Fibrinolytic Therapy for Thromboembolic Diseases: Approved Indications and Future Directions.

Fibrinolytic therapy plays a pivotal role in the management of thromboembolic diseases by promoting plasmin-mediated fibrin degradation and vessel recanalization, which frequently comes at the cost of increased risk of hemorrhagic events, including intracranial bleeding. Since the discovery of streptokinase in 1933, several additional fibrinolytic agents have been developed. Two plasminogen activators-urokinase and tissue plasminogen activator (tPA)-were identified in human samples. Through recombinant DNA technology, tPA was cloned and modified to produce alteplase. Subsequent efforts to prol

Ophthalmic complications associated with COVID-19: a large US national database analysis.

OBJECTIVE: To assess ophthalmic complications following COVID-19 infection and the impact of mRNA vaccination. METHODS: A retrospective cohort analysis was conducted using a large US database of de-identified electronic health records (1 March 2020-30 April 2021). Patients with COVID-19 and subsequent ophthalmology evaluation were included. The vaccinated COVID-19 cohort was matched to an unvaccinated cohort (n = 73,654 each). COVID-19 and influenza patients (n = 77,809 each) were also matched. Influenza served as a historical control. Ten ophthalmic conditions were assessed post-COVID-19 inf

A case of Escherichia coli native mitral valve endocarditis complicated by annular abscess and central retinal artery occlusion.

Infective endocarditis (IE) is most frequently caused by gram-positive organisms such as Staphylococci, Streptococci, and Enterococci. IE due to gram-negative organisms is uncommon and is most often associated with the HACEK group (Haemophilus, Aggregatibacter, Cardiobacterium, Eikenella, and Kingella) of organisms. IE due to other gram-negative organisms such as Escherichia coli (E. coli) is rare and less is known about its natural pathology. Valvular abscesses due to E. coli IE have been described, however these are not usually reported in a native mitral valve. Here we describe the case of

Incomplete Susac Syndrome With Recurrent Retinal Occlusions and Vertigo: A Case of Early Immunosuppressive Success.

Susac syndrome (SS) is a rare autoimmune microangiopathy that targets the brain, retina, and inner ear. Its variable and often incomplete clinical presentation frequently leads to misdiagnosis, increasing the risk of morbidity. We report the case of a 53-year-old Caucasian woman with a history of pityriasis lichenoides et varioliformis acuta (PLEVA) and severe myopia. She experienced recurrent branch retinal artery occlusion (BRAO) and retinal vein occlusion, following an episode of intense vertigo. Notably, her initial vascular occlusion occurred prior to the onset of vertigo. A comprehensive

Ocular ischaemic syndrome due to giant cell arteritis.

Giant cell arteritis (GCA) is a large vessel vasculitis with devastating visual consequences if left untreated. Classically, GCA has a predilection for the short posterior ciliary arteries which supply the optic nerve head, causing sudden painless visual loss with the development of arteritic anterior ischaemic optic neuropathy (AAION). Other ocular manifestations of GCA include central retinal artery occlusion, oculomotor nerve palsy, posterior ischaemic optic neuropathy and ocular ischaemic syndrome (OIS). OIS is a rare clinical entity, typically stemming from severe, unilateral carotid arte

Simultaneous bilateral branch retinal artery occlusion as a manifestation of sickle cell crisis: diagnostic and therapeutic implications.

Sickle cell disease may manifest as occlusion of arterial retinal vessels during a vaso-occlusive crisis, which can have multiple triggers. In this case, the patient had a simultaneous bilateral branch retinal artery occlusion, following physical activity, due to a sickle cell crisis. Timely treatment was started with exchange transfusion and erythrocytapheresis, which resulted in marked improvement of visual acuity and visual field over a 3-month follow-up. This case enlightens the importance of early recognition and communication with other medical specialties for prompt initiation of treatm

Ocular injuries and the most common acute conditions in ophthalmology.

Ocular traumas include a wide range of injuries from trivial conditions to extensive perforating injuries that can lead to visual function impairment of various ranges and even to the loss of the eye itself. In terms of first aid, not only an ophthalmologist but also a physician of any specialty can effectively intervene and minimize the consequences of these conditions. Open globe injuries represent prognostically the most severe conditions which have to be treated by an experienced ophthalmologist. Closed injuries are most commonly manifested as corneal erosions, foreign bodies on the surfac

LISDEXAMFETAMINE (VYVANSE) USE ASSOCIATED WITH BRANCH RETINAL ARTERY OCCLUSION.

BACKGROUND/PURPOSE: Branch retinal artery occlusions (BRAO) are primarily associated with the aging population and are rare in young adults. The etiology of BRAO includes embolic or nonembolic sources. Lisdexamfetamine has been associated with vasospastic ischemic events in multiple areas of the body. However, there are currently no reported cases of BRAO associated with lisdexamfetamine use. Here, we present a case that suggests a correlation between lisdexamfetamine use and a nonembolic BRAO in a young adult man. METHODS/RESULTS: A 32-year-old man presented with sudden left eye blurred visi

Adversarial learning-based domain adaptation algorithm for intracranial artery stenosis detection on multi-source datasets.

Intracranial arterial stenosis (ICAS) is characterized by the pathological narrowing or occlusion of the inner lumen of intracranial blood vessels. However, the retina can indirectly react to cerebrovascular disease. Therefore, retinal fundus images (RFI) serve as valuable noninvasive and easily accessible screening tools for early detection and diagnosis of ICAS. This paper introduces an adversarial learning-based domain adaptation algorithm (ALDA) specifically designed for ICAS detection in multi-source datasets. The primary objective is to achieve accurate detection and enhanced generalizat

IV Thrombolysis for central retinal artery occlusion - Real-world experience from a comprehensive stroke center.

OBJECTIVES: Central retinal artery occlusion (CRAO) is a stroke of the retina potentially amenable to intravenous thrombolysis (IVT). We aimed to determine feasibility of an emergency treatment protocol and risk profile of IVT for CRAO in a comprehensive stroke center (CSC). METHODS: We performed a retrospective, observational cohort study including patients with acute CRAO admitted to a CSC over 4 years. Patients are offered IVT if they present with acute vision loss of ≤ 20/200 in the affected eye, have no other cause of vision loss (incorporating a dilated ophthalmologic exam), and meet cr

[Hyperbaric oxygen therapy and eye disease: Review of the literature].

Hyperbaric oxygen therapy consists of breathing 100% oxygen continuously or intermittently in a chamber at a pressure equal to or greater than 1.4 absolute atmospheres. Indicated for the emergency treatment of carbon monoxide poisoning and other medical-surgical pathologies such as gas embolism or necrotizing soft-tissue infections, various studies have shown a beneficial effect of hyperbaric oxygen therapy in certain ocular pathologies, notably of microcirculatory origin, such as central retinal artery occlusion or macular edema linked to retinal vein occlusions. In addition, hyperbaric oxyge

Optical coherence tomography angiography in the diagnosis of ocular disease.

Clinical imaging provided by optical coherence tomography (OCT) and its variant, OCT-angiography (OCT-A), has revolutionised eyecare practice. The imaging techniques allow for the identification and quantification of ocular structures, supporting the diagnosis and prognosis of eye disease. In this review, an overview of the usefulness of OCT-A imaging in the diagnosis and management of a range of ocular conditions is provided when used in isolation or in combination with other imaging modalities and measures of visual function (visual field results). OCT-A imaging has the capacity to identify

Thrombolytic Therapy for Central Retinal Artery Occlusion in an Academic Multi-Site Stroke Centre.

Central retinal artery occlusion (CRAO) is a subtype of acute ischaemic stroke leading to severe visual loss. A recent American Heart Association scientific statement proposed time-windows for thrombolysis in CRAO similar to acute ischaemic cerebral strokes. We aimed to review our academic multi-site stroke centre experience with intravenous (IVT) and intra-arterial thrombolysis (IAT) in CRAO between 1997 and 2022. Demographic, clinical characteristics, thrombolysis timeline, concurrent therapies, complications, and 3-month follow-up visual acuity (VA) were collected. The thrombolysed cohort f

Artificial intelligence in therapeutic management of hyperlipidemic ocular pathology.

Hyperlipidemia has many ocular manifestations, the most prevalent being retinal vascular occlusion. Hyperlipidemic lesions and occlusions to the vessels supplying the retina result in permanent blindness, necessitating prompt detection and treatment. Retinal vascular occlusion is diagnosed using different imaging modalities, including optical coherence tomography angiography. These diagnostic techniques obtain images representing the blood flow through the retinal vessels, providing an opportunity for AI to utilize image recognition to detect blockages and abnormalities before patients present
