Impact of Glucagon-Like Peptide 1 (GLP-1) and Gastric Inhibitory Polypeptide (GIP) Receptor Agonists on Intraocular Pressure in Glaucoma: A Case Series.

PURPOSE: Glucagon-Like Peptide 1 Receptor Agonists (GLP-1RAs) are antidiabetic and weight management medications that bind to GLP-1 receptors throughout the body. Growing interest in this class has led to several retrospective cohort studies reporting a decreased occurrence of glaucoma in diabetic and non-diabetic patients with the use of GLP-1RAs, suggestive of anti-inflammatory and neuroprotective effects. Here we report five real world cases evaluating the impact of GLP-1RA and dual GLP-1/Gastric Inhibitory Polypeptide (GIP) receptor agonist use on Intraocular Pressure (IOP) measurements an

Multifocal Posterior Pigment Epitheliopathy in a Pachychoroid Eye Following Ahmed Glaucoma Valve Implantation: A Case Report.

We report a case of multifocal posterior pigment epitheliopathy (MPPE), a bullous variant within the pachychoroid disease spectrum, following Ahmed Glaucoma Valve (AGV) implantation in a patient with exfoliation glaucoma (XFG). An 81-year-old man with XFG underwent AGV implantation for uncontrolled intraocular pressure (IOP) of 48 mmHg despite maximal medical therapy. Preoperative best-corrected visual acuity (BCVA) was 20/20. Preoperative optical coherence tomography (OCT) demonstrated dilated pachyvessels in Haller's layer with attenuation of the inner choroid, consistent with an underlying

Win Ratios in Ophthalmology-a Hierarchical Metric to Evaluate Adverse Outcomes in Comparative Studies.

PURPOSE: Clinical trials in ophthalmology frequently report multiple efficacy and safety outcomes of varying clinical importance analyzed either in isolation or as simple composites. The Win Ratio (WR) is a hierarchical composite metric originally developed to address such limitations in cardiovascular research by integrating outcomes according to their clinical priority. In this study, we assess its potential utility in ophthalmology outcomes with glaucoma surgery as the exemplar. DESIGN: Retrospective registry-based study. SUBJECTS: There are 1257 eyes that underwent a Xen gel stent (Xen)

XEN63 Gel Stent in Glaucoma: Narrative Review and Expert Perspectives on Patient Selection, Surgical Technique, and Perioperative Management.

The XEN gel stent (AbbVie Inc. North Chicago, IL, USA) is a minimally invasive bleb-forming surgical device designed to lower intraocular pressure (IOP). The 63 μm gel stent has a larger lumen than the 45 μm gel stent, creating lower outflow resistance, which may translate into greater IOP-lowering potential. However, the comparative evidence base remains limited, heterogeneous, and largely retrospective; the magnitude and durability of any advantage over the 45 μm device should be interpreted cautiously. This narrative review summarizes the currently available evidence and expert surgical per

The FIRE technique - A tube ligation alternative.

Nonvalved glaucoma drainage devices (GDDs) carry a risk of early hypotony due to unregulated flow prior to capsule formation. Temporary flow restriction occurs through methods such as absorbable ligature sutures and intraluminal ripcords. Here, we propose an alternative approach, the Flanged Intraluminal Ripcord Element (FIRE) technique, that eliminates the need for a ligature suture and avoids the unpredictability associated with ligature autolysis. A patient case is discussed that demonstrated adequate prevention of hypotony and successful intraocular pressure reduction upon ripcord removal.

LONG-TERM FOLLOW-UP OF A COMPLEX CASE OF FOLDABLE CAPSULAR VITREOUS BAG IMPLANTATION.

PURPOSE: The Foldable Capsular Vitreous Bag (FCVB) implant is useful in maintaining an ocular structure in cases of chronic hypotony secondary to trauma or repeated surgery. There are a few published reports of long-term outcomes and no indication of the chronic cellular changes of the adjacent tissues associated with FCVB implantation. We describe one of the few FCVB cases in the United Kingdom and present histological findings associated with it following secondary enucleation. METHODS: A 33-year-old man underwent a left primary globe repair following a left penetrating injury and intraocul

This case study describes the rehabilitation efforts of a 25-year-old male reserve officer who sustained a penetrating intraocular metallic foreign body (IOFB) injury to his right eye during a military conflict. Penetrating ocular trauma can lead to severe tissue deformities, scarring, permanent visual impairment, and long-term complications like intraocular infection and ocular hypotension. The patient's initial presentation revealed vision loss, pain, irritation, a foreign body sensation, and discomfort in the right eye. Diagnostic imaging confirmed a severe globe rupture, including the pres
