PURPOSE: We report a unique case of recurrent Descemet membrane endothelial keratoplasty (DMEK) failure in a 69-year-old man with a history of pseudophakic bullous keratopathy (PBK) secondary to glaucoma surgeries.
METHODS: This is a retrospective case report.
RESULTS: The initial PAUL glaucoma implant was relocated to the sulcus, and the original sclerostomy was plugged with Tutoplast. The patient subsequently developed PBK and underwent 3 DMEK procedures, each resulting in immediate failure, beginning inferiorly despite uneventful surgery. Subsequent assessment revealed that the Tutoplast

Pseudophakic bullous keratopathy (PBK) is a vision-threatening corneal complication following cataract surgery, characterised by progressive endothelial cell loss, persistent corneal oedema, and painful epithelial bullae, leading to impaired vision. Corneal transplantation, either penetrating or endothelial keratoplasty, remains the primary treatment but faces challenges such as donor tissue shortages, graft rejection, and limited graft longevity. Recently, Rho-kinase (ROCK) inhibitors have emerged as promising pharmacological alternatives. These agents enhance corneal endothelial cell prolife

Corneal bee stings cause inflammation through direct venom toxicity and type I hypersensitivity reactions that can lead to endothelial cell damage. In severe cases, these injuries can progress to bullous keratopathy, a condition that necessitates corneal transplantation. Here, we reported two cases in which transparent healing was achieved through steroid treatment, yet subsequent corneal endothelial cell damage ensued. In the initial case, an 81-year-old male sustained a sting in the left eye from a honeybee. Initial visual acuity was 20/17 in the right eye and 20/33 in the left eye. A prelim

PURPOSE: The objective of this study was to evaluate the efficacy of scraping without endothelial keratoplasty technique using a novel cell scraper for treating corneal endothelial dysfunction and explore the adjuvant effects of a ROCK inhibitor (ROCKi).
METHODS: Human donor tissues (n = 3) were mounted on an artificial anterior chamber, and one-half of the corneal endothelium was scraped with a novel cell scraper. The tissues were then stained with hematoxylin and eosin (H&E) to assess cell removal. In parallel, central peeling (descemetorhexis) using sharp acute forceps (n = 8) and cell scr

BACKGROUND: Our earlier report described the scleral flap and patch technique for a Baerveldt glaucoma implant (BGI). In this procedure, the Hoffman elbow is covered not only with a scleral flap (autograft) but also with a grafted sclera from a donor. This technique is expected to be effective in preventing tube erosion. Herein, we investigated the surgical outcomes of this method.
PATIENTS AND METHODS: This was a single-facility retrospective analysis of 120 consecutive patients (140 eyes) who underwent BGI surgery with the scleral flap and patch technique and were followed or > 1 year. We u

PURPOSE: Epithelial downgrowth (EDG) is a relatively infrequent complication of intraocular surgery or eye trauma. The literature is limited on this condition, and it remains a diagnostic challenge, both histologically and clinically. We have conducted a 24-year single-institution clinical and pathological retrospective review of EDG cases.
METHODS: Cases of histologically suspected EDG were identified through the electronic record system at Casey Eye Institute Pathology Laboratory. Keywords entered in the search included "epithelial ingrowth" or "epithelial downgrowth" spanning the dates Jan

BACKGROUND: A long glaucoma implant tube in the anterior chamber (AC) may cause intermittent or persistent tube-corneal endothelial touch.[1-3] This would result in corneal endothelial cell loss, corneal edema, and bullous keratopathy. Various minimally invasive techniques have been described for tube shortening or repositioning of the tube in the AC.[1-4] To shorten the tube, it can be trimmed within the AC to protect the corneal endothelium.
PURPOSE: We describe a minimally invasive technique of trimming a long tube in the AC after Ahmed glaucoma valve (AGV) implantation for angle recession

BACKGROUND: Corneal blindness is a significant public health issue worldwide, particularly in resource-constrained countries with limited access to specialized ophthalmic care. Keratoplasty is one of the essential treatment approaches for rehabilitating vision in patients with corneal pathologies.
AIMS: The primary aim of the study was to assess the challenges faced during penetrating keratoplasty (PK) and the outcomes of surgeries in a resource-constrained country in the absence of a domestic eye bank. The secondary aim was to assess the outcomes of PK.
METHODS: This cross-sectional descrip

PURPOSE: Significant endothelial cell density (ECD) losses in Descemet membrane endothelial keratoplasty (DMEK) can precipitate graft failure. Currently, clinical practice only evaluates the central corneal endothelium and not the paracentral or peripheral cornea. Here, we evaluate a deep learning (DL) algorithm for automated image quality assessment and detection of low corneal ECD in widefield specular microscopy (WFSM) images following DMEK, at central, paracentral and peripheral regions.
METHODS: Prospective observational study in eyes with clear, stable DMEKs. WFSM imaging (CEM-530; Nide

BACKGROUND: Disruption of the corneal epithelial barrier and the use of contact lenses are important risk factors for keratitis. Severe cases of keratitis can cause serious problems, including corneal scar and perforation, which can cause vision loss. Although keratitis is a significant cause of blindness, the healing process can offer substantial relief to patients who develop it after bullous keratopathy. CASE PRESENTATION: An 85-year-old patient, using bandage contact lenses for bullous keratopathy, presented with redness, mild pain and a sticky sensation in their left eye. His best visual

Descemet's membrane (DM) serves as the specialized basement membrane that supports corneal endothelial cells (CECs), which actively maintain corneal transparency and fluid balance. In diseases like Fuchs' dystrophy and pseudophakic bullous keratopathy, damage to DM and the endothelial cell layer leads to corneal edema and significant visual impairment, often necessitating surgical intervention. Traditional endothelial keratoplasty techniques require donor tissue yet many countries have shortages which limit accessibility. The risk of endothelial keratoplasty failure is generally low, but eyes
