Surgical Management of Cyclodialysis Cleft: An Update.

UNLABELLED: A cyclodialysis cleft develops when the meridional fibers of the ciliary muscle detach from the scleral spur, creating an aberrant aqueous drainage pathway between the anterior chamber and suprachoroidal space. Blunt ocular trauma and intraocular surgery have been identified as the most frequent causes. Cyclodialysis has been documented after phacoemulsification, extracapsular cataract extraction, secondary intraocular lens (IOL) implantation, goniotomy, trabeculotomy, trabeculectomy, and other intraocular surgeries. The new aqueous drainage pathway may lead to persistent severe oc

Clinical characteristics and outcomes of keratoplasty in moderate to severe infective corneal ulcers.

BACKGROUND: Corneal ulcers are a major cause of visual impairment in developing countries. In Indonesia, severe infective ulcers often require keratoplasty (KP) to preserve ocular integrity and improve outcomes. AIM: To determine demographic, clinical and microbiological characteristics, complications, and KP outcomes in patients with moderate to severe infective corneal ulcers. METHODS: A retrospective review was conducted on patients with moderate to severe infective corneal ulcers who underwent KP between January 1, 2018 and December 31, 2020, with a minimum follow-up period of 3 weeks at

Purpureocillium lilacinum Keratitis and Endophthalmitis in an Aphakic Eye With Secondary Glaucoma: A Case Report.

Purpureocillium lilacinum(formerly Paecilomyces lilacinus) is a rare filamentous fungus that can cause keratitis in eyes with predisposing factors such as chronic ocular surface disease, previous surgery, trauma, or long-term use of topical steroids. This infection often exhibits resistance to standard antifungal agents and can result in severe visual loss. We report a case of keratitis combined with endophthalmitis in a male patient under 60 years of age with a history of childhood ocular trauma leading to aphakia and secondary glaucoma of the right eye. The patient had undergone several prev

Participation of Lens Proteins and miRs in Traumatic and Inheritance Cataract: A Review on Diagnostic and Therapeutic Approaches for Cataract Management.

Traumatic and inherited cataract spiking blindness is caused by accumulated deposition of mutant eye lens protein or lens microarchitecture alteration. A traumatic cataract is a clouding of the eye's natural lens that occurs as a result of physical trauma to the eye. This trauma can be caused by various incidents such as blunt force injury, penetration by a foreign object, or a significant impact on the eye area. Inheritance cataracts or hereditary cataracts are cataracts that are genetically inherited from one or both parents. Complications following cataract surgery encompass various adverse

[Firework induced Eye Injuries: Diagnostics and Therapy].

Wherever consumer fireworks are used, incidents with eye injuries do occur. In Germany, their number has increased in the past nine years, a clear overrepresentation of minors and bystanders is documented. Eyes are affected by a very individual combination of thermal, chemical and mechanical impact, resulting in minor to most complex injuries to adnexes, eye surface, iris, lens, sclera and/or posterior segment. Diagnostics may be complicated and require a structured approach, that is proposed here. Open globe injuries and intraocular foreign bodies necessitate early surgical intervention. In t

Acute Angle Closure Glaucoma Secondary to Suprachoroidal Hemorrhage Triggered by Valsalva Maneuver: A Rare Case.

Spontaneous suprachoroidal hemorrhage is a rare ophthalmic emergency, typically associated with ocular surgery or trauma. However, it can occur spontaneously in anticoagulated elderly patients after a sudden rise in venous pressure, such as during a Valsalva maneuver. We report a rare case of acute angle closure glaucoma secondary to suprachoroidal hemorrhage triggered by severe vomiting in an anticoagulated patient. An 84-year-old male with atrial fibrillation on apixaban was hospitalized for biliary colic. Following repeated vomiting episodes, he experienced sudden, painful vision loss in th

Ophthalmic complications of radiotherapy.

Radiotherapy is a vital tool in the management of benign and malignant conditions affecting the eye, orbit, and surrounding tissues. It may be used alone or in combination with surgery or systemic therapies. While conventional external beam radiotherapy (EBRT) is still indicated for selected orbital and periocular conditions, intraocular tumours are now predominantly managed with plaque brachytherapy, proton therapy, or stereotactic radiosurgery. Despite advances in radiotherapy precision, adjacent normal structures are still often exposed to radiation, resulting in a variety of ophthalmic com

Unilateral lens capsule rupture with anterior nuclear extrusion in a North American river otter (Lontra canadensis).

A 10-year-old intact male North American river otter (Lontra canadensis) was evaluated under general anesthesia by a zoo veterinarian and 2 Board-certified ophthalmologists concerning a white opacity in the anterior chamber of the right eye (OD). Ophthalmic examination of OD revealed ventral keratitis, a hypermature cataract, equatorial lens capsule rupture, and anterior luxation of the lens nucleus. The right eye lacked a dazzle, direct, and indirect pupillary reflexes. No ocular anomalies were noted in the left eye. Enucleation of OD was conducted due to vision loss, the anticipation of cons

Influence of Post-radiation Ocular Surface Disorder on Ocular Surgery: A Case Report and Review of the Literature.

BACKGROUND: Ocular surface disorder after ocular radiation therapy, even though commonly reported, is often overlooked. Any delay in diagnosis may lead to complications that threaten vision. The presented case highlights the clinical outcome of a severe post-radiation disorder of the ocular surface, the importance of intensive therapy, and the limitations of further surgical interventions. CASE PRESENTATION: A 34-year-old woman was referred for a second opinion due to a years-long history of pain and redness in her right eye (OD) after proton beam therapy for recurrent iris melanoma. Role of inflammation in a rat model of radiation retinopathy.

Radiation retinopathy (RR) is a major side effect of ocular tumor treatment by plaque brachytherapy or proton beam therapy. RR manifests as delayed and progressive microvasculopathy, ischemia and macular edema, ultimately leading to vision loss, neovascular glaucoma, and, in extreme cases, secondary enucleation. Intravitreal anti-VEGF agents, steroids and laser photocoagulation have limited effects on RR. The role of retinal inflammation and its contribution to the microvascular damage occurring in RR remain incompletely understood. To explore cellular and vascular events after irradiation, we

Harnessing Standing Sound Waves to Treat Intraocular Blood Cell Accumulation.

Certain ocular conditions result from the non-physiological presence of intraocular particles, leading to visual impairment and potential long-term damage. This happens when the normally clear aqueous humor becomes less transparent, thus blocking the visual axis and by intraocular pressure elevation due to blockage of the trabecular meshwork, as seen in secondary open-angle glaucoma (SOAG). Some of these "particle-related pathologies" acquire ocular conditions like pigment dispersion syndrome, pseodoexfoliation and uveitis. Others are trauma-related, such as blood cell accumulation in hyphema.

Rare consequences of a single fist punch to the orbital region - a description of two cases.

A direct punch with a clenched fist to the face most often results in soft tissue damage, which is usually not serious enough to be considered a severe health impairment. This article presents two cases in which a single punch to the orbital region led to a blowout fracture. The first case resulted in additional displacement of the right lens into the vitreous body, secondary glaucoma and retinal detachment. In the second case, the victim sustained retinal concussion and subretinal haemorrhage due to choroidal rupture at the level of the macula resulting in temporary, almost complete loss of v
