An Anatomy-Guided, Stepwise Microsurgical Reconstruction of a Posteriorly Projecting ICA-PCoA Aneurysm Beneath the Optic Apparatus: A Detailed Operative Sequence.

Background: Posteriorly directed aneurysms at the internal carotid-posterior communicating artery (ICA-PCoA) junction concentrate technical risk at the posteromedial neck where the PCoA origin and perforators exist beneath the optic apparatus. Our aim was to describe, in a reproducible fashion, an anatomy-driven sequence in the management of a ruptured ICA-PCoA aneurysm that visualized the posterior wall and a closing line parallel to the PCoA axis and which is placed within contemporary practice. Case Presentation: This is a single case study employing predetermined surgical techniques demons

Morphological characteristics of choroidal anastomosis as predictors of subsequent rupture in adult moyamoya disease: a longitudinal study using vessel wall imaging.

OBJECTIVE: Choroidal anastomosis (ChA) is considered a key indicator of increased hemorrhagic risk in adult moyamoya disease (MMD); however, its clinical course remains unclear. The aim of this study was to identify potential predictors of ChA rupture. METHODS: Retrospective evaluation of clinical and radiological data from 132 ChA-positive hemispheres (86 patients with MMD) treated at a single institution from March 2019 to March 2024 was conducted on a hemisphere-by-hemisphere basis. Two independent raters investigated the morphological characteristics of the ChA, including the lumen area (

Adult subarachnoid hemorrhage from vein of Galen aneurysmal malformation due to ruptured feeder aneurysm: A case report with long-term MRI review.

Adult hemorrhage from a previously treated vein of Galen aneurysmal malformation (VGAM) is exceedingly rare, and rupture of a flow-related feeder aneurysm has only been documented once in the adult case. We report a 20-year-old man with subarachnoid hemorrhage who presented with sudden headache. He had undergone palliative transarterial embolization (TAE) for choroidal-type VGAM during infancy for several times and had been followed annually with MRI at another institution. Digital subtraction angiography demonstrated two small feeder aneurysms, one presumed to have ruptured; additional emboli

Retrospective Study of 32 Kissing Aneurysms: Radiological Detection, Microsurgical Clipping, and Functional Outcomes.

INTRODUCTION: Kissing aneurysms (KA) are a rare variant of multiple intracranial aneurysms characterized by two adjacent aneurysms arising from separate vascular origins with adherent domes. Their atypical morphology often leads to misinterpretation as a single bilobulated aneurysm, which complicates diagnosis and surgical planning. Preoperative distinction between the aneurysms is especially crucial when rupture occurs. This study aimed to analyze the clinical, radiological, and surgical characteristics of 32 patients with KA treated microsurgically over two decades and provide a narrative re

Transection of the Vestigial Anterior Communicating Artery Limb during Microsurgical Clip Ligation of a Large, Ruptured Anterior Communicating Artery Trifurcation Aneurysm.

Unrecognized anatomic variants of the anterior communicating artery (AComm) complex can increase periprocedural risk during aneurysm treatment. A trifurcated AComm complex remains rare, with an incidence below 1%. Furthermore, vestigial AComm limbs may be difficult to appreciate on preoperative imaging. This video illustrates the intraoperative identification of a vestigial AComm limb and its transection to visualize and effectively clip a large, ruptured AComm trifurcation aneurysm. A middle-aged female presented with subarachnoid hemorrhage secondary to an 11-mm AComm aneurysm and was also f

Hypertensive Bilateral Thalamic Hemorrhage: Distribution of Ruptured Blood Vessels and Hematoma and Considerations on the Mechanism of Vascular Rupture.

The ruptured blood vessels of a 57-year-old Japanese man who had died 5 days after bilateral hypertensive thalamic hemorrhage (HTH) were investigated by preparing 9 paraffin-embedded tissue blocks, all containing the hematomas. Each block was cut serially into 6-μm-thick sections. The first of every 18 sections was stained with Victoria blue and hematoxylin-eosin, the second with elastica-Goldner, and the third with phosphotungstic acid-hematoxylin. Several additional stainings, including immunostaining for α-smooth muscle actin and synaptophysin, were performed as necessary. In the large hema

A ruptured blood blister like aneurysm in internal carotid artery communicating segment: An illustrated case.

Blood blister-like aneurysms (BBAs) of the internal carotid artery (ICA) are rare, fragile lesions associated with high bleeding and present technical challenges in management. BBAs of the ICA communicating segment are particularly uncommon and complex because they require preservation of critical branches, including the posterior communicating and anterior choroidal arteries. We report a case of a 34-year-old man with a subarachnoid hemorrhage from a ruptured BBA of the right ICA communicating segment, successfully treated with stent-assisted coiling (SAC). Four hours post-administration of l

Vitrectomy with Low-Dose Subretinal t-PA for Hemorrhage Due to Traumatic Choroidal Rupture: A Case Series.

BACKGROUND: Choroidal rupture with subretinal hemorrhage comprising the fovea is thought to have poor prognosis regardless of surgical interventions. Being a relatively rare entity, managing patients, referring them in time and standardizing a therapeutic approach remains challenging. Many eyecare practitioners opt solely for conservative treatment, without recurring to any surgical treatment. CASE REPORTS: Three young adult males with blunt ocular trauma complicated by choroidal rupture with subretinal hemorrhage presented to the vitreoretinal surgeon appointment. Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

A NEW TECHNIQUE OF STERILE POLYETHYLENE INSTRUMENT COVER AS TEMPORARY CORNEAL PATCH TO RECONSTRUCT ANTERIOR CHAMBER IN PENETRATING KERATOPLASTY COMBINED WITH INTRAOCULAR SURGERY.

PURPOSE: Complications such as explosive choroidal hemorrhage, residual cortex, and capsule rupture often occur during intraocular surgery combined with penetrating keratoplasty (PKP) due to poor maintenance of the anterior chamber. To address these challenges, the authors have innovatively used a sterile polyethylene instrument cover to temporarily reconstruct the anterior chamber. METHODS: In this report, the authors describe a technique where a "temporary corneal patch" was created from a sterile instrument cover, using a trephine to ensure a diameter approximately 1 mm to 2 mm wider than

Application of the Technique of Inferior Temporal Gyrus Resection for Microsurgical Treatment of Arteriovenous Malformation of the Fusiform Gyrus.

Arteriovenous malformations (AVMs) consist of multiple connections between arteries and veins without a capillary bed and are characterized by an early draining vein.1,2 This pathology affects mainly young adults, and the morbimortality is caused mostly by rupture and hemorrhage.3-6 In this Video 1, we present a case of a 29-year-old female patient presenting with a sudden severe headache. Magnetic resonance imaging demonstrated the presence of a left AVM located at the fusiform gyrus, above the tentorium cerebelli, and angiography depicted those temporal branches of the left middle cerebral a

Inflammatory changes in the choroid plexus following subarachnoid hemorrhage: the role of innate immune receptors and inflammatory molecules.

INTRODUCTION: The choroid plexus is located in the cerebral ventricles. It consists of a stromal core and a single layer of cuboidal epithelial cells that forms the blood-cerebrospinal barrier. The main function of the choroid plexus is to produce cerebrospinal fluid. Subarachnoid hemorrhage due to aneurysm rupture is a devastating type of hemorrhagic stroke. Following subarachnoid hemorrhage, blood and the blood degradation products that disperse into the cerebrospinal fluid come in direct contact with choroid plexus epithelial cells. The aim of the current study was to elucidate the pathophy
