BACKGROUND: VEXAS (vacuoles, E1 enzyme, X-linked, autoinflammatory, somatic) syndrome is a newly recognized adult-onset autoinflammatory disorder caused by somatic mutations in the UBA1 gene. It typically presents in older males with systemic inflammation, hematologic abnormalities, and a relapsing, treatment-refractory course. Reports in younger patients remain rare, and pulmonary involvement is often misdiagnosed as infection.
CASE PRESENTATION: We present a 30-year-old man with a two-year history of recurrent scleritis, fever, cough, auricular chondritis, rash, and transfusion-dependent ma

Peripheral ulcerative keratitis (PUK) is a rare and severe ocular manifestation frequently associated with systemic autoimmune diseases. This retrospective two-center observational study including patients with PUK and a systemic autoimmune disease at two tertiary referral centres aimed to characterize clinical and histopathologic features, treatment strategies, relapses, and ocular outcomes of autoimmune-associated PUK, and to analyse its relationship with systemic disease activity. A total of 24 patients (35 eyes) were identified, 62.5 % female, with a median age of 58.5 years and a mean fol

We report the case of a 17-year-old girl with history of anaplastic large-cell lymphoma on crizotinib therapy who presented with pain and blurred vision in the left eye. Examination demonstrated nodular non-necrotizing anterior scleritis in the left eye. The patient initially improved with oral glucocorticoid treatment until she developed nodular non-necrotizing anterior scleritis in the right eye during taper of this medication. Discontinuation of crizotinib allowed the patient to taper off glucocorticoid treatment, with subsequent resolution of scleritis.

BACKGROUND: Surgically induced necrotizing scleritis (SINS) is a blinding ocular disease characterized byinfl ammation with scleral necrosis. It occurs as early as one day after ocular surgery, although it can also occur years later. Commonly, it occurs after pterygium and cataract surgeries CASE PRESENTATION: Here, we report a case of a 40-year-old woman with underlying diabetes mellitus whounderwent a successful excisional biopsy via a swinging upper eyelid approach with lateral canthotomy andcantholysis, along with disinsertion of the superior and lateral rectus muscles, for an orbital cave

BACKGROUND: Variants in RELA (which encodes the p65 subunit of NF-κB) can cause a monogenic autoinflammatory disease with clinical manifestations that range from mucocutaneous lesions (Behçet's disease-like) to systemic inflammation. However, the diversity of the phenotype and its penetrance are uncertain.
CASE PRESENTATION: Patient 1 (p.Asp465Thrfs14) presented with classic Behçet's disease-like symptoms of oral and genital ulcers, fever, and elevated inflammatory markers. Colchicine (0.25 mg once daily) with low-dose glucocorticoids led to remission. Exploratory in vitro assays using murine

PURPOSE: To discuss the recent evidence that Janus kinase (JAK) inhibitors can cause viral retinitis. METHODS: A commentary on the case report in this issue and a review of cases in the literature. RESULTS: A total of 6 cases have now been reported of cytomegalovirus retinitis associated with Jak inhibitors. Additionally, there was a case of bilateral toxoplasma retinitis and a case of herpetic scleritis. DISCUSSION: It is important to be aware of the potential blinding complications associated with JAK inhibitors and if the patient complains of acute vision loss and/or floaters, the patient s

BACKGROUND: Necrotizing scleritis is a rare but severe ocular inflammatory disorder, frequently associated with systemic autoimmune diseases such as rheumatoid arthritis (RA). Although biologic agents targeting cytokines such as TNF-α and IL-6 have improved disease outcomes, some patients remain refractory to these treatments. We describe a case of necrotizing scleritis resistant to multiple biologic agents that responded favorably to the Janus kinase (JAK) inhibitor upadacitinib. CASE PRESENTATION: A 43-year-old woman with rheumatoid arthritis presented with bilateral conjunctival injection a

The objective of this article is to describe an unusual presentation of myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) with posterior scleritis and optic neuritis. We report the case of a 50-year-old woman who presented with acute visual deterioration, headache, and painful eye movements. Clinical examination revealed profound vision loss, optic disc edema, and posterior scleritis confirmed via B-scan ultrasound. Magnetic resonance imaging demonstrated optic nerve involvement with perineuritis, and myelin oligodendrocyte glycoprotein antibodies were detected. The patie

Noninfectious anterior scleritis is a painful, potentially vision-threatening inflammatory disorder that may become steroid dependent despite negative systemic evaluation, posing significant management challenges. We report a man in his early sixties with an approximately eight-year history of recurrent unilateral diffuse anterior scleritis who was referred for rheumatology evaluation after infectious etiologies were excluded by ophthalmology. His disease repeatedly improved with high-dose systemic corticosteroids but relapsed during tapering, resulting in prolonged steroid dependence. Extensi

BACKGROUND: Scleritis is a severe, painful inflammatory condition affecting the sclera. If untreated, scleritis can lead to significant ocular complications. Corticosteroids are often considered for the initial treatment of non-infectious scleritis. However, for steroid-resistant cases, cases requiring prolonged therapy, or to reduce steroid dependence, immunosuppressive agents, including methotrexate (MTX), are employed.
OBJECTIVES: This study compared the efficacy and safety of MTX versus steroids and placebo in treating scleritis.
METHODS: A comprehensive literature search was conducted v

The authors describe the case of a right eye sub-conjunctival lesion, initially misdiagnosed and treated as anterior scleritis, in a female patient in her early twenties. The lesion showed nearly complete clinical resolution on initial treatment with oral steroids. The lesion recurred, however, after discontinuing oral steroids, and at the subsequent visit, the lesion was more widespread and elevated. Anterior-segment optical coherence tomography showed a sub-conjunctival lesion, and CT scan of the orbits showed an isodense periocular lesion moulding around the globe. Crush artefacts prevented

Non-infectious scleritis is a potentially vision threatening inflammatory disease which often requires long-term treatment. The general approach to management consists of a thorough systemic workup and subsequent selection of the most appropriate therapeutic agent(s) for inflammatory control. This article reviews current and emerging therapies for scleritis treatment, including topical agents, systemic non-steroidal anti-inflammatory drugs and corticosteroids, conventional immunomodulatory therapies, and biologics.

To evaluate the clinical utility of Ultrasound Biomicroscopy (UBM) in diagnosing and managing ocular injuries caused by chestnut-burr spines, particularly for detecting non-metallic scleral foreign bodies and monitoring postoperative inflammatory progression. A retrospective study was conducted on 36 patients (36 eyes) presenting with non-penetrating ocular trauma from chestnut-burr spines. All patients underwent preoperative UBM for foreign body localization and assessment of scleral inflammation. The UBM data obtained at 1 week, 1 month, and 3 months postoperatively were analyzed to assess t

The sub-Tenon route for injecting triamcinolone acetonide is one of the widely practiced surgical techniques for postoperative inflammation control, but complicated conjunctival and scleral necrosis are rare occurrences. A 3-year-old boy underwent bilateral cataract surgery and an anterior sub-tendon injection of triamcinolone (AST) (20 mg). One week later, he presented with conjunctival necrosis at the injection site and thinning of the sclera in both eyes. Necroses being unresponsive to conservative management led to surgical debridement and removal of residual steroid deposits. The conjunct

INTRODUCTION: Scleritis is a rare and severe ocular inflammatory disease that is often associated with potentially sight-threatening ocular complications. The aim of the present study was to characterize ocular complications in non-infectious scleritis and identify predictive variables for their development.
METHODS: Data for this registry-based study were extracted from the AutoInflammatory Disease Alliance Network for Scleritis Registry. Univariate analysis was performed to examine potential associations of demographic and clinical variables with the development of ocular complications. Uve

We report the first documented case of isolated episcleral ALK-positive histiocytosis (ALK+H) in an adult. A 36-year-old woman presented with an episcleral nodule clinically mimicking nodular scleritis. Excisional biopsy revealed sheets of foamy CD68+/CD163+ histiocytes with Touton-type giant cells. Immunohistochemistry demonstrated strong cytoplasmic ALK1 expression with negative CD1a and BRAF V600E, and fluorescence in situ hybridization confirmed ALK gene rearrangement in 60% of nuclei. Whole-body PET-CT identified a suspicious inguinal node, subsequently proven reactive on biopsy. Followin
