Thyroid eye disease (TED) is an autoimmune inflammatory orbitopathy that can threaten vision in severe cases due to dysthyroid optic neuropathy (DON). While TED is primarily diagnosed clinically, orbital imaging with computed tomography (CT) and magnetic resonance imaging (MRI) plays a pivotal role in confirming the diagnosis in atypical presentations, distinguishing TED from other orbital diseases, assessing disease activity, and guiding management. This narrative review examines the role of CT and MRI in the diagnosis and management of TED, with particular focus on their application in diffe

BACKGROUND: Orbital decompression is an effective surgery for reducing orbital congestion and proptosis for patients with thyroid eye disease (TED). However, it is commonly stated that elective orbital decompression, i.e., for reasons other than compressive optic neuropathy, should await the quiescent phase of the disease. The reason is to avoid inducing a flare-up of the orbitopathy secondary to surgical inflammation. Still, this means that patients with moderate to severe TED who are suffering from symptoms associated with TED must wait months to years before initiating surgical treatment. T

BackgroundDysthyroid optic neuropathy (DON) is a vision-threatening complication of thyroid-associated ophthalmopathy that may require urgent surgical intervention. We describe a microsurgical two-wall orbital decompression technique designed to achieve effective orbital and optic nerve decompression in extremely severe cases as an additional option.MethodsA frontotemporal extradural approach is used to remove the lateral orbital wall and orbital roof, with optional decompression of the optic canal. Neuronavigation guides bone removal and helps avoid violation of the frontal sinus. The surgica

Thyroid eye disease (TED) is an autoimmune disorder commonly associated with hyperthyroidism of Graves' disease. The global outbreak of SARS-CoV-2 resulted in an unparalleled public health emergency. The rapid development and widespread administration of COVID-19 vaccines were instrumental in controlling viral transmission and decreasing severe disease outcomes and deaths. Although these vaccines have demonstrated strong safety and effective prevention, isolated cases of uncommon adverse events have been reported. Notably, instances of new-onset or worsening autoimmune disorders, including TED

BACKGROUND: Collision medullary and papillary thyroid carcinoma (MTC/PTC) is a rare entity, constituting less than 1% of all thyroid malignancies. The concurrent presence of these malignancies in patients with autoimmune thyroid disease, such as Graves' disease, is even more uncommon. Calcitonin (Ctn) is considered one of the key MTC biomarkers. Mixed tumors may alter this relationship.
CASE DESCRIPTION: We report the case of a 55-year-old female with a history of Graves' disease, who underwent total thyroidectomy for persistent dysthyroid orbitopathy. Histopathological analysis revealed a 9-

Dysthyroid optic neuropathy (DON) is the most severe complication of thyroid-associated ophthalmopathy (TAO). Although recent evidence indicates that reduced retinal capillary density (RCD) may increase DON risk independently of orbital apex crowding, the underlying mechanisms and associated metabolic reprogramming remain unclear. In a retrospective analysis of TAO patients with and without DON, those with DON demonstrated elevated pulse pressure (PP), decreased RCD, and higher incidences of dyslipidemia, hyperglycemia, and internal carotid artery calcification. To explore the metabolic basis

Thyroid eye disease is an immune-mediated orbital disorder increasingly encountered by neurologists, presenting with optic neuropathy, diplopia, and headache. This review offers a practical, treatment-focused framework, emphasizing staging activity and severity with tools like the Clinical Activity Score and EUGOGO categories to guide management. Key steps include restoring euthyroidism, smoking cessation, and structured orbital examinations. Urgent cases with vision loss require high-dose intravenous (IV) glucocorticoids and early orbital decompression. In active disease, IV methylprednisolon

BACKGROUND: High-dose intravenous methylprednisolone (HD-IVMP) is the first-line treatment for dysthyroid optic neuropathy (DON). However, a significant proportion of patients exhibit a poor response, risking irreversible vision loss when surgical decompression is delayed. Identifying reliable predictors for failure of HD-IVMP is crucial for optimizing treatment strategies. This retrospective study aimed to identify risk factors associated with a poor response to HD-IVMP in DON patients, develop a predictive model for clinical decision-making, and assist in determining the optimal timing of or

Graves' orbitopathy (GO) is a progressive autoimmune disorder of visual organ, mostly associated with Graves' disease (GD). Although rituximab has been used as an off-label therapy for steroid-resistant GO, paradoxical immune reactivation has also been reported. We present a rare case of GO that developed after bendamustine-rituximab (BR) therapy for chronic lymphocytic leukemia (CLL). A 73-year-old male with a 3-year history of GD, presented with new onset of bilateral proptosis, diplopia, and decreased visual acuity, that happened two weeks after his second BR cycle for CLL.
