We studied the lysosomal enzyme activities in corneas obtained from 12 patients with keratoconus. Three acid hydrolases--acid phosphatase, acid esterase, and acid lipase--were demonstrated by histochemical staining methods in the epithelium, stroma, and endothelium of corneas with keratoconus and normal human corneas. Analyses by an image-processing system indicated that the epithelium, especially the basal epithelium, of corneas with keratoconus had significantly higher levels of acid phosphatase, acid esterase, and acid lipase than those in normal human controls. Sixty-seven patients with keratoconus were classified according to atopic status. Keratoconus patients with and without atopy did not differ significantly with regard to sex, age of onset, or rate of keratoplasty, but patients with very high IgE levels were more prone to graft rejection. Using a computer-assisted video-keratoscope, we determined the topographic findings of 14 eyes of 10 patients with keratoconus. The EyeSyS Corneal Topography System manufactured by EyeSys Laboratories, Inc., Houston, Tex utilizes a unique data-acquisition design to perform rapid, cost-effective quantitative photokeratoscopy. We evaluated a first-generation prototype system. Repeated testing with this prototype demonstrated an accuracy and reproducibility to within 0.25 D on spherical surfaces. Sixteen corneal buttons were obtained from 16 patients with keratoconus at the time of penetrating keratoplasty (mean age, 34 +/- 3.2 yr) and prepared for phosphorus-31 nuclear magnetic resonance spectroscopy following perchloric acid extraction. Elective penetrating keratoplasty has been performed on an outpatient basis on 37 patients since July 1987. The commonest indications have been herpetic keratitis, keratoconus, and pseudophakic or aphakic corneal oedema. Local anaesthesia, with little or no sedation, was used in most cases, and only three patients had a general anaesthetic. A series of 86 penetrating keratoplasties for keratoconus was analysed. The surgery, using an 8.2 mm donor cornea into an 8.0 mm recipient opening, was performed between January 1983 and January 1986. The donor cornea was secured by two opposing continuous sutures, placed at full corneal thickness under surgical keratometry control. Both sutures were removed on average 30 weeks after surgery. A good anatomical result (clean graft) after keratoplasty with transparent taking in of the transplant is not always associated with a good functional result, i.e. with a sufficiently high vision acuity; this is due to high postoperative astigmatism. A surgical intervention is suggested to solve this problem; this operation may be performed on an outpatient basis. The results are reported.

Forty cases of keratoconus were surgically treated with epikeratoplasty. The surgical technique is aimed at reducing the degree of high irregular myopic astigmatism of keratoconus and providing the ectatic cornea with a permanent mechanical support. Nearly all patients showed a satisfactory improvement in uncorrected and best corrected visual acuity. This reduction of preoperative astigmatism and spherical equivalent with a regularization of the corneal surface permitted the fitting of contact lenses for daily wear in patients who were preoperatively intolerant of them.

Epikeratophakia surgery is a relatively new surgical technique which involves suturing of prelathed corneal tissue on top of the recipient's cornea. The epikeratophakia lens is lathed according to the recipient's refractive needs and therefore can correct myopia and hyperopia; a plano lenticle can be used to correct keratoconus. After the first description of epikeratoplasty for the correction of aphakia by Kaufman in 1980, the application of epikeratoplasty has been extended further to the treatment of keratoconus and high myopia. The etiology of keratoconus is still unclear. This study presents a new clinical sign, Thalasselis' syndrome, defined as: an association between keratoconus, magnesium deficiency, type-A behavior and allergy. Also, it introduces the hypothesis that magnesium deficiency could affect pathologically the osmotic mechanism of the cornea, specifically the Na-K and/or Ca-ATPase pumps; the collagen structure by alteration of the adenylate cyclase activity; and other mechanisms as well. The type of corneal ectasia, the presence of breaks in Descemet's membrane, and the success with contact lens wear or penetrating keratoplasty were studied in 61 patients with corneal ectasia and vernal keratoconjunctivitis.
