Study protocol for the volume targeted mask ventilation versus pressure ventilation in preterm infants-the VOLT-trial.

BACKGROUND: The rapid establishment of gas exchange after birth is vital for survival and long-term health. When newborn infants fail to initiate spontaneous breathing, positive pressure ventilation (PPV) is the cornerstone of respiratory support immediately after birth. The aim of PPV is to inflate the lungs, create a functional residual capacity, deliver an adequate tidal volume (VT), facilitate gas exchange, and stimulate breathing, without causing lung or brain injury. In the delivery room, PPV is routinely provided via a pressure-limited device (called a T-Piece resuscitator), where an ar

Angiogenic factors and fetal Doppler for predicting adverse pregnancy outcome in early-onset small fetuses with and without pre-eclampsia.

OBJECTIVES: To assess the predictive performance of angiogenic factors and fetal Doppler, alone and in combination, for composite adverse perinatal outcome (CAPO) in early-onset small-for-gestational age (SGA) and fetal growth restriction (FGR), in cases both with and without pre-eclampsia (PE), in order to evaluate the ability of these markers to predict adverse outcomes beyond their established association with PE and to better delineate the specific contribution of PE to their predictive value. METHODS: This was a retrospective observational study conducted at Vall d'Hebron University Hosp

Apgar Score Plus Umbilical Artery pH and Adverse Neonatal Outcomes in Very Preterm Infants.

IMPORTANCE: The Apgar score, the first clinical assessment to direct measures to stabilize newborn infants, is also used for risk assessment. Its accuracy in estimating outcomes remains poor among very preterm (VPT) infants. OBJECTIVE: To assess the utility of the combined 5-minute Apgar score and umbilical artery pH (UA-pH) for estimating risks of mortality and severe neonatal morbidity among VPT infants. DESIGN, SETTING, AND PARTICIPANTS: This cohort study (Effective Perinatal Intensive Care in Europe [EPICE]) analyzed infants born at less than 32 weeks' gestation between April 2011 and Se

Outcomes and complications of invasive versus non-invasive respiratory support in extremely preterm infants: a retrospective study.

BACKGROUND: Because the respiratory system of extremely/very preterm infants is immature, respiratory support is essential to their care; however, the effects of invasive and non-invasive ventilation modes on prognosis and complications remain controversial. The efficacy and safety of invasive mechanical ventilation (IMV), nasal continuous positive airway pressure (NCPAP), and non-invasive high-frequency oscillatory ventilation (NHFOV) were to be compared in extremely/very preterm infants.  METHODS: Clinical data from 220 extremely/very preterm infants admitted between January 2024 and March 2

Azithromycin therapy for prevention of chronic lung disease of prematurity (AZTEC): a randomised placebo-controlled trial.

BACKGROUND: Systematic reviews have reported conflicting evidence to confirm if macrolides reduce rates of chronic lung disease of prematurity in at-risk preterm-born infants, including in those colonised with pulmonary Ureaplasma spp. Since an adequately powered trial has been lacking, we conducted a double-blind, randomised, placebo-controlled trial to assess if the macrolide azithromycin improved survival without the development of physiologically defined moderate or severe chronic lung disease of prematurity in infants born at < 30 weeks' gestation. METHODS: Infants recruited from 30 neon

Identifying the risk factors of autism spectrum disorders in infants born preterm: a systematic review and meta-analysis.

Compared with infants born at term, infants born preterm are more likely to develop autism spectrum disorder (ASD), but specific risk factors remain uncertain. To identify possible risk factors associated with ASD development in infants born preterm. We systematically searched PubMed, Embase, Web of Science, CINAHL and Scopus to May 2025 for observational studies reporting ASD in infants born preterm. Meta-analyses used RevMan 5.4 and Stata 14.0. We assessed certainty of the evidence using the GRADE approach. A total of 45 studies were included in the systematic review, of which 40 were includ

An unusual segmental retinal venous abnormality detected during retinopathy of prematurity screening in an infant with a PAX2 variant.

PURPOSE: Congenital abnormalities of retinal venous development are rare and may be difficult to distinguish from prematurity-related vascular changes when detected during retinopathy of prematurity screening. In premature infants, atypical posterior pole vascular findings should not be automatically attributed to retinopathy of prematurity alone. The purpose of this report is to describe an unusual segmental retinal venous abnormality detected during retinopathy of prematurity screening in a premature infant with a PAX2 variant and renal dysplasia. METHODS: We report a premature male infant

Optical Coherence Tomography-Based Differentiation of Nonarteritic Anterior Ischemic Optic Neuropathy and Central Retinal Artery Occlusion: Longitudinal Analysis of Inner Retinal Layer Thinning.

BACKGROUND: Nonarteritic anterior ischemic optic neuropathy (NAION) and central retinal artery occlusion (CRAO) are both causes of acute vision loss. When patients present within the first several weeks of symptom onset, clinical signs such as optic disc or inner retinal edema often allow differentiation. However, as time progresses and edema resolves, both diseases lead to optic atrophy and retinal thinning, sometimes making them difficult to distinguish. Clinical experience suggests that CRAO results in more significant thinning of the inner retinal layers, particularly the ganglion cell lay

Delayed choroidal perfusion: a fluorescein angiographic finding in retinopathy of prematurity.

PURPOSE: Retinopathy of prematurity (ROP) is the leading cause of childhood blindness worldwide. While choroidal involution has been described in animal models, choroidal perfusion in premature infants remains poorly characterized because of limited use of intravenous fluorescein angiography (IVFA). METHODS: This retrospective study systematically analyzed IVFAs performed post-treatment in patients with ROP at the Hospital for Sick Children between 2011 and 2020. Eligible patients had received intravitreal bevacizumab or laser photocoagulation, and only transiting eyes were included to allow

Data-Driven Selection of Parametric and Travelling Fire Models for Performance-based Fire Design.

INTRODUCTION: In Australia, as in many other developed countries, the current healthcare environment is characterised by increasing differentiation and patient acuity, aging of patients and workforce, staff shortages and a varied professional skills mix, and this is particularly so in rural areas. Rural healthcare clinicians are confronted with a broad range of challenges in their daily practice. Within this context, the challenges faced by rural acute care clinicians were explored and innovative strategies suggested. This article reports the findings of a study that explored these challenges

Fetal hemoglobin fraction is correlated to the risk of prematurity complications.

BACKGROUND: Fetal hemoglobin (HbF) plays a critical role in the progressive adaptation to the postnatal oxygen-rich environment in preterm infants due to its peculiar properties and its postnatal decrease has been associated to the combined adverse effects of increasing tissue hyperoxia and decreasing antioxidant defenses in preterm infants. PURPOSE: We aimed to assess the association between HbF fractions and the risk of bronchopulmonary dysplasia (BPD) intraventricular hemorrhage (IVH), and retinopathy of prematurity (ROP). METHODS: We studied 166 preterm infants with a gestational age of

A direct method for imaging gradient levels of retinal hypoxia in a model of retinopathy of prematurity (ROP).

BACKGROUND: Retinal hypoxia may contribute to the development of preretinal neovascularization (NV) in patients with retinopathy of prematurity (ROP). We hypothesized that levels of NV may be associated with levels of retinal hypoxia. Imaging retinal hypoxia could be an important diagnostic tool to predict levels of disease severity in ROP patients. In this study, we have investigated a direct method for imaging gradient levels of retinal hypoxia using a model of ROP. We believe that this discovery will help understand the ROP pathogenesis in premature infants. METHODS: The rat 50/10 oxygen-in

The Clinical Association of Maternal Asthma and the Development of Retinopathy of Prematurity in Preterm Infants.

Purpose: Retinopathy of prematurity (ROP) is a major cause of childhood blindness worldwide. Infant risk factors such as low birth weight, early gestational age, and oxygen exposure are strongly associated with development of ROP. Establishing possible maternal factors, including maternal asthma, may assist in quantifying preterm infants' risk of developing severe ROP. Methods: A retrospective observational study was performed using the records of the University of Colorado ROP registry from January 2006 to February 2024. Maternal asthma was identified upon review of patients' clinic notes. Od

Pulse Oximetry Histogram Profiles Before and After Red Blood Cell Transfusion in Very Preterm Infants: A Prospective Observational Cohort.

BACKGROUND/OBJECTIVES: Red blood cell (RBC) transfusion is frequently used to treat anemia of prematurity, yet bedside metrics that capture its short-term impact on oxygenation stability are limited. We assessed whether pulse oximetry histogram-derived oxygen saturation (SpO2) exposure changes after transfusion and whether responses differ across clinical subgroups. METHODS: This prospective observational cohort included preterm infants born <32 weeks' gestation who received a standardized RBC transfusion (15 mL/kg). Continuous SpO2 histograms quantified the percentage of monitored time spent
