A direct method for imaging gradient levels of retinal hypoxia in a model of retinopathy of prematurity (ROP).

BACKGROUND: Retinal hypoxia may contribute to the development of preretinal neovascularization (NV) in patients with retinopathy of prematurity (ROP). We hypothesized that levels of NV may be associated with levels of retinal hypoxia. Imaging retinal hypoxia could be an important diagnostic tool to predict levels of disease severity in ROP patients. In this study, we have investigated a direct method for imaging gradient levels of retinal hypoxia using a model of ROP. We believe that this discovery will help understand the ROP pathogenesis in premature infants. METHODS: The rat 50/10 oxygen-in

The Clinical Association of Maternal Asthma and the Development of Retinopathy of Prematurity in Preterm Infants.

Purpose: Retinopathy of prematurity (ROP) is a major cause of childhood blindness worldwide. Infant risk factors such as low birth weight, early gestational age, and oxygen exposure are strongly associated with development of ROP. Establishing possible maternal factors, including maternal asthma, may assist in quantifying preterm infants' risk of developing severe ROP. Methods: A retrospective observational study was performed using the records of the University of Colorado ROP registry from January 2006 to February 2024. Maternal asthma was identified upon review of patients' clinic notes. Od

Pulse Oximetry Histogram Profiles Before and After Red Blood Cell Transfusion in Very Preterm Infants: A Prospective Observational Cohort.

BACKGROUND/OBJECTIVES: Red blood cell (RBC) transfusion is frequently used to treat anemia of prematurity, yet bedside metrics that capture its short-term impact on oxygenation stability are limited. We assessed whether pulse oximetry histogram-derived oxygen saturation (SpO2) exposure changes after transfusion and whether responses differ across clinical subgroups. METHODS: This prospective observational cohort included preterm infants born <32 weeks' gestation who received a standardized RBC transfusion (15 mL/kg). Continuous SpO2 histograms quantified the percentage of monitored time spent

Direct infusion nano-electrospray ionization mass spectrometry for untargeted metabolomics of trace-volume aqueous humor from prematurity infants.

Reactivation of retinopathy of prematurity (ROP) after anti-VEGF therapy poses a serious risk of retinal detachment. Metabolomics of aqueous humor (AH) holds promise for the discovery of early biomarkers, but the extremely limited sample volumes (as low as 2 μL) from prematurity infants hinders traditional liquid chromatography-mass spectrometry (LC-MS). Here, we applied direct infusion nano-electrospray ionization mass spectrometry (DI-nESI-MS) for metabolomic profiling of trace-volume AH samples without chromatographic separation. By comparing the aqueous humor of infants with and without RO

Mathematical Modeling of Human Retinal Vascular Pattern Around the Foveal Avascular Zone.

PURPOSE: Human retinas have unique anatomical structures called the macula and an associated characteristic vascular pattern. Despite its clinical importance, the mechanism underlying the human-specific vascular pattern remains unknown because of limitations in experimental approaches using primate samples. This study aimed to elucidate such vascular formation process. METHODS: We first examined the effects of four hypothetical factors contributing to foveal avascular zone (FAZ) formation: inhibitory molecule secretion, chemoattractant depletion, tissue deformation (towing), and tip cell migr

Clinical predictors of disease severity in nasal versus temporal retinopathy of prematurity.

OBJECTIVE: Recognition of factors associated with a greater need for treatment may be useful in managing retinopathy of prematurity (ROP). We aim to assess the relationship between nasal and temporal ROP on initial presentation and an infant's eventual need for treatment. METHODS: All patients screened for ROP between January 2018 and December 2020 were retrospectively reviewed. Data were collected on all patients (n = 286) with stage 1 or higher ROP. Eyes were classified as either nasal ROP or temporal-only ROP at initial presentation of ROP. Data from all ROP screening exams until treatment

Exploiting porphyrin metabolism to inhibit angiogenesis.

Heme is an essential iron-containing porphyrin that plays a critical role in endothelial cell (EC) function, regulating processes such as cell signalling and energetic metabolism. Nevertheless, the role of de novo heme synthesis and porphyrin metabolism during angiogenesis remains poorly understood. In this study, a pharmacological approach using 5-aminolevulinic acid (ALA) was employed to dysregulate heme/porphyrins homeostasis in EC. ALA treatment resulted in intracellular porphyrins accumulation and extensive release into the extracellular environment. ALA-treated EC exhibited diminished pr

Multi-enzyme nanozyme targeting redox-senescence-angiogenesis axis ameliorates pathological angiogenesis in retinopathy models.

Pathological retinal neovascularization, a hallmark of proliferative diabetic retinopathy and retinopathy of prematurity, is driven by reactive oxygen species (ROS)-induced vascular endothelial cell senescence. Current therapeutic strategies remain limited by their inability to concurrently address the interconnected pathological triad of oxidative stress, inflammation, and cellular senescence. Nanozymes, which mimic the activities of natural enzymes, have emerged as promising candidates for modulating complex disease microenvironments; however, their application in retinal vascular disorders

Degree of thrombocytopenia and severity of retinopathy of prematurity.

BACKGROUND: Growing evidence links thrombocytopenia to retinopathy of prematurity (ROP) in preterm infants. However, it remains unclear whether the degree of thrombocytopenia affects the severity of ROP. METHODS: VLBW (<1500 g) infants were divided into two groups. Group (A) with and Group (B) without thrombocytopenia. Chi square test, logistic regression, and correlation coefficient analysis models were used for statistical analysis. Statistical significance was set at a p value < 0.05. RESULTS: Of the 305 VLBW infants, 47% developed ROP in group A compared to 16% in group B. Severe ROP occ

Clinical exposure thresholds associated with retinopathy of prematurity in preterm infants born at high altitude.

This study characterizes clinical and hematologic risk factors for retinopathy of prematurity (ROP) among preterm infants born at high altitude, where chronic environmental hypoxia represents a relevant physiological stressor but cannot be evaluated as an independent causal determinant. A retrospective cohort study was conducted in a tertiary neonatal intensive care unit located at 2240 m above sea level (Mexico City). Preterm infants with gestational age < 35 weeks and birthweight < 2000 g were included. Exclusion criteria were major congenital anomalies, death prior to ophthalmologic screeni

High-Frequency Oscillation vs Mechanical Ventilation for Neonatal Acute Respiratory Distress Syndrome: A Randomized Clinical Trial.

IMPORTANCE: Key clinical features of neonatal acute respiratory distress syndrome (NARDS) are broadly comparable to those observed in pediatric and adult ARDS; however, evidence is insufficient to recommend high-frequency oscillatory ventilation (HFOV) or conventional mechanical ventilation (CMV) as the preferred first-line therapy. OBJECTIVE: To evaluate whether HFOV is superior to CMV in reducing bronchopulmonary dysplasia (BPD) and other neonatal adverse outcomes, including death, among preterm infants (≤34 weeks' gestational age) with NARDS.
