Sight-saving outcome with thrombolysis in central retinal artery occlusion.

'Time is brain' is an important concept in ischaemic brain stroke, where expeditious intervention is essential for the preservation of neural tissue. Therapeutic strategies involve the prompt administration of thrombolytic agents and in select circumstances the implementation of mechanical thrombectomy. Central retinal artery occlusion (CRAO) constitutes an ophthalmological emergency and is a form of ischaemic stroke, frequently resulting in profound and irreversible visual impairment. There are currently no universally accepted guidelines delineating the optimal therapeutic time window. In th

Intravenous thrombolysis for acute central retinal artery occlusion: Protocol for a systematic review and individual participant data meta-analysis of randomized controlled trials.

IMPORTANCE: Central retinal artery occlusion (CRAO) is a disabling subtype of acute ischemic stroke. It is not known whether intravenous (IV) thrombolysis delivered within 4.5 hours of time last known well (LKW) improves visual outcomes. STUDY DESIGN: Systematic review and individual participant data meta-analysis. OBJECTIVE: The objective of this study is to determine whether IV thrombolysis improves visual outcomes among patients with acute non-arteritic CRAO when administered within 4.5 hours of time LKW compared with placebo, no IV thrombolysis, and/or anti-thrombotic therapy. EVIDENCE

Management of Visual Loss Following Hyaluronic Acid Filler Injections: An Algorithm-based Approach.

BACKGROUND: Hyaluronic acid (HA) filler injections have been associated with the rare but vision-threatening complication of retinal artery embolism. Clinical management of this condition remains challenging due to the lack of high-level evidence. Randomized controlled trials are neither ethically justifiable nor practically feasible, and current treatment recommendations are derived almost exclusively from individual case reports and small case series. METHODS: The aim of this article is to offer evidence-informed recommendations for the management of central retinal artery occlusion followi

The Underutilized Ocular Fundus in Emergency Departments: Current Progress and Future Prospect of Imaging and Artificial Intelligence for Patient Care.

CLINICAL RELEVANCE: The retina is a unique and accessible window to the central nervous system and its vasculature. Ocular fundus examination is crucial for identifying diagnostic red flags indicative of life- and sight-threatening conditions, such as papilledema and central retinal artery occlusion. In the emergency department (ED), prompt recognition of these conditions is essential to reduce the risk of vision loss and serious complications. METHODS: This review summarizes advances in ocular fundus examination, imaging, and artificial intelligence (AI) for emergency care. RESULTS: The dir

Simultaneous Bilateral Central Retinal Artery Occlusions: Partial Recovery With Multimodal Therapy.

Central retinal artery occlusion (CRAO) is a rare cause of sudden, profound vision loss, and bilateral involvement is exceptionally uncommon, with no clearly established optimal management. We describe a patient who developed near-simultaneous, painless vision loss in both eyes and underwent urgent multidisciplinary assessment followed by systemic and ocular therapies intended to restore perfusion and limit ischemic injury. Although partial visual recovery was achieved, the course was complicated by retinal neovascularization requiring treatment and resulting in severe long-term visual impairm

Fenofibrate in ophthalmology: therapeutic efficacy and mechanisms.

Ophthalmic diseases are a growing global cause of visual impairment, driven by population aging and the rising prevalence of diabetes mellitus. Fenofibrate, a peroxisome proliferator-activated receptor alpha (PPARα) agonist widely used for dyslipidemia, has emerged as a candidate for therapeutic repurposing in ophthalmology. This narrative review synthesizes current preclinical and clinical evidence on fenofibrate across anterior and posterior segment diseases, including diabetic corneal neuropathy, diabetic keratopathy, dry eye disease, corneal burns, Fuchs endothelial corneal dystrophy, diab

Systemic Inflammation and Structural Retinal Damage on OCT in Acute Non-Arteritic Central Retinal Artery Occlusion-A Retrospective Study.

Central retinal artery occlusion (CRAO) is an acute ophthalmic emergency causing irreversible vision loss due to retinal ischemia. Inflammation is increasingly recognized as a contributor to vascular occlusions, but the relationship between systemic inflammatory biomarkers and structural retinal damage in CRAO remains unclear. This retrospective study included 37 patients with non-arteritic CRAO presenting within 168 h of symptom onset. Optical coherence tomography (OCT) classified ischemic severity as mild, moderate, or severe based on inner retinal morphology. Systemic inflammatory indices,

Incidence or Coincidence: Subaortic Membrane, Bicuspid Aortic Valve, and Aortic Root Dilatation in a Patient Presenting With Stroke.

Bicuspid aortic valve (BAV) is the most common congenital cardiac malformation and is frequently associated with progressive aortopathy. Subaortic membrane, a discrete form of left ventricular outflow tract (LVOT) obstruction, is uncommon in adults and rarely coexists with BAV outside of complex congenital syndromes. The simultaneous presence of BAV, aortic root dilation, and a subaortic membrane represents an unusual anatomical constellation with important diagnostic and clinical implications. We present the case of a 49-year-old man with no known medical history who developed sudden, painles

Perioperative visual loss after spine surgery: a systematic review of risk factors and report of a reversible case.

BACKGROUND: Perioperative visual loss (POVL) is a rare but devastating complication of spinal surgery, most commonly associated with posterior ischemic optic neuropathy (PION) and central retinal artery occlusion (CRAO). Its pathophysiology remains multifactorial and incompletely understood. METHODS: A systematic review of the English-language literature was conducted using MEDLINE/PubMed and Scopus according to PRISMA guidelines. Studies reporting POVL after spinal surgery were included if they provided sufficient clinical, surgical, intraoperative, or outcome data. An illustrative reversibl

Ophthalmic artery occlusion following local hyaluronic acid injection: A case report.

RATIONALE: Ophthalmic artery occlusion (OAO) following hyaluronic acid (HA) injection is a rare but devastating complication of facial aesthetic procedures, often resulting in profound and irreversible visual loss. Due to its rarity and poor prognosis, standardized management of iatrogenic OAO remains limited. To our knowledge, long-term multimodal imaging follow-up of HA-induced OAO with integrated traditional Chinese and Western medicine treatment is rarely documented. Here, we report a case of OAO following nasal root HA injection and describe its clinical manifestations, multimodal imaging
