Organ-limited ANCA-associated orbital inflammatory disease: a diagnostic challenge.

Antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) is a group of autoimmune disorders that cause inflammation and necrosis of small- to medium-sized blood vessels. It most commonly affects the kidneys and lungs, leading to complications such as rapidly progressive glomerulonephritis and pulmonary haemorrhage. Ocular involvement occurs in up to 50% of patients with AAV, most commonly presenting as scleritis, episcleritis, or orbital inflammatory disease (also known as orbital pseudotumor). Other ocular findings may include keratitis, uveitis, or retinal vasculitis. Eyelid ne

A Case Report and Brief Literature Review of Cerebrospinal Fluid Rhinorrhea in Granulomatosis With Polyangiitis: A Rare Complication.

Granulomatosis with polyangiitis (GPA) is a necrotizing small-vessel vasculitis characterized by granulomatous inflammation and antineutrophil cytoplasmic antibody (ANCA) positivity. Sinonasal involvement is common, but cerebrospinal fluid (CSF) rhinorrhea is exceedingly rare. We present a rare case of a 44-year-old woman with established GPA who developed CSF rhinorrhea secondary to erosion of the cribriform plate. Her initial disease manifestations included episcleritis, septal perforation, atrophic turbinates, and saddle-nose deformity, with high PR3-ANCA titers and biopsy-proven small-vess

DNASE1L3 Deficiency With Novel Missense Variant: Enzymatic and Plasma Fragmentomic Evidence of Pathogenicity and Partial Response to JAK Blockade.

OBJECTIVE: Biallelic loss-of-function variants in DNASE1L3 cause inherited systemic lupus erythematosus and hypocomplementemic urticarial vasculitis. These disorders arise from defective clearance of extracellular chromatin, leading to autoantibody formation, immune complex deposition, and complement consumption. The full clinical and therapeutic spectrum of DNASE1L3 deficiency remains incompletely defined. METHODS: We evaluated a 24-year-old woman with lifelong systemic inflammation characterized by polyarthritis, urticarial vasculitis, episcleritis, recurrent abdominal pain with intestinal

Ophthalmic involvement in VEXAS syndrome and its influence on mortality: insights from the international AIDA network registry.

Orbital inflammation is the most common presentation of VEXAS, with any orbital structure potentially affected. Non-sight-threatening and uncomplicated anterior non-granulomatous uveitis and anterior diffuse scleritis follow in frequency, along with episcleritis. Ophthalmic involvement was significantly associated with relapsing polychondritis (p = 0.014), with an increased chance of a fatal outcome (RR 5.87, p = 0.016) and independently predicts a higher mortality rate (OR 3.72, p = 0.026). Treatment of ophthalmic involvement showed full or partial response to glucocorticosteroids alone in 66

Scleral image analysis via artificial intelligence to evaluate inflammatory status in inflammatory bowel disease.

PURPOSE: Among the extraintestinal ocular manifestations of patients with inflammatory bowel disease (IBD), episcleritis is associated with the inflammatory activity of IBD. The use of artificial intelligence to evaluate scleral images may help establish a non-invasive method for assessing the degree of inflammation. This study aims to provide a new approach for evaluating IBD inflammation by analyzing scleral images. METHODS: We collected scleral images from patients with IBD across different inflammatory states and from controls. Then, we analyzed the images using the MASK R-CNN and HRNet m

Livedoid Vasculopathy Mimicking Rheumatoid Vasculitis in a Patient With Active Rheumatoid Arthritis.

Livedoid vasculopathy (LV) is a rare thrombo-occlusive disorder of the dermal microcirculation that may clinically resemble inflammatory vasculitis. In patients with rheumatoid arthritis (RA), the development of painful lower extremity ulcers often raises concern for rheumatoid vasculitis, making histopathological evaluation essential for accurate diagnosis and treatment selection. We present a case of a 45-year-old man with an eight-year history of seropositive RA receiving methotrexate, sulfasalazine, and baricitinib who developed painful lower extremity ulcers, edema, and episcleritis in th

IL-17 at the eye-joint interface in psoriatic disease: mechanisms, clinical correlates, and therapeutic implications.

INTRODUCTION: Psoriatic arthritis (PsA) is an immune-mediated inflammatory disease in which the IL-23/Th17/IL-17 axis contributes to musculoskeletal and extra-musculoskeletal inflammation. Ocular manifestations, particularly dry eye disease (DED), uveitis, and less frequently episcleritis and scleritis, are increasingly recognized across psoriatic disease, although their pathobiology and relationship to systemic inflammatory pathways remain incompletely defined. AREAS COVERED: This narrative review examines evidence linking IL-17 to ocular disease in psoriatic disease, with emphasis on ocular

Systemic immunosuppressive therapy in idiopathic non-infectious uveitis and scleritis: disease remission, discontinuation, and relapse patterns.

BACKGROUND: Immunosuppressive therapy (IST) is indicated in idiopathic non-infectious uveitis and scleritis when local or topical treatments are insufficient, but long-term outcomes and predictors of relapse remain incompletely defined. METHODS: We retrospectively reviewed clinical records of 110 patients with non-infectious uveitis or scleritis in the absence of systemic manifestation of disease who received IST. Time to remission, treatment duration, and relapse after IST withdrawal were analyzed by anatomic subtype. Predictors of remission and relapse were assessed using multivariable Cox

Bilateral Conjunctival Small Lymphocytic Lymphoma Simulating Inflammatory Ocular Surface Disease: A Case Report.

Conjunctival lymphoma classically appears as a painless salmon-pink subepithelial infiltrate, but its indolent forms can closely imitate inflammatory ocular surface disease and delay diagnosis, particularly when an uncommon subtype such as small lymphocytic lymphoma presents bilaterally and in isolation. A woman in her early fifties presented with one month of left ocular discomfort and sectoral redness, and slit-lamp examination revealed two multilobulated hyperaemic bulbar conjunctival nodules, clinically indistinguishable from nodular episcleritis. Topical corticosteroids and a topical non-

Validation of a Portable Tool for Conjunctival Ultraviolet Autofluorescence Imaging.

PURPOSE: Conjunctival ultraviolet autofluorescence (CUVAF) is an objective biomarker of ocular sun exposure, aiding in the assessment of ophthalmohelioses and other UV-related eye conditions. However, conventional CUVAF imaging requires a bulky and costly benchtop digital single-lens reflex (DSLR) camera system. This study evaluates the validity of a smartphone-based clip-on tool as a portable, affordable alternative for CUVAF imaging. METHODS: 95 participants were recruited from an ophthalmology clinic in Sydney, Australia. Nasal and temporal conjunctival images were captured using both DSLR

Development of a large porcine model of osteogenesis imperfecta type I.

Osteogenesis imperfecta (OI) comprises a group of disorders that lead to bone fragility. The most common forms are caused by pathogenic variants in COL1A1 and COL1A2. There are many different animal models OI from zebrafish to dogs, with murine models being the most used model. In this brief study, we describe the successful generation of the first large animal porcine model of OI Type I carrying a clinically relevant pathogenic variant, c.757C>T (p.Arg253Ter). These pigs exhibited classic features of OI Type I including blue sclera, low bone mass, and bone fragility. This model is more geneti

Butylphthalide nanoemulsion for management of dry age-related macular degeneration via antioxidant and anti-inflammatory mechanisms.

Dry age-related macular degeneration (dry AMD) is a posterior ocular segment disease characterized primarily by progressive retinal pigment epithelium degeneration, which is driven largely by oxidative stress. Effective treatment remains challenging owing to limited therapeutic agents and intrinsic physiological barriers that hinder drug delivery to the posterior eye. To address this, we developed a butylphthalide-loaded nanoemulsion (NBP-NE) that exhibits potent reactive oxygen species-scavenging and anti-inflammatory properties. Ex vivo permeation studies and pharmacokinetic analysis reveale

Pyroxd1 is essential for murine viability with the homozygous N155S recurrent variant linked to myopathy, muscle hypotrophy and osteopenia.

Biallelic variants in PYROXD1 are associated with a life limiting muscle and connective tissue disorder characterised by generalised muscle weakness, breathing and feeding difficulties, distal laxity, hypernasal speech, blue sclera and osteopenia. PYROXD1 encodes an oxidoreductase implicated in mitochondrial function and tRNA ligase activity. Herein we present the first comprehensive suite of mouse models designed to elucidate PYROXD1 redox functions and the underlying pathogenetic basis for PYROXD1 disorders. A LacZ reporter strain reveals ubiquitous expression of Pyroxd1, particularly in the

Sex-based differences in retinal detachment in a genotyped Stickler syndrome cohort: implications for preventive management.

AIMS: Stickler syndrome is a genetically inherited vitreoretinopathy that can lead to retinal detachment from early childhood. There is currently no consensus preventive treatment. We aimed at determining whether sex influences the age of onset of rhegmatogenous retinal detachment (RRD) in COL2A1 and COL11A1 Stickler patients. METHODS: This is a retrospective study including genotyped Stickler patients from two tertiary centres with at least one RRD. We analysed differences in the age of occurrence of RRD according to sex and genetic mutation. A multivariate analysis was used to analyse risk

Glaucoma drainage devices as a novel treatment option in the management of recurrent iris cysts.

IntroductionEpithelial inclusion cysts of the iris are a rare complication following ocular surgery or trauma and typically exhibit a more aggressive course than other types of iris cysts. Their management remains challenging due to their tendency for rapid growth, which can lead to obscuration of the visual axis, elevated intraocular pressure, corneal decompensation, and secondary glaucoma or uveitis. Particularly in pediatric and young adult patients, the high proliferative capacity of epithelial cells increases the risk of recurrence. Current treatment modalities, including Nd:YAG laser cys
