[The significance of ciliary circulation in the development of iris neovascularisation (author's transl)].

Three cases of peripapillary choroidal filling defects demonstrated by fluorescein angiography are presented. In two cases the defect was interpreted as an occlusion of a posterior ciliary artery. One of these patients had neovascularization of the iris and ischemia of the upper half of the retina. Later a contralateral hemiplegia developed. The other patient had no other known vascular disease of the eye except occlusion of the posterior ciliary artery by giant cell arteritis. One patient had underfilling of the peripapillary choroid which was interpreted as insufficiency of the ciliary circu

Central retinal artery spasm.

Central retinal artery spasm, a seldom witnessed ocular sign, has been recognized in one eye of a patient complaining of transient monocular blindness. Removal of an ulcerative atheromatous plaque of the carotid artery on the same side as the eye with central retinal artery spasm resulted in complete disappearance of symptoms. No other signs of ocular pathology were noted in addition to the central retinal artery spasm. Central retinal artery spasm occurring in both eyes simultaneously may be the mechanism explaining the visual loss of amaurosis fugax. Physiologic mechanisms associating brief

Recurrent stenosis after carotid endarterectomy.

Thirteen of 1,250 patients required a second operation for recurrent stenosis following carotid endarterectomy performed at the Cleveland Clinic between 1958 and 1978. Two other patients underwent reoperation because of recurrent stenosis following primary operations at other institutions. Thirteen of the 15 patients experienced neurologic symptoms caused by recurent stenosis, while two patients remained asymptomatic. Atherosclerosis was responsible for recurrent stenosis in 12 patients and appeared to be related to hypercholesterolemia. Three of the patients had myointimal fibroplasia. Eleven

Platelet coagulant activities in arterial occlusive disease of the eye.

Ischemic optic neuropathy and retinal arterial occlusion are 2 forms of arterial occlusive disease affecting the eye. Reports in the literature suggest platelet hyperactivity in acute arterial occlusive diseases affecting other organ systems. Therefore, 14 patients with ischemic optic neuropathy and 17 patients with central or branch retinal artery occlusion were studied to determine whether platelets have a role in the pathogenesis of these vascular occlusive disorders. The results of the following investigations were no different in these patients compared with those in 18 control patients w

Controversies regarding giant cell (temporal, cranial) arteritis.

It is important to establish the diagnosis of temporal arteritis because the disease is treatable; treatment may prevent blindness and even death. Temporal arteritis usually occurs in people older than 51 years of age, although very rarely, histologically documented disease occurs in younger people. The onset may be occult, so that there are few findings. A multitude of signs and symptoms may occur such as fever, headaches, malaise, weight loss, anemia, stroke, cranial nerve palsies, polymyalgia rheumatica, aortitis and other large vessel involvement. The eye may suffer from ischemic optic neu

Studies on acute and late stages of experimental central retinal artery occlusion in the Cynomolgus monkey. I. Intensity-amplitude relations of the D.C. recorded ERG with special reference to the c-wave.

The main positive component of the c-wave of the ERG is generated by the pigment epithelium-receptor complex, which is supplied from the choroidal circulation. Occlusion of the central retinal artery (OCRA) causes serious morphological damage only to the inner retina. An effect on the c-wave is therefore not primarily to be expected. In this study on five Cynomolgus monkeys with unilateral, laser-induced OCRA the c-wave was decreased, slightly in the early stage and markedly in the late stage. A hypothetical explanation of the reduced c-wave amplitude, involving changes in potassium ion concen

Experimental retinal vascular occlusion. I. Pathogenesis of central retinal vein occlusion.

In 45 eyes of rhesus monkeys, five types of central retinal vascular occlusion were produced by lateral orbitotomy: group 1, central retinal vein occlusion (CRVO) alone; group 2, CRVO with simultaneous central retinal artery (CRA) occlusion; group 3, CRVO with transient clamping of the CRA for either 2 to 2 1/2 hours (group 3A) or 6 to 7 1/2 hours (group 3B); and group 4, CRVO with segmental retinal ischemia. The eyes were examined by fundus photography and fluorescein angiography for up to nine months. Group 1 developed venous stasis retinopathy (VSR) and group 3B hemorrhagic retinopathy (HR)

The c-wave of the human electroretinogram in central retinal artery occlusion.

The pigment epithelium-receptor complex, which is supplied from the choroidal circulation, is the main source of the c-wave of the ERG. Occlusion of the central retinal artery (OCRA), which causes serious damage to the inner retinal layers, should therefore not primarily be expected to affect the c-wave amplitude. Nevertheless, conflicting reports of increased, decreased and unchanged c-waves have appeared in the literature. In the present study on four patients with OCRA the affected eyes showed diminished b- and c-waves. On the other hand, both the a-wave and the trough between the b- and c-

Bilateral retinal artery and choriocapillaris occlusion following the injection of long-acting corticosteroid suspensions in combination with other drugs: I. Clinical studies.

Two well-documented cases of bilateral retinal artery and choriocapillaris occlusions with blindness following head and neck soft-tissue injection with methylprednisolone acetate in combination with lidocaine, epinephrine, or penicillin are reported. One case had only a unilateral injection. The acute observations included hazy sensorium, superior gaze palsy, pupillary abnormalities, and conjunctival hemorrhages with edema. Follow-up changes showed marked visual loss, constricted visual fields, optic nerve pallor, vascular attenuation, and chorioretinal atrophy. The literature is reviewed, and

Bilateral retinal artery and choriocapillaris occlusion following the injection of long-acting corticosteroid suspensions in combination with other drugs: II. Animal experimental studies.

An experimental dog model was used to reproduce the clinical picture of bilateral arteriole and choriocapillaris occlusion from a unilateral intracarotid injection of long-acting corticosteroids combined with other drugs including lidocaine, epinephrine, and penicillin. All five known long-acting corticosteroids, when combined with 1:1000,000 epinephrine, elicited vasoconstriction followed by dilatation of the retinal vessels with particulate material occlusion of the retinal vessels and choriocapillaris associated with small retinal hemorrhages. The fluorescein picture is described. The parti

Central retinal vascular occlusion associated with oral contraceptives.

A 40-year-old woman took oral contraceptives for four years. She developed a vascular glaucoma and one year later the eye was enucleated. No other apparent cause for the occlusion was present. The vascular occlusion consisted of a combined central retinal artery and vein occlusion with severe intimal hyperplasia. An intravascular granulomatous reaction is attributed to chronic vascular injury.

Dissociation of the vitreo-retinal junction following experimental embolization of retinal circulation.

Spheric plastic beads (7-25 mu in diameter) were injected into the central retinal artery of the owl monkey (Aotus trivirgatus) using a micro-surgical technique. The occlusion of retinal arteriolar branches resulted in posterior vitreous detachment and microcystoid degeneration in inner retinal layers. The dissociation of the vitreoretinal junction showed three different entities: detachment of the vitreous from the basal lamina (inner limiting lamina of the retina), separation of the basal lamina from the retina, and detachment of shreds of glial cells and other degenerated retinal tissue (in

Occlusive retinal vascular disease and deafness.

An 8-year-old white girl with a history of vertigo, nausea, and vomiting developed a progressive hearing loss, bilateral retinal arteriolar narrowing in each eye, vasoproliferation, and subsequent intravitreal hemorrhage. An attempt at peripheral retinal ablation with cryotherapy in the left eye resulted in retinal detachment. Spontaneous retinal detachment occurred in the right eye and was successfully repaired. Repeated intermittent hemorrhages occurred despite intraocular diathermy. Three years after onset, visual acuity was R.E.: 6/21 (20/66) and L.E.: light perception. She remains totally

Effect of the intraocular lens on intraocular pressure.

In the Miami Intraocular Lens Collaborative Study, 17 patients with glaucoma had a Copeland lens implanted at the time of cataract extraction. Seven additional glaucoma patients received a Binkhorst lens. Lens implantation did not adversely affect intraocular pressure or control of glaucoma, except in one patient who developed central retinal artery occlusion with rubeosis 1 1/2 years postoperatively and in one patient who had the lens removed because of iritis with uncontrolled glaucoma. Of 606 eyes that did not have glaucoma before cataract extraction and Copeland lens implant, many had tran

[Unilateral blindness as a complication of carotid angiography (author's transl)].

In a patient with stenosis of the left internal carotid artery acute amaurosis of the left eye was observed after carotid arteriography. The macular branches of the central retinal artery and of the cilioretinal artery were obliterated by embolic material. Two further patients with carotid occlusion and stenosis, in whom spontaneous loss of vision occured, are presented. In such cases the diagnosis may be made with comparable accuracy by other methods, such as ophthalmodynamometry or angioscintigraphy, which entail risk to the patient.

[Etiology and final clinical cause for 1000 enucleations. (A clinico-pathologic study) (author's transl)].

From 1969-1974 1000 unselected enucleated globes have been examined histopathologically. 277 derive from the University Eye Hospital in Hamburg, 723 from various Eye Hospitals in northern and southern Germany. They originate from 589 men and 408 women, three times the sex was unknown. 86 globes had to be removed from children less than 15 years old. 6 groups of etiologies have been distinguished: trauma (308), histologically confirmed neoplastic disease (281), ocular manifestations of systemic diseases (diabetes mellitus, occlusions of central retinal vessels presumably following generalized v

Vascular occlusions in the eye from cardiac myxomas.

Vascular occlusion in the eyes from cardiac myxomas was diagnosed in two cases, belatedly in one and not until after death in the other. Suspicion of a myxomatous origin should be aroused by the combination of unexplained retinal (or choroidal) vascular disease occurring with multifocal neurological symptoms and with systemic symptoms suggesting atypical subacute bacterial endocarditis. The first patient had unilateral retinal artery occlusion by embolic material believed to have been myxomatous. Removal of the cardiac myxoma resulted in disappearance of this material (although the eye remaine

[Contribution to the problem of central serous retinal detachment (author's transl)].

We described extensive and multifocal serous detachment of the neuroretina in cases of Harada's disease, optic head pitting, coloboma of choroid and optic nerve-head and bullous retinal detachment (Gass). Peripapillary serous detachment occurred in an additional case after branch occlusion of central retinal artery. Systemic steroids were useful in Harada's disease. The other cases required photocoagulation or argon laser treatment. Bullous retinal detachment did not heal until a buckling procedure was performed. The serous detachment of the neuroretina is considered as an uniform tissue-react

Ischemia of ciliary arterial circulation from ocular compression.

Two patients developed unilateral central retinal artery and posterior ciliary artery occlusions related to ocular compression during general anesthesia. One patient had evidence of extensive choroidal ischemia with diffuse hypopigmentation, pigmentary mottling of the posterior pole, and disc edema. Electroretinography showed diminution of the A and B waves. The other patient showed patchy choroidal ischemia with subsequent development of wedge-shaped areas of pigmentary atrophy and mottling in the midperiphery. Iridocyclitis and prolonged hypotony were also present. Retinal and posterior cili
