Rapid Pterygium Progression in a Child on Growth Hormone Therapy: A Case Report and Literature Review.

Pterygium is rare in children, and the effects of growth hormone (GH) therapy on its progression have not been previously described. Because GH and GH-releasing hormone receptor pathways influence cellular proliferation and inflammation, GH supplementation may contribute to accelerated pterygium growth in susceptible patients. This case highlights a rapidly progressive pterygium in an adolescent receiving GH therapy. A 13-year-old male presented with blurred vision and a rapidly enlarging nasal pterygium in the left eye. A small red nodule had first been noted in this patient at six years of a

Bridging the Gap: A Case of Unique Anterior Segment Optical Coherence Tomography Presentation of Retrocorneal Membrane and Fibrous Bridging After Birth-Related Descemet's Membrane Detachment.

Descemet's membrane detachment (DMD) is an uncommon corneal pathology that typically occurs following intraocular surgeries or direct ocular trauma. Congenital or birth trauma-related DMD is rare and often remains undiagnosed in asymptomatic individuals. Longstanding cases may develop fibrous bridging or retrocorneal membrane formation, making diagnosis challenging without advanced imaging. We report the case of a 32-year-old asymptomatic male who presented for a routine ophthalmic evaluation. Slit-lamp examination of the right eye revealed a localized inferotemporal corneal opacity with subtl

The "Polish Sign": A Novel Intraoperative Visual Indicator During Descemet Membrane Polishing in Cultured Human Corneal Endothelial Cell Injection Therapy.

PURPOSE: The aim of this study was to describe a novel intraoperative visual indicator, the "polish sign," which aids safe and effective Descemet membrane (DM) polishing during cultured human corneal endothelial cell (cHCEC; Vyznova) therapy for bullous keratopathy. METHODS: Thirty-two consecutive eyes with bullous keratopathy underwent cHCEC injection therapy. After a 1.2-mm limbal incision, the central corneal endothelium and associated abnormal extracellular matrix within an approximately 8-mm zone were gently polished using a Kinoshita-type DM polisher, consisting of a blunt needle covere

Robotic Assisted-Intravitreous Light Pipe Illumination for Cataract Surgery in Eyes with Corneal Opacity: Case Reports.

INTRODUCTION: In cataract surgery for eyes with corneal opacity, reduced intraoperative visibility remains a major surgical challenge. To enhance intraoperative visualization, OQrimo, a robotic medical device approved in Japan, was used to hold a light guide. Outcomes of 3 eyes from 2 cases are reported. CASE PRESENTATION: Case 1: A 48-year-old man developed bilateral corneal opacity from graft-versus-host disease following umbilical cord blood transplantation for acute myeloid leukemia. At age 45, he underwent penetrating keratoplasty in the right eye but declined surgery in the left eye. Bi

A bioactivity-enhanced thermosensitive amnion-derived hydrogel with sustained IGF-1 release: A multitargeted and efficient strategy for corneal injury repair.

Corneal alkali burn (CAB) remains difficult to treat because rapid tear clearance limits topical drug exposure, while the injured cornea develops a complex microenvironment involving inflammation, neovascularization, fibrosis, epithelial barrier loss, and impaired nerve repair. Here, we developed a human amniotic membrane-derived thermosensitive hydrogel (hAMgel) and loaded it with insulin-like growth factor-1 (IGF-1) to create a bioactive sustained-release system (IGF1@hAMgel) for CAB repair. The hAMgel transformed from a flowable precursor into a semi-transparent film on the ocular surface w

A deep dive into radiation keratopathy; Going beyond the current frontierss.

Radiotherapy is one of the conventional treatments for head and neck malignancies. Despite the implementation of protective measures to minimize the detrimental impact on healthy tissues surrounding the radiation site, radiation keratopathy remains a prevalent complication. We aimed to establish a mouse model of radiation keratopathy to characterize the pathophysiology of the disease and enable future identification of potential treatments. Thirty-six mice were divided equally into six groups. One eye of each mouse was irradiated with 5, 10, 15, 20, 25, and 30 Gy and the other eye used as a co

Long-term ocular complications after kidney transplantation.

To assess long-term ocular complications and identify factors affecting these complications in pre-emptive and dialysis patients who have undergone kidney transplantation. We included 548 patients who had a complete ophthalmologic examination at least one year after kidney transplantation. The patients were divided into two groups: those who received dialysis before kidney transplantation (group 1) and those who did not (group 2). We recorded the presence of diabetes mellitus (DM) and hypertension (HT), which could contribute to concomitant retinopathy. Data collected included best corrected v

Schnyder Corneal Dystrophy in an Adolescent: A Case Report With Multimodal Imaging.

Schnyder corneal dystrophy (SCD) is a rare, autosomal dominant, bilateral corneal dystrophy characterized by progressive deposition of cholesterol and phospholipids within the central corneal stroma. The condition is associated with pathogenic variants in the UBIAD1 gene, responsible for lipid metabolism. We report the case of a 17-year-old female presenting with progressive bilateral visual deterioration. Comprehensive ophthalmological examination, including slit-lamp biomicroscopy, in vivo confocal microscopy, and anterior segment optical coherence tomography (AS-OCT), was performed. The exa

Retrolental Opacity: Atypical Manifestation of Lens-Induced Uveitis.

The crystalline lens can cause intraocular inflammation when its capsule is ruptured or allows leakage of lens proteins into the anterior chamber, a condition known as lens-induced uveitis (LIU). LIU is a rare form of uveitis and can be classified based on the integrity of the lens capsule. When the capsule is ruptured, the inflammation is called phacoantigenic or phacoanaphylactic uveitis, whereas if the capsule remains intact but leaks proteins, it is termed phacolytic uveitis. Common causes include ocular trauma, previous surgery, or spontaneous rupture in hypermature cataracts. Initial tre

Ipsilateral, Nonrotational Autokeratoplasty (INRA) Enabling Cataract Surgery in Eyes With Severe Ocular Surface Disease and Corneal Opacity.

PURPOSE: In patients with severe ocular surface disease (OSD) and cornea and cataract-related vision impairment, where the risk of allograft rejection after allogeneic corneal transplantation is unacceptably high and cataract surgery impossible because of corneal opacity, cataract extraction may still offer meaningful improvement in vision-related quality of life. We describe ipsilateral, nonrotational autokeratoplasty (INRA) as option to perform open-sky cataract surgery after removal of the central cornea and prevent immune responses by placing back the original host cornea. METHODS: This r

Presumed Autoimmune Keratitis in Both Eyes Without Systemic Manifestations: A 40-Year Course of a Patient With Corneal Infiltrates and Melt.

Peripheral corneal infiltration, corneal ulcer, and melt are recognized complications linked to systemic immunological diseases, such as antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis. These manifestations, which occur in isolation, might be autoimmune keratitis but are difficult to prove underlying immunological abnormalities. This report described a patient with presumed autoimmune keratitis who repeatedly presented corneal infiltration and perforation in both eyes even after penetrating keratoplasty. The 68-year-old patient with a stable condition of keratoconjunctivitis s

Native Bio-Lenticules Mediated Rapamycin Delivery for Corneal Immunomodulation and Tissue Repair Promotion.

Corneal damage-related disorders, characterized by stromal opacity due to trauma, infection, or other etiologies, remain a major cause of visual impairment. Despite therapeutic advances, the scarcity of donor corneal grafts and the complexity of immune regulation continue to pose critical challenges. To address these limitations, we developed a novel immunomodulatory biomaterial composed of decellularized human stromal lenticules and rapamycin (RAPA) solubilized with deep eutectic solvents. Designed for interlamellar corneal implantation, this drug-loaded construct enables localized, sustained

Dystrophia Adiposa Corneae: An Out-of-the-Box Diagnosis.

Dystrophia adiposa corneae is a degenerative corneal disorder characterised by fatty deposition, leading to corneal opacity and reduced visual acuity. We report a case of an Indian male in his early 40s with bilateral peripheral deep lipid deposition in the corneal stroma, beginning in an arcuate fashion and progressing to an annular-shaped structure. Furthermore, cholesterol crystals, as well as crystalline structures, were seen without any neovascularisation. We confirmed these findings on anterior segment optical coherence tomography (AS-OCT). We also noticed an increase in central corneal

A Case of Bacterial Keratitis in a Patient Using Orthokeratology and Soft Contact Lenses.

A 63-year-old male with high myopia developed sudden visual loss, eyelid swelling, eye pain, discharge, and tearing in his left eye while wearing soft contact lenses (CLs) during the day and orthokeratology lenses at night. At the initial visit, his corrected visual acuity in the left eye was 20/1000, with a ring-shaped ulcer in the central cornea, corneal infiltration across the entire cornea, and conjunctival hyperemia. Pseudomonas aeruginosa was detected from corneal scrapings, and after antibiotic treatment, the ulcer healed with corneal opacity remaining. On the 60th day, corrected visual

Addressing corneal opacity after herpes zoster infection.

A 15-year-old boy was referred for corneal opacity evaluation. The patient had a previous herpes zoster virus (HZV) infection-varicella-zoster virus (VZV)-with ocular manifestation 1 year ago. After the infection, he developed a central corneal scar and decreased corrected distance visual acuity (CDVA) in the right eye. The slitlamp examination showed the right eye with central corneal opacity (involving anterior stroma), lacuna area between the haze, fluorescein negative, and no vascularization near the scar (Figure 1JOURNAL/jcrs/04.03/02158034-202406000-00019/figure1/v/2024-07-10T174224Z/r/i
