Toward changing of the pathophysiologic basis of acute hydrocephalus after subarachnoid hemorrhage: a preliminary experimental study.

BACKGROUND: Acute hydrocephalus (ventricular enlargement within 72 hours) is a common complication in patients with aneurysmal subarachnoid hemorrhage (SAH). Cerebrospinal fluid (CSF) secretion may be increased in the early phases of SAH, but it has not been proved definitively. We studied the histologic features of choroid plexus (CP) in the early and late phases of SAH. METHODS: This study was conducted on 20 rabbits, with 5 rabbits in the control group, 5 rabbits in the sham group, and 10 rabbits in the SAH group. In the SAH group, five of the animals were decapitated after 2 days of ciste

Imaging in orbital trauma.

Orbital trauma is one of the most common reasons for ophthalmology specialty consultation in the emergency department setting. We survey the literature from 1990 to present to describe the role of computed tomography (CT), magnetic resonance imaging (MRI) and their associated angiography in some of the most commonly encountered orbital trauma conditions. CT orbit can often detect certain types of foreign bodies, lens dislocation, ruptured globe, choroidal or retinal detachments, or cavernous sinus thrombosis and thus complement a bedside ophthalmic exam that can sometimes be limited in the set

Endovascular coil embolization for ruptured aneurysm associated with persistent primitive anterior choroidal artery--case report.

An 18-year-old man presented with a rare case of a ruptured internal carotid artery (ICA)-persistent primitive anterior choroidal artery (PPAchA) manifesting as sudden onset of headache. Computed tomography (CT) showed subarachnoid hemorrhage. Three-dimensional CT angiography showed a saccular aneurysm at the right ICA-AchA region. Right internal carotid angiography showed a PPAchA and saccular aneurysm. Endovascular treatment of the aneurysm achieved complete aneurysm occlusion.

SD-OCT to differentiate traumatic submacular hemorrhage types using automatic three-dimensional segmentation analysis.

Traumatic submacular hemorrhage may present with significant decrease in vision and may have varying outcomes. Following injury, the hemorrhage can collect either between the neurosensory retina and retinal pigment epithelium (RPE) or below the RPE. This differentiation may be important to prognosticate and to guide treatment. In two patients with post-traumatic submacular hemorrhage, Cirrius spectral domain high-definition optical coherence tomography (OCT) (Carl Zeiss Meditec, Dublin, CA) was used to differentiate traumatic submacular hemorrhage types using automation three-dimensional segme

Unusual cisternal coil migration in the follow-up period after aneurysm perforation during endovascular coiling.

A 61-year-old woman suffered cisternal coil migration in the follow-up period after endovascular coil embolization for a ruptured cerebral aneurysm. She presented with sudden onset of headache. Computed tomography demonstrated diffuse subarachnoid hemorrhage, and cerebral angiography disclosed a left anterior choroidal artery aneurysm. The aneurysm was treated by endovascular embolization with Guglielmi detachable coils. During the embolization procedure, the microcatheter perforated the aneurysm. For direct closure of the perforation site with coils, the microcatheter was withdrawn and coils

Experience with coil embolization of previously clipped aneurysms presenting with rupture.

INTRODUCTION: Endovascular coil embolization has an established role alongside microsurgical clipping in the treatment of aneurysms. We studied previously clipped aneurysms that presented as subarachnoid hemorrhage and were treated by coil embolization. METHODS: A retrospective review was performed of two prospectively maintained databases from two institutions (Cleveland Clinic, Emory University) that spanned 12 years. RESULTS: Seven patients were identified (mean age 56.9 years) who had previously undergone surgical clipping for aneurysm obliteration; six (86%) were previously ruptured. Pa

[Use of intravitreal bevacizumab for the treatment of choroidal neovascularization secondary to choroidal rupture].

CASE REPORT: A 28 year-old male attended our Emergency Department with a traumatic choroidal rupture and macular haemorrhage. After pneumatic displacement of the haemorrhage with C(3)F(8) and tissue plasminogen activator, the haemorrhage was reabsorbed and visual acuity (VA) improved. Three months later the patient presented with decreased VA and a juxtafoveal choroidal neovascularisation (CNV) that was treated with intravitreal bevacizumab. One year after a single bevacizumab injection the CNV remained inactive, with a final VA of 0.5. DISCUSSION: Intravitreal bevacizumab injection is a new

Distal anterior choroidal artery aneurysm following iatrogenic posterior cerebral artery occlusion : a case report and review of literature.

Aneurysms of the anterior choroidal artery are uncommon and account for only 2-5% of all intracranial aneurysms. Distal anterior choroidal artery aneurysms are rare and the pathogenesis of this aneurysm may be different from typical proximal ones. We describe an unusual case of ruptured de novo distal anterior choroidal artery aneurysm in a 55 year-old man who had previously undergone endovascular proximal occlusion for a fusiform aneurysm of the ipsilateral posterior cerebral artery (PCA). A comprehensive review of literature using Medline, PubMed and all related journals was also performed.

Ruptured aneurysm originating from an aberrant medullary branch of the proximal posterior inferior cerebellar artery: case report.

OBJECTIVE: Aneurysms that arise from a small branch of the posterior inferior cerebellar artery (PICA) are very rare. All reported PICA branch aneurysms originated from the choroidal branches or PICA communicating branches. No aneurysms of the PICA medullary branch have been reported previously. CLINICAL PRESENTATION: A 62-year-old woman presented with a subarachnoid hemorrhage and intraventricular hemorrhage. Angiography showed a globular-shaped aneurysm arising from an aberrant branch of the anterior medullary segment of the left PICA. At the time of operation, the arterial branch was found

[The other pigmented lesions].

Not all intraocular pigmented lesions are melanomas or nevi. In this article, we present what we classically consider to be the main pigmented stains other than melanomas and nevi and their possible forms of treatment. Regarding tumor-related stains, we describe melanocytomas, congenital hypertrophy or reactive hyperplasia of the retinal pigment epithelium, combined hamartoma of the retina and retinal pigment epithelium, and acquired vascular pseudotumors. Among the hemorrhage-related stains, we present pseudotumoral age-related macular degeneration, peripheral exudative and hemorrhagic chorio

Ruptured internal carotid artery aneurysm at the origin of a perforating artery associated with a hyperplastic anomalous anterior choroidal artery--case report.

A 65-year-old man presented with a hyperplastic anomalous anterior choroidal artery (AChA) associated with a ruptured internal carotid artery aneurysm at the origin of a perforating artery manifesting as sudden onset of headache and vomiting. The aneurysm was too small for endovascular embolization, so we performed open surgery via the left pterional approach. Aneurysm clipping with preservation of the perforator was impossible, so we clipped the aneurysm neck and sacrificed the perforator. We also performed dome clipping because dome puncture resulted in continuous bleeding. Head computed tom

Aneurysm of the distal anterior cerebral artery radiologically mimicking a ventricular mass.

The ventricular system is a rare localization for intracranial aneurysms. Most ventricular aneurysms arise from a distal branch of the choroidal arteries and a major branch point of the circle of Willis. A 41-year-old-man suffering from dizziness of 2 weeks' duration was admitted to the clinic. On radiological examination, he had a well-circumscribed mass involving the frontal horn of the right lateral ventricle without radiological evidence of a prior or recent hemorrhage. Localization and radiological appearance were not typical of a ventricular mass and did not allow diagnosis. After cerebr

Choroidal neovascularization in a patient with blunt trauma-caused traumatic retinopathy without choroidal rupture.

PURPOSE: To report a case of choroidal neovascularization (CNV) following blunt trauma without choroidal rupture, with past history of central serous chorioretinopathy (CSC). METHODS: A 43-year-old man with a nonperforating injury caused by a baseball that hit his right eye exhibited traumatic retinopathy. The damage was localized to the outer layer of the retina at the macula and there was no choroidal rupture. Five years prior to the accident, the patient suffered from a CSC attack with retinal pigment epithelium detachment. Two weeks after the blunt trauma, fundus examination revealed CNV

An aneurysm rupturing into a middle cranial fossa arachnoid cyst presenting as an intracystic hemorrhage.

We report a case of an aneurysm rupturing into an arachnoid cyst presenting as a confined intracystic hemorrhage. A 37-year-old man with a history of sporadic headaches for several years complained of a headache of several days' duration. Brain computed tomography and magnetic resonance imaging revealed a cystic lesion in the left side of the middle cranial fossa that included a 3-cm-diameter hematoma in the Sylvian fissure without extracystic hemorrhage. Left carotid artery angiography showed an internal carotid artery-posterior communicating artery (IC-PC) aneurysm and an anterior choroidal

Microneurosurgical management of anterior choroid artery aneurysms.

OBJECTIVE: Anterior choroid artery aneurysms (AChAAs) constitute 2%-5% of all intracranial aneurysms. They are usually small, thin walled with one or several arteries originating at their base, and often associated with multiple aneurysms. In this article, we review the practical microsurgical anatomy, the preoperative imaging, surgical planning, and the microneurosurgical steps in the dissection and the clipping of AChAAs. METHODS: This review, and the whole series on intracranial aneurysms (IAs), are mainly based on the personal microneurosurgical experience of the senior author (J.H.) in t
