Paul tube implant: a meta-analysis and systematic review evaluating clinical outcomes.

OBJECTIVE: A systematic review and meta-analysis evaluating the peer-reviewed literature describing the efficacy of the Paul glaucoma implant (PGI) in managing intraocular pressure (IOP) and its outcomes. METHODS: Comprehensive searches were conducted across major databases to identify relevant studies. The analysis focused on glaucoma patients who underwent PGI. The primary outcomes included failure rate, success rates (complete and qualified), reduction in IOP and post-operative complications. Reduction in medication use was assessed as secondary outcome. A random-effects model was applied

Management of postoperative hypotony with fibrin glue following scleral tunnel-based explantation of pIOL and simultaneous ICL implantation: A case report.

RATIONALE: Long-term implantation of anterior phakic intraocular lenses (pIOLs) can lead to progressive corneal endothelial cell loss, ultimately necessitating explantation. Although scleral tunnel approaches reduce corneal trauma during lens exchange, postoperative hypotony due to wound microleakage is an uncommon but vision-threatening complication. This report aims to present the 1st documented clinical case in which fibrin glue was successfully used to seal microleakage-induced hypotony following pIOL explantation and simultaneous toric implantable collamer lens (ICL) implantation. PATIEN

A Case of Refractory Childhood Glaucoma Associated With Sturge-Weber Syndrome Treated With Baerveldt Glaucoma Implant.

We reported the clinical course of refractory childhood glaucoma associated with Sturge-Weber syndrome (SWS) treated with Baerveldt glaucoma implant (BGI). The patient was a 14-year-old male diagnosed with SWS after birth. He had previously undergone four trabeculotomies for glaucoma and vitrectomy for submacular hemorrhage from a diffuse choroidal hemangioma in his left eye. Before the BGI surgery, his intraocular pressure was 30 mmHg under full medications. The patient experienced extensive serous retinal and choroidal detachments on Postoperative Day 9. The cause may have been the increased

Inferior exten filtering bleb formation after laser goniopuncture in a patient with conjunctivochalasis.

PURPOSE: Inferior extension of filtering blebs after glaucoma surgery is rare. Conjunctivochalasis, an age-related redundancy of the conjunctiva, may influence bleb morphology but is often overlooked. This report describes an inferiorly extended filtering bleb following deep sclerectomy and subsequent Nd:YAG laser goniopuncture in a patient with pre-existing conjunctivochalasis. METHODS: A 66-year-old man with advanced primary open-angle glaucoma underwent combined phacoemulsification, deep sclerectomy, and intraoperative mitomycin C in the right eye. Initial recovery was uneventful, with IOP

[Revision for hypotension after trabeculectomy-Specific features in a young male patient].

A 34-year-old male patient with primary juvenile open-angle glaucoma presented 3 months after trabeculectomy with mitomycin C in the left eye due to progressive visual deterioration. Ocular hypotension with an intraocular pressure of 2 mmHg was noted along with a prominent filtering bleb and macular folding of the retina and choroid as the cause of reduced visual acuity. The diagnosis of hypotony maculopathy was established. After an initial lack of improvement, revision surgery was performed. Intraoperatively, a very thin and undersized scleral flap with a torn suture as well as a disproporti

Cholinergic Crisis Associated With Standard-Dose Pyridostigmine in a Hemodialysis Patient With Ocular Myasthenia Gravis and Marked Weight Loss.

Pyridostigmine is an acetylcholinesterase inhibitor widely used for the symptomatic treatment of myasthenia gravis (MG). Cholinergic crisis is a rare but life-threatening adverse effect caused by excessive cholinergic stimulation. Since pyridostigmine is primarily excreted by the kidneys, standard doses may become excessive in the presence of severe renal impairment. We report a case of cholinergic crisis in an elderly patient on maintenance hemodialysis (HD) who exhibited significant weight loss and severe hypoalbuminemia. An 81-year-old male with acetylcholine receptor antibody-positive ocul

Heterozygous X-linked Alport syndrome in a pregnant woman: A case report.

Alport syndrome is a genetic disorder of chronic kidney disease, hearing loss, and ocular abnormalities, caused by mutations in type IV collagen. While X-linked Alport Syndrome demonstrates characteristic severe renal failure in males, it has a variable presentation in females. Here, we present a case of a pregnant woman who was found to have X-linked Alport Syndrome with a heterozygous c.3587G>A (p.Gly1196Glu) mutation in the COL4A5 gene. The patient presented in her third trimester of pregnancy with preexisting chronic proteinuria. Family history was notable for kidney failure in her mother

Perioperative ischemic optic neuropathy: a comprehensive review of anesthetic implications, hemodynamic pathophysiology, and risk stratification.

INTRODUCTION: Perioperative visual loss (POVL) is a rare but catastrophic complication of non-ocular surgery, most commonly attributable to ischemic optic neuropathy (ION). Spine and cardiac surgeries account for the majority of cases, with posterior ION (PION) predominating after prone spine procedures and anterior ION (AION) more frequently observed following cardiac surgery. Despite its low incidence, the permanent nature of vision loss necessitates a clear understanding of underlying mechanisms and modifiable perioperative risk factors. METHODS: This narrative review synthesizes evidence
