Drug- and Cell-Type-Specific Effects of ROCK Inhibitors as a Potential Cause of Reticular Corneal Epithelial Edema.

Rho-associated kinase (ROCK) inhibitors have gained popularity as novel treatment options in the management of glaucoma and corneal endothelial disorders. Among the various ocular side effects, reticular corneal epithelial edema has been most frequently reported, mainly after treatment with netarsudil. To explain the potential mechanisms, we comparatively analyzed the effects of ripasudil and netarsudil on corneal endothelial and epithelial function in vitro. Primary human corneal endothelial and epithelial cells were incubated with netarsudil dihydrochloride and ripasudil hydrochloride dihydr

The "Cut It Out" Technique Approach in Open Globe Injury Management Caused by a Penetrating Fishhook Trauma.

This case report explores fishhook-related ocular injuries, which vary in severity and may involve different ocular structures. It focuses on the "cut-it-out" technique for removing an embedded fishhook and highlights its effectiveness. Additionally, it emphasizes the necessity of immediate first aid and infection control in managing these complex ophthalmic cases. A nine-year-old male patient arrived at our clinic six days post-injury. The ocular examination revealed a corneal laceration, edema, folds in Descemet's membrane, hypopyon, and a retained lens fragment. Visual acuity was significan

Ocular ischaemic syndrome due to giant cell arteritis.

Giant cell arteritis (GCA) is a large vessel vasculitis with devastating visual consequences if left untreated. Classically, GCA has a predilection for the short posterior ciliary arteries which supply the optic nerve head, causing sudden painless visual loss with the development of arteritic anterior ischaemic optic neuropathy (AAION). Other ocular manifestations of GCA include central retinal artery occlusion, oculomotor nerve palsy, posterior ischaemic optic neuropathy and ocular ischaemic syndrome (OIS). OIS is a rare clinical entity, typically stemming from severe, unilateral carotid arte

Ahmed and baerveldt in glaucoma surgery: what is the safest choice? - a systematic review and meta-analysis.

BACKGROUND/AIMS: Glaucoma drainage devices (GDDs) are essential in managing complex glaucoma cases. This review focuses on the Ahmed glaucoma valve (AGV) and Baerveldt glaucoma implant (BGI), the most commonly used GDDs. We aim to evaluate complications associated with AGV and BGI, particularly post-operative hypotony. METHODS: We systematically reviewed randomized controlled trials (RCTs) and non-randomized studies (NRSs) comparing AGV and BGI. The primary outcome was persistent hypotony (IOP < 5 mmHg). Secondary outcomes included hypotony maculopathy, choroidal effusion, suprachoroidal hemo

Clinicopathological characterization of non-guttata corneal endothelial dystrophy in Saudi patients with idiopathic endothelial failure.

Corneal endothelial cell dysfunction is a common cause of corneal decompensation in elderly. We describe a non-guttata corneal endothelial dystrophy with correlation of the clinical features to the histopathological and ultrastructural characteristics in a cohort of Saudi patients. A retrospective study of all consecutive cases of primary corneal decompensation in phakic eyes due to endothelial attenuation, in the absence of guttata is conducted. Patients were treated by either penetrating keratoplasty (PKP) or Descemet's Stripping Automated Endothelial Keratoplasty (DSAEK) as a primary proced

Risk of Permanent Corneal Injury in Microgravity: Spaceflight-Associated Hazards, Challenges to Vision Restoration, and Role of Biotechnology in Long-Term Planetary Missions.

Human space exploration presents an unparalleled opportunity to study life in extreme environments-but it also exposes astronauts to physiological stressors that jeopardize key systems like vision. Corneal health, essential for maintaining precise visual acuity, is threatened by microgravity-induced fluid shifts, cosmic radiation, and the confined nature of spacecraft living environments. These conditions elevate the risk of corneal abrasions, infections, and structural damage. In addition, Spaceflight-Associated Neuro-Ocular Syndrome (SANS)-while primarily affecting the posterior segment-has

A feasibility of computational drug screening for Fuchs endothelial corneal dystrophy.

Fuchs endothelial corneal dystrophy (FECD) remains a leading cause of corneal blindness globally, with corneal transplantation being the primary treatment. FECD is characterized by the formation of guttae, extracellular matrix (ECM) deposits beneath the corneal endothelium, and progressive endothelial cell loss. These pathological changes cause visual deterioration through light scattering by guttae and corneal edema due to endothelial cell loss. However, limitations such as donor shortage and graft failure necessitate alternative therapeutic approaches. We employed computational drug screenin

Descemet's Membrane Detachment During Cataract Surgery in Lattice Corneal Dystrophy Type I: Histopathological Analysis of Posterior Corneal Involvement.

Lattice corneal dystrophy Type I (LCD1) is characterized by amyloid deposition in the corneal stroma, however its impact on Descemet's membrane adhesion remains poorly understood. This study reports a rare case of Descemet's membrane detachment (DMD) during cataract surgery in a patient with LCD1 and investigates the potential role of posterior corneal amyloid deposits in this complication. A 68-year-old male presented with LCD1 was admitted for cataract surgery for the left eye. DMD appeared during the irrigation and aspiration (I/A) phase after intraocular lens (IOL) implantation. After intr

Vitamin A Deficiency in Children With Autism Spectrum Disorder.

Vitamin A deficiency (VAD) can manifest as xerophthalmia, a spectrum of eye conditions ranging from night blindness to severe outcomes like keratomalacia and corneal scarring. We report two cases of a seven-year-old girl and a 13-year-old boy in Japan with xerophthalmia caused by VAD associated with autism spectrum disorder (ASD). The first case demonstrates severe complications of prolonged VAD, including optic disc edema and irreversible corneal damage, which then results in blindness. The second case identified and treated earlier in the stage of corneal xerosis shows favorable prognosis. T
