Chronic optic neuropathy: correlating MR imaging features and etiology.

BACKGROUND AND PURPOSE: Chronic optic neuropathy (CON) represents a final common pathway of sustained optic nerve injury from diverse etiologies, yet characteristic MRI features and their clinical correlations are not well defined. This study evaluated the predominant MR imaging findings of non-compressive CON and correlated these patterns with underlying clinical etiologies. MATERIALS AND METHODS: We retrospectively identified 186 patients with MRI reports referencing chronic optic neuritis or neuropathy from 2009 to 2024. Of these, 162 met inclusion criteria, defined as high-resolution orbi

The causal effects between autoimmune thyroid disease and urticaria: evidence from a Mendelian randomization study.

Previous studies have suggested a comorbidity between autoimmune thyroid disease (AITD) and urticaria. However, the causal effects between AITD and urticaria remain unclear. This study employed genome-wide association study (GWAS) data to analyze the associations between AITD and urticaria through bidirectional Mendelian randomization (MR), meanwhile, examined the role of 731 immune traits as mediators between the two diseases through mediation analysis. The IVW results indicated that Graves' disease (GD) and Hashimoto's thyroiditis (HT) have a potential risk effect on urticaria, particularly

Case Report: Methimazole-Induced Parotitis - An Unusual Presentation.

A 56-year-old female with a medical history of unspecified hyperthyroidism and a recent thyroid storm presented to the ED with tachycardia, hypertension, and bilateral enlarged parotid glands. During a previous hospitalization, she was diagnosed with unspecified hyperthyroidism and started on methimazole. During hospitalization, laboratory findings suggested Graves' disease with an acute thyroid storm. The patient also complained of enlarged parotid glands bilaterally. CT tomography of the neck revealed no calculi of the parotid glands but showed extensive fatty replacement, possibly related t

Validity of a thyrotoxicosis diagnosis code among women of fertile age.

INTRODUCTION: Graves' disease (GD) is the leading cause of hyperthyroidism in women of fertile age. Register-based studies rely on diagnostic codes; this study examined the validity of diagnosed thyrotoxicosis among women of fertile age in the Danish National Hospital Register (DNHR). METHODS: All women in the North Denmark Region aged 18-45 years with a diagnosis of thyrotoxicosis (Tenth International Classification of Diseases: E05.0-05.9) in the DNHR from 2017-2018 were identified and medical records reviewed. RESULTS: Among 263 women included, thyrotoxicosis was confirmed in 249 cases wi

The risk of thyroid diseases in lichen sclerosus patients: a systematic review and meta-analysis.

BACKGROUND/OBJECTIVE: Lichen sclerosus (LS) is a chronic inflammatory skin disease affecting both genital and extra-genital areas and is found predominantly in female patients. The relationship between LS and thyroid diseases remains unclear, with conflicting evidence reported in the literature. This systematic review and meta-analysis aimed to investigate the association between LS and thyroid diseases rigorously. MATERIALS AND METHODS: We conducted a comprehensive search using two electronic databases, MEDLINE and Embase, from inception to May 2024, to identify observational studies investi

Rare Aneurysmal Bone Cyst Presentation in the Orbit: A Systematic Review of the Literature with an Illustrative Case Report.

OBJECTIVE: Aneurysmal bone cysts (ABCs) are benign but aggressive vascular lesions within bone. Orbital ABCs, though rare, are clinically significant due to the risk of debilitating symptoms such as vision loss. METHODS: Our systematic review was conducted according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines using PubMed, Google Scholar, Embase, and SCOPUS databases to identify and assess primary articles exploring orbital ABC cases. Additionally, we present an illustrative case report. RESULTS: Among the 54 cases from the 51 studies included, the me

Thyrotropin Receptor Autoantibody Assessment in Thyroid Eye Disease: Does the Assay Type Matter?

PURPOSE: Thyroid receptor antibodies can quantify thyroid eye disease activity, predict outcomes and aid timing of interventions. The type and generation of assay is frequently unspecified, complicating meta-analyses. To determine the clinical and biochemical relationships between a second-generation thyrotropin receptor-binding inhibition antibody (TRAb) immunoassay, detecting stimulatory and blocking antibodies, with the thyroid stimulating immunoglobulin (TSI) bridging immunoassay detecting the stimulatory component only. METHODS: Retrospective review of 100 consecutive patients attending

18F-FDG PET/CT versus conventional investigations for cancer screening in autoimmune inflammatory myopathy in the era of novel myopathy classifications.

BACKGROUND: To compare the performance of fluorine-18-fluorodeoxyglucose (F-FDG) PET/computed tomography (CT) and conventional tests for cancer screening in autoimmune inflammatory myopathy (AIM) patients. PATIENTS AND METHODS: We carried out a retrospective cohort study of AIM patients from one academic center in Montreal, Canada, classified using myositis-specific antibodies, who underwent F-FDG PET/CT between April 2005 and February 2018 and were followed up on average 3.5±2.4 years. Patients were excluded if follow-up was insufficient, AIM diagnosis was indeterminate, and/or malignancy wa

Invasive mucormycosis in children: an epidemiologic study in European and non-European countries based on two registries.

BACKGROUND: Mucormycosis has emerged as a rare but frequently fatal invasive fungal disease. Current knowledge on paediatric mucormycosis is based on case reports and small series reported over several decades. Contemporary data on a large cohort of patients is lacking. METHODS: Two large international registries (Zygomyco.net and FungiScope™) were searched for mucormycosis cases in ≤19 year-old patients. Cases enrolled between 2005 and 2014 were extracted, and dual entries in the two databases merged. Epidemiology, clinical characteristics, diagnostic procedures, therapeutic management and f

Fluctuation-driven magnetic hard-axis ordering in metallic ferromagnets.

We demonstrate that the interplay between soft electronic particle-hole fluctuations and magnetic anisotropies can drive ferromagnetic moments to point along a magnetic hard axis. As a proof of concept, we show this behavior explicitly for a generic two-band model with local Coulomb and Hund's interactions and a spin-orbit-induced easy plane anisotropy. The phase diagram is calculated within the fermionic quantum order-by-disorder approach, which is based on a self-consistent free-energy expansion around a magnetically ordered state with unspecified orientation. Quantum fluctuations render the

What is the best definitive treatment for Graves' disease? A systematic review of the existing literature.

BACKGROUND: The management guidelines of the American Thyroid Association and American Association of Clinical Endocrinologists for Graves' disease (GD) include any of the following modalities: (131)I therapy, antithyroid medication, or thyroidectomy. No in-depth analysis has been performed comparing the treatment options, even though a single treatment option seems to be universally accepted. METHODS: A systematic review of the literature was performed to examine contemporary literature between 2001 and 2011 evaluating the management options of GD. We compiled retrospective and prospective s

Autoimmune thyroid encephalopathy presenting with epilepsia partialis continua.

We report the first case of an autoimmune thyroid encephalopathy presenting with multifocal motor status epilepticus. A 37-year-old female patient was admitted with multifocal motor seizures intractable to intravenous status epilepticus treatments, asymmetrical quadriparesis, truncal ataxia and continuous semi-rhythmical jerks. Pathological signal alterations were detected in both precentral cortices in MRI examination. Autoimmune thyroiditis was diagnosed after radiological examinations of the thyroid gland and thyroid function tests. Seizures promptly ceased following intravenous steroid tre

Primary liposarcoma of the orbit: a clinicopathologic study of seven cases.

Liposarcoma, the most common soft tissue sarcoma in adults, will rarely involve the orbit, either primarily or as a metastasis. We describe seven primary orbital liposarcomas, representing the largest documented series of primary orbital liposarcoma to date. Affected patients were three males and four females ranging in age from 28 to 69 years (median, 51 years). Five patients presented with painless proptosis, one patient had painful proptosis, and no details of presenting symptoms are available in one case. The site distribution was retrobulbar (3 cases), lateral orbital wall (2 cases), medi

Blindness after reduction of facial fractures.

Blindness in patients suffering maxillofacial trauma is usually caused by optic nerve or optic canal injuries. It is, however, an uncommon complication of facial trauma, with a reported incidence of only 2 to 5 percent. Blindness may also follow surgical repair of facial fractures. Many mechanisms, such as intraoperative direct nerve injury, retinal arteriolar occlusion associated with orbital edema, or delayed presentation of indirect optic nerve injury sustained at the time of the initial trauma, have been implicated in causing this blindness. In this article, four cases of visual loss after

Sinus histiocytosis with massive lymphadenopathy (Rosai-Dorfman disease).

Sinus histiocytosis with massive lymphadenopathy (Rosai-Dorfman disease) is a unique disease of unknown etiology with a childhood predilection. Reports have established the worldwide distribution of the disorder. Although peripheral lymphadenopathy is the most common mode of presentation, numerous studies and individual case reports have established the fact that 30-40% of affected individuals have extranodal manifestations particularly in the head and neck region where the adenopathy tends to be concentrated. As yet, the pathogenesis of SHML has not been established but speculation relates th

Immunogenetics of autoimmune thyroid disorders.

The loci determining polymorphism of genes involved in the immune response feature prominently in the susceptibility to autoimmune thyroid disease. However, disease phenotypes do not segregate perfectly with the genetic markers. The interaction between the influence of these loci on susceptibility is complex, and when taken in the context of the multilayered, multidirectional workings of the immune network and that of as-yet-unspecified environmental factors, our difficulty in predicting the susceptible phenotypes is not surprising.

Embolic stroke in a woman with mitral valve prolapse who used oral contraceptives.

A case of angiographically-documented embolism is presented in a patient using oral contraceptives (OC) with marked mitral valve prolapse (MVP) and an atrial thrombus. OC use has been shown to decrease levels of antithrombin III and increase platelet coagulant activity. This increased coagulability may increase the risk of intra-atrial thrombus formation and subsequent cerebral embolism in patients with MVP. We believe that MVP, especially when redundant valve leaflets are recognized, may be a relative contraindication to OC use.
