplant Recipient.

We reported the unusual presentation of a 62-year-old kidney transplant recipient with acute unilateral eye swelling and fever suggestive of severe orbital cellulitis with features of a space-occupying inflammatory lesion of the orbit. The clinical course deteriorated despite the therapy with broad-spectrum antibiotics alone. The computed tomography scan findings were significant for moderate thickening of the right superior rectus muscle and asymmetric enlargement of the superior ophthalmic vein. These features brought an orbital pseudotumor, or nonspecific orbital inflammation, to the differ

Ocular injuries and the most common acute conditions in ophthalmology.

Ocular traumas include a wide range of injuries from trivial conditions to extensive perforating injuries that can lead to visual function impairment of various ranges and even to the loss of the eye itself. In terms of first aid, not only an ophthalmologist but also a physician of any specialty can effectively intervene and minimize the consequences of these conditions. Open globe injuries represent prognostically the most severe conditions which have to be treated by an experienced ophthalmologist. Closed injuries are most commonly manifested as corneal erosions, foreign bodies on the surfac

Acute Orbital Inflammatory Syndrome and Vision Loss After Aminobisphosphonate Infusion.

BACKGROUND: Aminobisphosphonates (AP) are used in the management of osteoporosis and Paget's disease because of their action as an inhibitor of bone resorption. Systemic and orbital inflammatory syndromes have been reported after infusion with AP medications, most commonly after intravenous (IV) zoledronate. CASE PRESENTATION: A 62-year-old male received COVID-19 and influenza vaccinations followed two days later by IV zoledronate infusion. The next day he developed flu-like symptoms followed two days later by right eye pain. An MRI showed right orbital enhancement prompting a short course of

Acute Macular Neuroretinopathy in Association with Orbital Inflammation and Optic Perineuritis.

A 17-year-old girl experienced acute onset of decreased vision with pain on movement in her left eye. Her best-corrected visual acuity was 20/20 in the right eye and 20/100 in the left eye. Pupillary examination revealed a positive relative afferent pupillary defect. Fundus examination showed optic disc hyperemia and retinal folds. A central scotoma was detected on a perimetry test. Fluorescein angiography indicated mild disc leakage in the late phase. Orbital magnetic resonance imaging disclosed enhancement in the soft tissue of the left retrobulbar area and at the sheath of the left optic ne

[A Case of Fungal Sinusitis and Preseptal Cellulitis Caused by Lichtheimia corymbifera in a Patient with Acute Myeloid Leukemia].

Lichtheimia corymbifera (formerly known as Absidia corymbifera) is a mold fungus belonging to the family Mucorales. Mucormycosis caused by L.corymbifera is a newly recognized and rare opportunistic infection in immunocompromised patients. Rapid diagnosis and treatment are crucial due to the high mortality rates, especially in patients with hematologic malignancies. Early clinical suspicion, surgical removal of necrotic tissue and appropriate antifungal chemotherapy constitute the main steps of treatment. Lipid formulations of Amphotericin B are the first-line treatment for mucormycosis. In thi

Orbital apex syndrome secondary to Sweet syndrome.

Sweet syndrome (SS), or acute febrile neutrophilic dermatosis, is a dermatologic, auto-inflammatory disorder of unclear origin, often accompanied by systemic inflammation affecting various tissues, including the eyes. Common ocular manifestations include conjunctivitis but can extend to other ocular tissues. Orbital apex syndrome (OAS) involves damage to several cranial nerves transversing the orbital apex, leading to ophthalmoplegia and vision loss. This case report describes a rare case of OAS secondary to SS. A woman in her 50s with a history of SS presented with right eye loss of vision an

Herpes Zoster Ophthalmicus With Orbital Inflammation and Viral Meningitis in an Immunocompetent Patient.

Common ocular manifestations of Herpes zoster ophthalmicus (HZO) include conjunctivitis, keratitis, anterior uveitis and ocular hypertension. Immunosuppressed patients are at an increased risk of sequelae including orbital inflammation and meningitis. We present a case of a 47-year-old immunocompetent patient who presented with acute orbital inflammation and viral meningitis, on a background of recent HZO without ocular involvement. Magnetic resonance imaging (MRI) demonstrated enhancement and enlargement of the left extraocular muscles, lacrimal gland and optic nerve sheath and enhancement of

Impact of the Minimization of Standard Deviation Before Zeroization of the Mean Bias on the Performance of IOL Power Formulas.

PURPOSE: In cataract surgery, accurate intraocular lens (IOL) power calculations are crucial for optimal postoperative refractive outcomes. This study explores the impact of prioritizing the reduction of the standard deviation (SD) of prediction errors before mean prediction error (PE) adjustment on IOL calculation formula precision and accuracy. METHODS: We conducted a retrospective analysis of 4885 eyes from 2611 patients, all implanted with the same IOL model, comparing four traditional IOL power calculation formulas: SRK/T, Holladay 1, Haigis, and Hoffer Q. We introduced new constants aim
