Severe Permanent Visual Decline After Hypotony-Induced Retinal Pigment Epithelial Changes.

Postoperative hypotony accompanied by choroidal effusions and maculopathy are well-recognized complications following glaucoma shunt surgery. We report a rare case of an elderly patient with irreversible visual loss in his left eye (OS) caused by retinal pigment epithelium (RPE) changes secondary to short-term hypotony maculopathy and choroidal detachment after a Paul glaucoma drainage device implantation. An 83-year-old man presented with recurrent episodes of transient loss of vision in his OS. Examination of his OS revealed high intraocular pressure (IOP), a one-piece in-the-bag posterior c

Clinical Outcomes of Two Versions of the Ahmed Glaucoma Valve FP7 in Glaucoma Patients.

BACKGROUND: The primary goal of surgical treatment for glaucoma is the sustained reduction of intraocular pressure (IOP). The Ahmed® Glaucoma Valve (AGV) (New World Medical, Inc., Rancho Cucamonga, CA) is a surgically implanted drainage device that allows bidirectional aqueous humor outflow and IOP regulation. Although multiple generations of the AGV have been developed, comparative studies evaluating patient outcomes remain limited. This retrospective study aimed to compare clinical outcomes following implantation of two different generations of the AGV FP7 in patients with complicated, refra

Subconjunctival triamcinolone acetonide injection around lumen of XEN Gel Stent effectively resolves persistent choroidal effusion.

The XEN Gel Stent is a minimally invasive surgical implant for glaucoma designed to reduce intraocular pressure (IOP) while minimizing hypotony by creating an aqueous outflow pathway into the subconjunctival area. The XEN stent has been shown to reduce IOP effectively, with few adverse events reported; however, complications due to hypotony, such as choroidal detachment, may still occur. We report the case of an 86-year-old woman with primary open-angle glaucoma who underwent XEN stent placement followed by Nd:YAG laser lysis to clear the fibrin-obstructed lumen. Postoperatively, she experienc

Serous retinal detachment with retinal pigment epithelium tear after PreserFlo MicroShunt surgery: a case report.

BACKGROUND: The PreserFlo MicroShunt is a minimally invasive glaucoma drainage device designed to lower intraocular pressure (IOP) with fewer complications compared with those of traditional filtration surgery. Hypotony and choroidal detachment (CD) are known postoperative risks. However, retinal pigment epithelium (RPE) tears and serous retinal detachment following PreserFlo MicroShunt have not been reported previously. We report a case of retinal detachment with an RPE tear under hypotony following minimally invasive glaucoma surgery using a drainage device that required surgical interventio

Retinal Pigment Epithelial Tear and Retinal Detachment Following Hypotony After Trabeculectomy: A Case Report.

Hypotony after trabeculectomy (TLE) can lead to choroidal detachment (CD) and maculopathy. We present a case of retinal detachment caused by extensive retinal pigment epithelial (RPE) tears following hypotony following TLE surgery. A 67-year-old man underwent TLE in the left eye for open-angle glaucoma. Postoperatively, intraocular pressure (IOP) remained stable at approximately 10 mmHg. However, after 50 days, the IOP decreased due to aqueous humor leakage, which required several conjunctival sutures. CD occurred on day 87. On day 108, retinal detachment was detected in the inferior retina of

Choroidal Detachment Secondary to Scleral Wound Leak After Ozurdex® Implantation in an Eye With Previous Vitrectomy: A Case Report.

Retinal vein occlusion (RVO) is a major cause of visual morbidity, and macular oedema secondary to RVO is increasingly managed with repeated dexamethasone intravitreal implant (Ozurdex®) therapy, particularly in eyes with suboptimal response to anti-vascular endothelial growth factor (VEGF) agents. While its efficacy is well established, the long-term cumulative risk to scleral integrity is less well characterised, especially in post-vitrectomy eyes. We report a case of an 86-year-old woman with a history of pars plana vitrectomy and recurrent macular oedema from branch RVO, treated over sever

Surgical Management of Cyclodialysis Cleft: An Update.

UNLABELLED: A cyclodialysis cleft develops when the meridional fibers of the ciliary muscle detach from the scleral spur, creating an aberrant aqueous drainage pathway between the anterior chamber and suprachoroidal space. Blunt ocular trauma and intraocular surgery have been identified as the most frequent causes. Cyclodialysis has been documented after phacoemulsification, extracapsular cataract extraction, secondary intraocular lens (IOL) implantation, goniotomy, trabeculotomy, trabeculectomy, and other intraocular surgeries. The new aqueous drainage pathway may lead to persistent severe oc

A NEW TECHNIQUE OF STERILE POLYETHYLENE INSTRUMENT COVER AS TEMPORARY CORNEAL PATCH TO RECONSTRUCT ANTERIOR CHAMBER IN PENETRATING KERATOPLASTY COMBINED WITH INTRAOCULAR SURGERY.

PURPOSE: Complications such as explosive choroidal hemorrhage, residual cortex, and capsule rupture often occur during intraocular surgery combined with penetrating keratoplasty (PKP) due to poor maintenance of the anterior chamber. To address these challenges, the authors have innovatively used a sterile polyethylene instrument cover to temporarily reconstruct the anterior chamber. METHODS: In this report, the authors describe a technique where a "temporary corneal patch" was created from a sterile instrument cover, using a trephine to ensure a diameter approximately 1 mm to 2 mm wider than

Application of the Technique of Inferior Temporal Gyrus Resection for Microsurgical Treatment of Arteriovenous Malformation of the Fusiform Gyrus.

Arteriovenous malformations (AVMs) consist of multiple connections between arteries and veins without a capillary bed and are characterized by an early draining vein.1,2 This pathology affects mainly young adults, and the morbimortality is caused mostly by rupture and hemorrhage.3-6 In this Video 1, we present a case of a 29-year-old female patient presenting with a sudden severe headache. Magnetic resonance imaging demonstrated the presence of a left AVM located at the fusiform gyrus, above the tentorium cerebelli, and angiography depicted those temporal branches of the left middle cerebral a

Inflammatory changes in the choroid plexus following subarachnoid hemorrhage: the role of innate immune receptors and inflammatory molecules.

INTRODUCTION: The choroid plexus is located in the cerebral ventricles. It consists of a stromal core and a single layer of cuboidal epithelial cells that forms the blood-cerebrospinal barrier. The main function of the choroid plexus is to produce cerebrospinal fluid. Subarachnoid hemorrhage due to aneurysm rupture is a devastating type of hemorrhagic stroke. Following subarachnoid hemorrhage, blood and the blood degradation products that disperse into the cerebrospinal fluid come in direct contact with choroid plexus epithelial cells. The aim of the current study was to elucidate the pathophy
