Metformin-related fatalities - The importance of combining forensic toxicology and postmortem biochemistry analyses in death involving antidiabetics.

Metformin is the most commonly used drug in the treatment of type 2 diabetes mellitus and appears as a promising drug for several other conditions. While generally well-tolerated and relatively safe, metformin can cause serious side effects, and intoxication cases have been reported. Classically, the severity of metformin intoxications is better assessed by measuring lactate blood concentration and/or blood pH than by directly determining blood drug concentration. However, such an approach is more complicated in postmortem investigation. Indeed, biochemistry changes occur after death, affectin

RNA dysfunction in age-related macular degeneration: the role of U1 snRNP complex and neurodegenerative diseases.

BACKGROUND: Age-related macular degeneration (AMD), a leading cause of vision loss in elderly individuals, is a multifactorial disease driven by genetic, environmental, and cellular aging processes. Emerging evidence highlights the critical role of ribonucleic acid (RNA) splicing dysfunction in AMD pathogenesis, with a focus on the U1 small nuclear ribonucleoprotein (U1 snRNP) complex, a key spliceosome component. U1 snRNPs ensure the fidelity of RNA cotranscription and pre-mRNA splicing initiation, and their dysfunction has been implicated in neurodegenerative disorders and other age-related

IL-6 trans-signaling: an overlooked driver of retinal neovascularization?

BACKGROUND: Vascular endothelial growth factor (VEGF) is regarded as the essential promoter of vitreoretinal vascular diseases that threaten eyesight, such as proliferative diabetic retinopathy (PDR). Therefore, VEGF is the primary therapeutic target in these diseases, but not all patients respond adequately to VEGF inhibition. This raises the question if other factors contribute to disease modulation. PDR evolves in an interplay of pathological processes including inflammation, barrier integrity loss, aberrant angiogenesis, and metabolic dysregulation. Interleukin-6 (IL-6), recognized for its

Clinical Assessment of a Highly Multiplexed Panel Assay for the Diagnosis of Infectious Uveitis.

PURPOSE: Current diagnostic workup for infectious uveitis relies on the use of various immunoassays and polymerase chain reaction (PCR) tests on ocular fluids that are time consuming and not comprehensive, requiring large sample volumes to probe for all causes. To streamline the diagnostic process, improve turnaround time, and target inclusivity, we developed a highly multiplexed pan-ocular-pathogen panel (OcuPan) that can be used for broad-range pathogen detection in only 1 assay, with PCR-level sensitivity and in a timely fashion (12 hours). Here, we assessed the clinical usefulness and perf

Paediatric Neoplastic Uveitis Masquerade Syndromes.

Paediatric neoplastic uveitis masquerade syndrome (UMS) is a rare but serious condition in which intraocular malignancies mimic inflammatory uveitis. Children may present with symptoms of intraocular inflammation such as redness, pain, light sensitivity, and blurred vision. The diagnostic challenge is compounded by the difficulty of examining younger patients and the absence of systemic warning signs. Accurate diagnosis is essential because the therapies for uveitis and ocular neoplasms vary fundamentally, and delayed recognition can be life-threatening.Although the most frequent neoplastic ma

Suprachoroidal injection of a dexamethasone implant in a case of secondary macular oedema.

BackgroundNon-infectious uveitis (NIU) is a common ocular inflammation that frequently leads to Macular oedema (MO). MO can cause irreversible tissue damage and permanent visual impairment. Dexamethasone implant is approved for intravitreal injection (IV) in the treatment of MO. Despite its proven efficacy, IV poses risks of complications. Recently, suprachoroidal injection has emerged as a novel strategy for targeted drug delivery to the posterior segment of the eye, reducing the risk of complications.Case presentationA 70-year-old woman with no history of diabetes underwent left eye cataract

Intraocular lens subluxation obstructing a glaucoma tube shunt after a Valsalva maneuver: a case report.

BACKGROUND: Late subluxation of an intraocular lens (IOL) within the capsular bag is a rare complication of cataract surgery, primarily associated with progressive zonular weakening in conditions such as pseudoexfoliation syndrome, high myopia, prior vitreoretinal surgery, connective tissue disorders, or ocular trauma. While IOL-capsular bag subluxation is well-documented, its impact on glaucoma drainage devices remains poorly understood. We present a rare case of Valsalva-induced IOL subluxation obstructing an Ahmed glaucoma valve, resulting in acute intraocular pressure (IOP) elevation in a

Traumatic Glaucoma Managed With Ab Interno Trabeculotomy (Trabectome®): A Case Report.

A 55-year-old patient sustained a bungee cord injury to his left eye, resulting in a traumatic cataract, vitreous prolapse into the anterior chamber, and extensive angle recession. Despite cataract surgery with pars plana vitrectomy and maximal medical therapy, intraocular pressure (IOP) remained elevated at 40 mmHg. The patient was diagnosed with mixed-mechanism traumatic glaucoma. Ab interno trabeculotomy with the Trabectome® system (MicroSurgical Technology, Redmond, WA, USA) successfully lowered IOP over 12 months, significantly reducing the medication burden. To the best of our knowledge,

Design and evaluation of curcumin, naringenin, and alpha-lactalbumin encapsulated nanostructured lipid carriers for targeted ocular drug delivery in diabetic retinopathy.

Diabetic retinopathy (DR), a leading cause of vision impairment and blindness, poses significant challenges for ocular drug delivery due to barriers limiting drug penetration to the eye's posterior segment. This study aimed to develop and characterize nanostructured lipid carriers (NLCs) encapsulating curcumin, naringenin, and alpha-lactalbumin for sustained and targeted ocular delivery via topical administration in DR management. The NLCs exhibited a uniform particle size of 138.7 ± 2.6 nm, high encapsulation efficiency (∼97 %), and a stable zeta potential of -25.6 mV. Transmission electron m

Advances in imaging retinal inflammation.

Imaging of retinal inflammation has emerged as a promising approach for the diagnosis and management of a wide range of ocular inflammatory conditions, particularly those involving uveal edema-including the iris, ciliary body, and choroid. Recent advancements in imaging technologies have markedly enhanced our ability to quantify and monitor retinal inflammation with greater precision and objectivity. Optical coherence tomography (OCT), especially spectral-domain and swept-source OCT, enables high-resolution visualization of retinal architecture and allows for the quantification of parameters s
