Tofacitinib in uveitis and scleritis.

Janus kinases (JAKs) are a family of intracellular, non-receptor tyrosine kinases that play a crucial role in the JAK signaling pathway involved in the inflammatory process. Over the years, this pathway has garnered significant interest among researchers, leading to the development of various therapeutic agents aimed at blocking the pathway, thereby contributing to remarkable progress in the management of inflammatory diseases across multiple medical specialties. Although the use of JAK inhibitors in ophthalmology is still limited, evidence is gradually emerging on their role in managing uveit

[A case of Aspergillus flavus-associated scleral abscess].

A 70-year-old male patient presented to Wuhan Aige Eye Hospital with a 1-month history of redness, pain, decreased vision in the right eye, and the pain was aggravated at night. Examination revealed multiple nodular scleral abscesses. After incision and drainage, nanopore targeted sequencing of the specimens identified Aspergillus flavus as the pathogen. The patient was diagnosed with fungal scleritis (scleral abscess) of the right eye. Susceptibility-guided systemic and topical antifungal therapy was administered for one month, leading to complete recovery. No recurrence was observed during t

Strep pyogenes infection causing rapid onset orbital cellulitis, superior ophthalmic vein thrombosis, and necrotizing scleritis with dehiscence of extraocular muscle following strabismus surgery.

PURPOSE: To report an infrequent but potentially devastating complication following strabismus surgery and its approach to management. OBSERVATIONS: A 5-year-old girl underwent bilateral medial rectus recessions for esotropia. She quickly developed progressive orbital cellulitis that was confirmed on computed tomography along with non-occlusive thrombosis of the superior ophthalmic vein. She was treated with intravenous antibiotics, additional surgery (which found a dehisced medial rectus muscle), and anticoagulation. Cultures grew Streptococcus pyogenes (Group A). Ultimately the patient made

Panophthalmitis Caused by Providencia stuartii: A Rare Opportunistic Ocular Infection.

This case report describes a case of Providencia stuartii panophthalmitis, a rare opportunistic ocular infection. A 58-year-old lady with diabetes mellitus presented with painful vision loss in the left eye for one day. Visual acuity was light perception with a relative afferent pupillary defect. Examination showed limitation of extra-ocular muscle movement in all gazes, proptosis, erythematous lid swelling, conjunctival injection, oedematous cornea, and hypopyon with fibrin in the anterior chamber. B-scan showed vitritis, loculations, and scleritis. Vitreous culture revealed P. stuartii. Conc

Uveitis and Scleritis as Early Signals of Undiagnosed IBD: A Tiered Screening Framework to Reduce Diagnostic Delay.

Uveitis and scleritis are well-recognized extraintestinal manifestations of inflammatory bowel disease (IBD). Although ocular involvement typically follows IBD diagnosis, a meaningful subset of patients presents to ophthalmologists before intestinal disease is recognized, sometimes years earlier. Despite this, no standardized ophthalmologic screening protocols exist to guide early detection. We synthesized cohort studies, meta-analyses, and Mendelian randomization evidence on the ocular-intestinal temporal relationship in IBD. Patients with uveitis or scleritis have approximately double the ri

Evaluation of the Painful Eye.

Eye pain is a common presentation in outpatient, urgent care, and emergency settings. Causes range from benign to vision-threatening. Family physicians should promptly identify red-flag features that require urgent or emergent ophthalmology referral. A painful eye with vision loss, severe photophobia, proptosis, hyphema, or corneal ulceration is a medical emergency. Patient history should include onset, associated vision changes, contact lens use, trauma, chemical exposure, systemic symptoms, and rheumatologic disease. Physical examination should include evaluation of visual acuity (eg, using

Bilateral Diffuse Anterior Scleritis With Ocular Hypertension Revealing Rheumatoid Arthritis.

Rheumatoid arthritis is a chronic systemic autoimmune disease that may involve extra-articular organs, including the eye. Scleritis is a severe and potentially sight-threatening ocular manifestation frequently associated with rheumatoid arthritis. We report the case of a 63-year-old male presenting with a 1-month history of bilateral red and painful eyes associated with photophobia and headache, initially misdiagnosed as glaucoma due to elevated intraocular pressure. Ophthalmologic examination revealed decreased visual acuity, diffuse conjunctival hyperemia, and intraocular pressure of 30 mmHg

Ocular Clues in Musculoskeletal Disorders: A Narrative Review for Orthopaedic Specialists.

Musculoskeletal disorders (MSDs) are primarily characterized by joint, bone, and connective tissue involvement, but they can also present with ocular manifestations that carry diagnostic and clinical significance. These findings such as uveitis, blue sclera, lens subluxation, and scleritis may serve as early markers of systemic disease or indicate ongoing inflammation that affects orthopaedic decision making. Despite their relevance, ocular symptoms are frequently underrecognized in orthopaedic settings, potentially delaying comprehensive care. This narrative review aims to explore the spectru

Cryptococcal scleritis mimicking scleromalacia perforans.

Scleromalacia perforans is a rare, non-inflammatory type of anterior scleritis that usually signals an underlying autoimmune disorder. However, certain infectious pathogens can mimic this clinical presentation which often leads to diagnostic difficulty. We describe a rare case of primary cryptococcal scleritis in an elderly man with diabetes who showed no evidence of systemic cryptococcal disease. He initially presented with progressive scleral thinning and minimal pain, which was interpreted initially as scleromalacia perforans. But surgical intervention with scleral patch grafting revealed Cr

JAK Inhibitors for Refractory Non-Infectious Ocular Inflammation: A Systematic Review and Meta-Analysis of Efficacy, Safety, and Optimal Positioning.

PURPOSE: Janus kinase (JAK) inhibitors have entered clinical testing for non-infectious ocular inflammation, but trial evidence is unsynthesized. We evaluated efficacy and safety within a pre-specified tiered framework. METHODS: We searched five databases, three registries, and grey literature to July 4, 2026. Eligible studies were pre-assigned to Stratum A (systemic agents for intraocular or scleral inflammation; primary) or Stratum B (topical, ocular surface disease); bias was assessed with RoB 2 and ROBINS-I. Single-arm proportions were pooled with a beta-binomial model; with I2 above the

Rosacea scleritis and sclero-uveitis; under recognised and over treated.

BACKGROUND: Rosacea episcleritis, scleritis and uveitis may be missed when clinicians focus on the eye without appraising the adnexae and facial skin. Addressing rosacea can reduce treatment failures and avoid systemic complications of less effective or more toxic drugs. CASE SERIES AND NARRATIVE REVIEW: This series presents four cases with scleritis and / or sclero-uveitis in the setting of facial or ocular rosacea. Where relevant, negative systemic scleritis work up is included. Common features were incomplete resolution or poor response to one or more of topical and systemic steroid, system

Organ-limited ANCA-associated orbital inflammatory disease: a diagnostic challenge.

Antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) is a group of autoimmune disorders that cause inflammation and necrosis of small- to medium-sized blood vessels. It most commonly affects the kidneys and lungs, leading to complications such as rapidly progressive glomerulonephritis and pulmonary haemorrhage. Ocular involvement occurs in up to 50% of patients with AAV, most commonly presenting as scleritis, episcleritis, or orbital inflammatory disease (also known as orbital pseudotumor). Other ocular findings may include keratitis, uveitis, or retinal vasculitis. Eyelid ne

Ophthalmic involvement in VEXAS syndrome and its influence on mortality: insights from the international AIDA network registry.

Orbital inflammation is the most common presentation of VEXAS, with any orbital structure potentially affected. Non-sight-threatening and uncomplicated anterior non-granulomatous uveitis and anterior diffuse scleritis follow in frequency, along with episcleritis. Ophthalmic involvement was significantly associated with relapsing polychondritis (p = 0.014), with an increased chance of a fatal outcome (RR 5.87, p = 0.016) and independently predicts a higher mortality rate (OR 3.72, p = 0.026). Treatment of ophthalmic involvement showed full or partial response to glucocorticosteroids alone in 66

Scleral image analysis via artificial intelligence to evaluate inflammatory status in inflammatory bowel disease.

PURPOSE: Among the extraintestinal ocular manifestations of patients with inflammatory bowel disease (IBD), episcleritis is associated with the inflammatory activity of IBD. The use of artificial intelligence to evaluate scleral images may help establish a non-invasive method for assessing the degree of inflammation. This study aims to provide a new approach for evaluating IBD inflammation by analyzing scleral images. METHODS: We collected scleral images from patients with IBD across different inflammatory states and from controls. Then, we analyzed the images using the MASK R-CNN and HRNet m

IL-17 at the eye-joint interface in psoriatic disease: mechanisms, clinical correlates, and therapeutic implications.

INTRODUCTION: Psoriatic arthritis (PsA) is an immune-mediated inflammatory disease in which the IL-23/Th17/IL-17 axis contributes to musculoskeletal and extra-musculoskeletal inflammation. Ocular manifestations, particularly dry eye disease (DED), uveitis, and less frequently episcleritis and scleritis, are increasingly recognized across psoriatic disease, although their pathobiology and relationship to systemic inflammatory pathways remain incompletely defined. AREAS COVERED: This narrative review examines evidence linking IL-17 to ocular disease in psoriatic disease, with emphasis on ocular
