Painful vision loss during air travel after vitrectomy with air tamponade: a case report.

BACKGROUND: While the potentially hazardous effects of intraocular perfluorocarbon gases during air travel have been recognized, the equivalent risk of intraocular air tamponade is less known and has, to the best of our knowledge, not been reported yet. CASE PRESENTATION: A 52-year-old white female experienced a complete loss of vision and pain in her left eye during air travel following pars plana vitrectomy with air tamponade. Clinical and multimodal imaging findings only a few hours after emergency landing indicated a transient central retinal artery occlusion due to a significant increase

Visual outcomes of central retinal artery occlusion: Exploring treatment strategies beyond the conventional time window.

BACKGROUNDS/AIMS: Central retinal artery occlusion (CRAO) is a vision-devastating emergency. However, widely-acknowledged treatment consensus is lacking and prehospital delays commonly occur. Hence, we aimed to investigate the visual outcomes of conservative treatments (CT), local intra-arterial fibrinolysis (LIF) and hyperbaric oxygen (HBO) therapy for non-arteritic CRAO (NA-CRAO) patients beyond the conventional time window. METHODS: This retrospective comparative study included 99 NA-CRAO patients followed up for over 6 months. The subjects were divided into three groups: the CT (50 patien

Ankle-Brachial Index as a Predictor of Acute Ischemic Cerebrovascular Event After Central Retinal Artery Occlusion.

BACKGROUND Identifying patients at higher risk of acute ischemic cerebrovascular events (AICE) following central retinal artery occlusion (CRAO) is crucial for secondary prevention of stroke. This study aimed to investigate whether a low ankle-brachial index value is associated with an increased risk of AICE after CRAO. MATERIAL AND METHODS We prospectively analyzed patients who were admitted to our hospital because of CRAO between February 2019 and March 2023 and whose ankle-brachial index was no greater than 1.40. We explored the potential association between the index and risk of AICE (defi

Risk factors for visual loss after excision of orbital cavernous venous malformations: a systematic review.

BACKGROUND: Orbital cavernous venous malformations (OCVMs) are the most common primary orbital mass lesion and presenting symptoms are usually secondary to a mass effect. Surgical excision presents unique challenges and vision loss is a rare, but devastating, complication. This review aims to identify risk factors for vision loss with excision of OCVMs. METHOD: A systematic search of the databases PubMed/MEDLINE, Embase and CENTRAL was performed to May 2024, prior to data collection and risk of bias analysis in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Anal

Sequential central retinal artery occlusion in two brothers: a fight to prevent blindness.

IMPORTANCE: Central retinal artery occlusion (CRAO) is typically associated with older patients with cardiovascular risk factors. However, its occurrence in younger patients without these risk factors suggests the need to explore rare genetic conditions. Identifying genetic disorders like adenosine deaminase 2 deficiency (DADA2), a vasculitic disease, can be critical in such cases to prevent further complications. OBJECTIVE: To report the challenging diagnosis of two cases of CRAO in brothers under the age of 40, leading to the diagnosis of DADA2, a rare genetic vasculitic disorder. RESULTS:

Bilateral retinal artery occlusion as the initial presentation of infectious endocarditis: a case report.

BACKGROUND: Retinal artery occlusion due to infective endocarditis is rare, and only a few cases have been reported in literature. Systematic and comprehensive studies of patients with this ophthalmological disease may help uncover serious underlying medical conditions. CASE PRESENTATION: We present a case of an 80-year-old Brazilian woman with bilateral retinal artery occlusion as the initial presentation of Erysipelothrix rhusiopathiae endocarditis. CONCLUSION: Infective endocarditis has many different forms of presentation and a high clinical suspicion is often required to reach a diagnos

Cardioembolic stroke secondary to a cardiac myxoma.

We present the case of a young male who developed a cardioembolic stroke secondary to a cardiac myxoma, a rare but important cause of ischemic stroke, especially in patients without traditional stroke risk factors. The patient, a 32-year-old male, initially presented with altered consciousness and unilateral hemiplegia, with early neuroimaging showing no significant findings; however, follow-up imaging revealed bilateral, multi-territorial ischemic infarctions. Diagnosis was confirmed through echocardiography, cardiac magnetic resonance imaging (CMR) and histopathology, and the patient later e
