The levels of cholesterol, triglycerides, lipoproteins (LP), lipid peroxidation products (LPP), and tissue antioxidants were determined in 49 patients with diabetes mellitus (DM), types I and II, at the stage of subcompensation. The study groups were composed of patients with stages II and III capillaroscopic changes with corresponding changes in the fundus of the eye in the form of common and proliferating stages of retinopathy. Patients with DM of both types revealed significant changes in lipid metabolism expressed in the accumulation of highly atherogenic LP, an increase in the content of

Nineteen eyes with background sickle cell retinopathy, eleven from patients with SC disease, eight from patients with SS disease, and twelve eyes with proliferative sickle cell retinopathy were examined by direct and indirect ophthalmoscopy, slit-lamp, fluorescein angiography and vitreous fluorophotometry. Calculation of the alteration of the blood-retinal barrier (BRB) and estimation of the diffusion coefficients of fluorescein in the vitreous were performed by fluorophotometry. In background sickle cell retinopathy, the results show a normally functioning BRB in the posterior pole. Abnormall

The pattern electroretinogram (PERG) and other tests were given to a selected group of patients with diabetes, ranging from those who had no retinopathy or funduscopic changes to those in the preproliferative state. None had visual symptoms. The PERG was found to be normal in patients with microaneurysms and a few blot haemorrhages. However, when cotton-wool spots and angiographic evidence of areas of capillary non-perfusion were present, the PERG was reduced below the normal value. Similar changes occurred with the oscillatory potentials of the ERG evoked by an intense flash, but the results

The recent development of continuous insulin infusion by pump as a treatment method of diabetics and the fact that several publications have shown some doubt as to the efficiency of this method of treatment on the development of diabetic retinopathy, have led us to study this problem. In our study we have followed 24 patients for an average of 22 months (13-41). These patients were diabetics type I, non-obese, normotensive and were divided into three groups based on the extent of their diabetic retinopathy. Group I: no retinopathy, 5 patients. Group 2: background retinopathy without ischaemia,

The effect of plasma glucose control on retinal morphology, urinary albumin excretion and related haematological and lipid measurements was studied prospectively for 30 weeks in 17 patients with insulin-dependent diabetes mellitus. All had background diabetic retinopathy (BDR) and absence of albustix positive proteinuria: 9 were allocated to a continuous subcutaneous insulin infusion (CSII) group and 8 to a conventional therapy (CT) group. There was a sustained reduction (p less than 0.01) in haemoglobin A1 (HbA1) in the CSII group, but not in the CT group. Mean HbA1 over the 30 week study per

This study presents an obstetric protocol offering better management and prediction for normoglycemic insulin-dependent patients (White Class D4, F, R, or RF) who conceived after they were diagnosed as having vascular disease secondary to diabetes mellitus. Normoglycemia was accomplished during the pregestational phase, and conception occurred only after the glycosylated hemoglobin level was documented to be normal. Normoglycemia was maintained during pregnancy in the outpatient setting through the use of blood glucose monitoring performed by the patient. The obstetric protocol emphasized thre

To look for possible determinators of the pathological readings of vitreous fluorophotometry in diabetes reported by other groups, we studied 32 insulin-treated patients, 22 of whom had fluorescein angiograms without pathological changes, while 10 had background retinopathy. 14 healthy subjects matched for age and blood pressure served as controls. Sodium fluorescein, 17 mg/kg body weight, was injected intravenously and ocular fluorophotometry performed 60 and 120 minutes later. Blood drawn 5, 45 and 120 minutes after the injection was assayed for total and ultrafiltrable fluorescein and the i

Abnormalities in ocular fluorophotometry occur in human and experimental diabetes mellitus. To determine the reversibility of these abnormalities, we studied prospectively 11 intensively treated and 25 conventionally treated insulin-dependent diabetics (IDD) preselected because of abnormal vitreous fluorophotometry. Among the 25 IDD treated conventionally with one or two insulin injections daily, hemoglobin A1C concentrations remained elevated and fluorescein concentration, 1 h after the intravenous injection of fluorescein (7 mg/kg), did not change significantly in either the anterior chamber

A progressive failure in phagocytosis by the retinal pigment epithelium (RPE) occurs in the Royal College of Surgeons rat with inherited retinal degeneration. Another change that can be attributed to a defect in the RPE is a breakdown in the blood-retinal barrier. RPE cell junctions, which form a part of this barrier, become permeable to extracellular tracer during the dystrophic process. We have used the freeze-fracture technique to study the structure of RPE cell junctions in normal and dystrophic retinas. In normal retinas, tight junctions between RPE cells consisted of 8 to 16 anastomosing

To determine the influence of pregnancy on the retinas of insulin-dependent diabetics, we assessed 53 diabetic women by retinal photography every six weeks throughout pregnancy and for six months post partum. Thirty-nine nonpregnant insulin-dependent diabetics of childbearing age served as controls. Of the 39 controls, 18 (46.2%) had retinopathy. Of the 53 pregnant women, 33 (62%) had retinopathy at the first examination and eight others (15%) developed it as pregnancy advanced, significantly increasing the prevalence of retinopathy during pregnancy to 77.4%. Progressive changes occurred as pr

Retinal changes resembling those of background diabetic or hypertensive retinopathy commonly occur in leukemia, whereas retinal neovascularization is rare. When neovascularization does occur, it is usually the result of hyperviscosity caused by a greatly increased number of circulating leukocytes. A 42-year-old woman with diabetes mellitus developed chronic myelocytic leukemia and peripheral retinal neovascularization. The hyperviscosity leading to the neovascularization was probably caused by an increased number of circulating platelets.

The authors present the results of their color fluorescein angiographies carried out with a pocket retinograph equipped with new interferential filters. After the graphic examination, they propose to use these same filters mounted on different instruments for binocular and monocular angioscopy. Their results show that retinal vessels are translucid and that their color changes according to their background. The authors termed this change 'versicolored effect'. Moreover, color angiography with these new filters eliminates pseudofluorescence and adds to the fluoroscopic examination an element of

Retinopathy is present in 60%-80% of long-term diabetics, and 5%-10% of diabetics surviving 20 yr from the time of diagnosis will be blind, mostly from retinopathy, which is now the commonest cause of newly diagnosed blindness in the 30-65 yr age group. The mean survival time after a diagnosis of retinopathy is only 5 yr. The natural history of diabetic retinopathy is now being understood more clearly. Mild background retinopathy, characterised by microaneurysms or scattered hard exudates, may progress to maculopathy, with macular vascular pathology leading to exudates and edema at the macula;
