Hypermucoviscous Klebsiella corneal abscess: Infection from inside out.

BackgroundCorneal ulcers are typically caused by external ("exogenous") factors such as trauma, contact lens use, or preexisting ocular surface diseases. Internal systemic ("endogenous") sources have not been reported as direct causes of corneal ulcers. Here, we report, to our knowledge, the first documented case of a corneoscleral abscess that resulted from an underlying bacteremia.MethodThis is a single case report.ResultA previously healthy 35-year-old male presented to the Emergency Department with left eye redness, pain, and blurred vision. Initial examination showed reduced visual acuity

Comparison of Subconjunctival Triamcinolone with Topical Prednisolone for Routine Anti-Inflammatory Prophylaxis in Manual Small Incision Cataract Surgery: A Single-Center, Randomized Controlled Trial Pilot Protocol.

BACKGROUND: Cataract surgery is the most commonly performed operation worldwide. Prophylaxis against postoperative endophthalmitis and inflammation currently includes perioperative, patient-administered instillation of topical antibiotic and anti-inflammatory drops in the US, Europe and Latin America. As patients can face challenges with adherence to topical drops for self-instillation, injected drugs may offer a solution. Intracameral antibiotic injection is superior to drops for endophthalmitis prevention in manual small incision surgery (MSICS) and phacoemulsification eyes. Subconjunctival

Design and evaluation of curcumin, naringenin, and alpha-lactalbumin encapsulated nanostructured lipid carriers for targeted ocular drug delivery in diabetic retinopathy.

Diabetic retinopathy (DR), a leading cause of vision impairment and blindness, poses significant challenges for ocular drug delivery due to barriers limiting drug penetration to the eye's posterior segment. This study aimed to develop and characterize nanostructured lipid carriers (NLCs) encapsulating curcumin, naringenin, and alpha-lactalbumin for sustained and targeted ocular delivery via topical administration in DR management. The NLCs exhibited a uniform particle size of 138.7 ± 2.6 nm, high encapsulation efficiency (∼97 %), and a stable zeta potential of -25.6 mV. Transmission electron m

Investigating the histomorphometric changes of rabbit cornea after infection with Pseudomonas Aeruginosa and treatment with cold plasma.

INTRODUCTION: Pseudomonas aeruginosa-caused ocular infections are dangerous since they can cause corneal inflammation, damage, and blindness. This work aims to carefully assess the histomorphometric and clinical changes in rabbit corneas caused by Pseudomonas aeruginosa infection, focusing on investigating the potential therapeutic benefits of cold plasma. MATERIALS AND METHODS: The right eye served as an untreated control, and the animals were randomly divided into four groups: Hariprasad et al. (2008) [1] Control group (the left eye infected with P. aeruginosa); Statham et al. (2008) [2] An

Descemet Membrane Detachment Assessed by Anterior Segment-Optical Coherence Tomography and Managed With Descemetopexy and Corneal Venting Incision: A Case Series.

Descemet membrane detachment (DMD) is a serious complication of cataract surgery noted mostly in the immediate post-operative period. We present a case series of five patients who experienced unresolved corneal edema during the post-operative period with DMD, as observed in slit-lamp examination and confirmed by anterior segment-optical coherence tomography. In all these cases, descemetopexy, along with a corneal venting incision, is performed for better and faster results, resulting in a clearer cornea. Early detection and management of DMD can give excellent visual outcomes and prevent visio

Acute Angle Closure Glaucoma Secondary to Suprachoroidal Hemorrhage Triggered by Valsalva Maneuver: A Rare Case.

Spontaneous suprachoroidal hemorrhage is a rare ophthalmic emergency, typically associated with ocular surgery or trauma. However, it can occur spontaneously in anticoagulated elderly patients after a sudden rise in venous pressure, such as during a Valsalva maneuver. We report a rare case of acute angle closure glaucoma secondary to suprachoroidal hemorrhage triggered by severe vomiting in an anticoagulated patient. An 84-year-old male with atrial fibrillation on apixaban was hospitalized for biliary colic. Following repeated vomiting episodes, he experienced sudden, painful vision loss in th

Pluripotent Stem Cell-Derived Corneal Endothelial Cells Form a Functional Corneal Endothelium in Canines.

PURPOSE: We previously reported that cryopreserved human embryonic stem cell (hESC)-derived corneal endothelial cells (CECs) can form a functional corneal endothelium (CE) on denuded Descemet's membrane (DM) in rabbits and monkeys. Here, we extend the utility of cryopreserved CECs to develop a possible treatment for canines with corneal endothelial dysfunction. METHODS: We developed a corneal endothelial dysfunction canine model (n = 2) by mechanical scraping of central (8 mm diameter) CE. To regenerate the CE, the canines received an injection of cryopreserved hESC-derived CECs in the anteri

Cataract Surgery in Pet Rabbits: Clinical Presentation, Treatment, and Long-Term Outcomes.

Cataracts cause vision loss in rabbits, often either spontaneously or as secondary to uveitis. This study considers the ophthalmic presentation, treatment, and outcome of phacoemulsification in seven pet rabbits: six presenting with lens cloudiness and one presenting with a white mass in the iris. Ophthalmic examinations revealed cataracts. The treatment plan was phacoemulsification. Encephalitozoon cuniculi was identified via an enzyme-linked immunosorbent assay technique performed on all rabbits. Ocular ultrasonography was performed to rule out retinal detachment. Phacoemulsification using t

Corneal Endothelial Cell Damage Following Bee Stings: A Report of Two Cases.

Corneal bee stings cause inflammation through direct venom toxicity and type I hypersensitivity reactions that can lead to endothelial cell damage. In severe cases, these injuries can progress to bullous keratopathy, a condition that necessitates corneal transplantation. Here, we reported two cases in which transparent healing was achieved through steroid treatment, yet subsequent corneal endothelial cell damage ensued. In the initial case, an 81-year-old male sustained a sting in the left eye from a honeybee. Initial visual acuity was 20/17 in the right eye and 20/33 in the left eye. A prelim

Understanding the role of hydration and hydration gradient in corneal disease.

Maintaining proper corneal hydration is critical for vision. Yet, a quantitative understanding of hydration throughout the corneal stroma is lacking. The purpose of this work is to to better understand the corneal hydration gradient including its origins, diagnostic utility, and role in different diseases. A theoretical model was developed for fluid flow in the cornea, which is driven largely by the stromal swelling pressure. The model employs a novel, first principles equation of state for stromal swelling pressure that captures the strong dependence on hydration that is observed experimental
