Visual outcomes and short-term complication profile following Yamane scleral fixation of intraocular lenses across different surgical indications.

PURPOSE: To evaluate visual and safety outcomes of double-flanged (Yamane) intrascleral intraocular lens fixation across different clinical scenarios of capsular support loss. METHODS: This retrospective cohort study included 55 eyes undergoing Yamane scleral fixation. Eyes were stratified by surgical indication into aphakia (10), dislocated intraocular lens (12), complicated phacoemulsification (15), and dropped intraocular lens managed with pars plana vitrectomy (18). Best-corrected visual acuity, intraocular pressure, and postoperative complications were assessed preoperatively and at 1-we

Anterior segment optical coherence tomography-guided management of capsular contraction and anterior-posterior capsular adhesion after congenital cataract surgery: a case report.

BACKGROUND: Capsular changes after congenital cataract surgery may compromise the visual axis in infants and may necessitate secondary surgical intervention. In such situations, identifying a safe dissection plane can be challenging under conventional microscopic visualization, and adjunctive intraoperative imaging may be beneficial. CASE PRESENTATION: A 2-month-old boy diagnosed with bilateral congenital cataracts underwent phacoaspiration, posterior capsulotomy, and anterior vitrectomy with planned aphakia. Despite continued topical treatment, the patient developed progressive posterior syn

Accuracy of intraocular lens calculation formulas in aphakic eyes undergoing simultaneous silicone oil removal and intraocular lens implantation.

PURPOSE: To evaluate the accuracy of intraocular lens (IOL) power calculation formulas in aphakic eyes undergoing simultaneous silicone oil removal (SOR) and IOL implantation, including in-the-bag implantation and ciliary sulcus implantation, and assess the effect of 'Rule of 9s' adjustment for ciliary sulcus implantation. METHODS: The performance of six formulas (BUII, EVO, LSF, Hoffer Q, Holladay 1, SRK/T) was evaluated. Outcomes were assessed using mean prediction error (ME) for trueness, standard deviation (SD) for precision, and median absolute error (MedAE) for accuracy. Ciliary sulcus

[Clinical study of scleral suture fixation of astigmatism-correcting intraocular lenses without capsular support].

Objective: To investigate the surgical technique, positional stability and clinical efficacy of scleral suture fixation of single-focus and multifocal toric intraocular lenses (TIOLs) in the ciliary sulcus for eyes without capsular support. Methods: This retrospective case series study enrolled 20 patients (25 eyes) who underwent posterior chamber TIOL fixation with a 9-0 polypropylene suture for both haptics combined with a Z-shaped intrascleral suture technique at the Ophthalmic Center Affiliated to School of Medicine, Shanghai University from December 2023 to December 2024. A rare case of aphakia associated with anterior segment malformation in a cocker spaniel puppy.

This report describes a rare case of bilateral congenital aphakia with anterior segment malformation in a 12-week-old male cocker spaniel puppy. The puppy was initially suspected of having glaucoma and was subsequently diagnosed with anterior segment dysgenesis and aphakia resulting in blindness. Ophthalmic examination showed corneal opacity, absence of pupillary openings, and negative pupillary light reflexes. B-mode and high-frequency ultrasonography showed absence of the lens, poorly defined anterior chambers, and hyperechoic tissue adherent to the cornea, consistent with anterior segment d

Corneal Morphometrics in Congenital Primary Aphakia: A Multimodal Imaging Perspective.

PURPOSE: To characterize in vivo corneal architectural changes in congenital primary aphakia (CPA) using depth-resolved, layer-specific AS-OCT findings with those of age-matched controls. METHODS: This observational study includes 31 eyes of 16 children with clinically diagnosed CPA. High-resolution AS-OCT was used to obtain meridional and lamellar corneal thickness measurements: anterior, central stromal, and posterior layers were quantified across 5 standard locations (central, nasal, temporal, superior, and inferior). Quadrant-wise and layer-wise values were compared within the CPA cohort

A Novel Modification of the Yamane Intrascleral Haptic Fixation Technique with Scleral Groove-Flap Coverage: A Case Series Study.

PURPOSE: To describe a modification of the Yamane double-needle technique using partial-thickness scleral groove-flap coverage of the terminal haptic flanges for secondary IOL implantation in eyes without adequate capsular support. METHODS: This surgical technique report describes a modified approach to flanged intrascleral haptic fixation (ISHF). At two fixation sites 180° apart, partial-thickness scleral grooves are created. After haptic externalization and cauterization, the terminal flanges are guided into the grooves and buried beneath scleral tissue rather than positioned directly benea

Application of modified pre-set double-wire intraocular lens suture fixation technique in aphakic eyes: A retrospective study.

ObjectiveSurgery for aphakic eyes without capsular bag support is challenging and remains a key area of research for ophthalmologists. This retrospective study introduces a novel surgical method for suture fixation of intraocular lenses in such eyes.MethodsThe modified surgery was performed on eight eyes without a natural lens. The two haptic tabs of the three-piece intraocular lens were secured with two crystal sutures using a wireless junction-defined as a connection point where the loop or haptic is anchored to the eye wall without knots through techniques such as intrascleral pocket embedd

Trocar sleeve adapter accelerates silicone oil injection in real-world vitreoretinal surgery.

PURPOSE: This retrospective, non-randomised, observational cohort study investigated the efficiency of a novel 23G trocar adapter to accelerate silicone oil (SO) injection during endotamponading vitreoretinal surgery. SUBJECTS: A total of 105 eyes were included consecutively. Only 23G procedures were eligible. Indications for SO injection were retinal detachment, recurrent retinal detachment, or SO exchange in eyes with retinal instability using either the high-flow viscous-fluid-extraction accessory cannula as the 23G trocar sleeve adapter (1362.VFE2, Dutch Ophthalmic Research Centre [DORC],

Concurrent mutations in DNAH5 and FOXE3 genes: a unique occurrence in infancy.

A 3-month-old female infant from South India presented with microphthalmia and features suggestive of primary ciliary dyskinesia (PCD). She was born to a third-degree consanguineous couple and showed signs of breathing difficulty at birth, frequent respiratory infections, bilateral microphthalmia, hypertelorism, a flat nasal bridge, rounded lips, crackles in the lungs, and situs inversus. Whole exome sequencing revealed homozygous pathogenic mutations in two genes: a frameshift mutation in Dynein Axonemal Heavy Chain 5 (DNAH5), linked to PCD, and a null mutation in Forkhead Box E3 (FOXE3), ass

Modified ab externo 2-point pierce technique for scleral fixation of single-piece intraocular lenses.

Foldable acrylic lenses are the most extensively used 'in the bag' monofocal or multifocal intraocular lenses (IOLs) in cataract surgery, toric lenses in astigmatism, and refractive lens exchange surgeries. When subluxation or dislocation of these lenses is encountered, they must be explanted as they cannot be fixed with conventional scleral fixation techniques. The 'pierce' technique is a novel way of fixing these lenses to the sclera, wherein a 9-0/10-0 polypropylene suture with a straight needle is passed through the IOL at the haptic-optic junction. The ends of the threads are sutured unde
