Functional and structural outcomes in paediatric myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD): a prospective study.

PURPOSE: Myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) is a rare autoimmune disease affecting the central nervous system, frequently manifesting with optic neuritis (ON). Despite often favourable functional visual recovery, structural retinal changes may persist. This prospective study assessed the utility of functional and structural visual outcome measures in paediatric MOGAD. METHODS: Children with serologically confirmed MOGAD were recruited from across Switzerland, alongside age-matched healthy controls. All participants underwent pattern-reversal visual evoked

Bilateral Optic Neuritis Following Acute Glyphosate Inhalation: A Case Report.

BACKGROUND: Bilateral optic neuritis is a rare condition generally associated with inflammatory, demyelinating, or toxic causes. Its association with glyphosate exposure has rarely been documented. CASE PRESENTATION: A 66-year-old man with hypertension, hypercholesterolemia, and hypothyroidism developed rapid bilateral vision loss within hours after acute inhalational exposure to glyphosate during agricultural work. MRI showed bilateral optic nerve hyperintensity consistent with optic neuritis. Cerebrospinal fluid and serum anti-NMO antibody tests were negative, while visual evoked potentials

Optic neuropathies induced by immune checkpoint inhibitors: A case series and systematic review of the literature.

INTRODUCTION: Immune checkpoint inhibitor (ICI)-related optic neuritis is rare but clinically significant as visual sequelae are reported in around half of affected patients. MATERIALS AND METHODS: We retrospectively collected all cases of ICI-related optic neuropathy referred to two tertiary centers. A systematic review of PubMed, Embase, and Medline was conducted following PRISMA guidelines. Cases were classified into: i) optic neuritis, defined by the presence of consistent symptoms (visual loss, dyschromatopsia, afferent pupillary defect) and optic nerve abnormalities on MRI or visual evo

OCT-Based Differentiation of First Acute Optic Neuritis: An International Study of 111 Patients With NMOSD and MOGAD.

BACKGROUND AND OBJECTIVES: Severe optic neuritis (ON) is a common clinical manifestation in myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) and neuromyelitis optica spectrum disorder (NMOSD). Given distinct prognoses and often the necessity of early plasma exchange in NMOSD, prompt differentiation is crucial. In this study, we investigated the utility of optical coherence tomography (OCT) in differentiating between first acute NMOSD-ON and MOGAD-associated optic neuritis (MOGAD-ON), as well as specific factors associated with disc edema. METHODS: In this retrospective

Relapse-related outcomes in the Danish population-based AQP4- antibody seropositive neuromyelitis optica spectrum disorder cohort.

BACKGROUND: The disease course of AQP4-antibody seropositive neuromyelitis optica spectrum disorder (AQP4-Ab + NMOSD) is unpredictable and variable. We aimed to examine relapse-related outcomes. METHODS: In a nationwide historically prospective AQP4-Ab + NMOSD cohort, relapse outcomes, relapse and maintenance treatments were analyzed by mixed-effects linear and logistic regression models. RESULTS: Among sixty-six patients (median follow-up: 99.5 months) there were 350 relapses and 272 relapse treatments. Delayed immunosuppression after the first relapse and brainstem (BS) relapses were assoc

Complement Inhibition for Acute Neuromyelitis Optica Spectrum Disorder Attacks: Insights From an International Case Series.

BACKGROUND AND OBJECTIVES: Neuromyelitis optica spectrum disorder (NMOSD) is a severe autoimmune disease mainly driven by aquaporin-4 antibodies (AQP4-IgG). During an attack, AQP4-IgG activates the complement system, leading to astrocyte destruction, inflammation, neuronal damage, and thus devastating and often irreversible neurologic deficits. Terminal complement inhibitors such as eculizumab and ravulizumab effectively prevent relapses, yet their therapeutic potential in stopping ongoing complement-mediated injury during acute attacks remains insufficiently explored. METHODS: We conducted a

Relapse risk factors in double seronegative neuromyelitis optica spectrum disorder: Insights from a multicenter study.

OBJECTIVE: To identify the risk factors associated with the relapse rate in patients with DSN-NMOSD. METHODS: This is a multicenter retrospective study of cases of DSN-NMOSD at the Mass General Brigham, the University of Sao Paulo, and Koc University. Patients were excluded if they had an alternative diagnosis. To examine the association of the different clinical and paraclinical factors on relapses, we calculated the incidence rate ratio (IRR) using a Poisson regression analysis. RESULTS: A total of 35 relapsing DSN-NMOSD patients were analyzed. In a univariate Poisson regression analysis,

Non-Arteritic Anterior Ischaemic Optic Neuropathy and Its Masquerades: A Comprehensive Diagnostic Review.

Non-arteritic anterior ischaemic optic neuropathy (NAION) is the most common acute optic neuropathy in individuals over 50 years and presents with sudden, painless vision loss. However, due to the limited phenotypic repertoire of optic nerve injury, other conditions can closely mimic NAION, leading to diagnostic uncertainty. This review outlines the key clinical, imaging and systemic features that help differentiate NAION from its major masquerades, including arteritic anterior ischaemic optic neuropathy, demyelinating optic neuritis, autoimmune and granulomatous diseases, hereditary optic neu

[Neuritis nervi optici (NNO)].

The term optic neuritis describes an inflammation of the optic nerve. It encompasses a group of different diseases that vary in their prevalence, clinical course, treatment, and visual prognosis. These include autoimmune demyelinating, frequently recurrent inflammations of the optic nerve, which may be associated with an inflammatory CNS disease, as well as monophasic infectious or postinfectious inflammations of the optic nerve and those associated with systemic disease or vaccination.

A Case Report of Rare MOG Antibody-Associated Optic Neuritis Related to Hepatitis E Virus Infection.

Hepatitis E infection is a highly prevalent viral infection, with neurological manifestations being the most common extrahepatic symptoms. However, cases of hepatitis E virus (HEV) infection presenting with optic neuropathy symptoms remain scarce. We reported a case of a 49-year-old male who presented with fatigue, poor appetite, slightly yellowish urine, and fever. The patient had no significant past medical history. Upon admission, liver and kidney function tests were abnormal, and serological testing was positive for HEV IgM antibodies. After targeted therapy for liver protection and jaundi

Updates on Pediatric Optic Neuritis.

Optic neuritis is a central demyelinating disease that can significantly affect vision. Favorable outcomes are contingent on prompt recognition, work-up, and treatment. Understanding key differentiations in the pediatric population compared with adults is necessary for accurate and timely diagnosis. Recent studies highlighting these distinctions have deepened insight into clinical variabilities in presentation, as well as neurological associations, treatments, and prognostic factors. Ongoing collaborative studies continue to shed light on the main considerations guiding research on this topic.

Using Optical Coherence Tomography in Pediatric Neuro-ophthalmology.

Optical coherence tomography (OCT) has transformed adult neuro-ophthalmology, but pediatric adoption has lagged because of perceived technical, financial, and practical barriers. This chapter provides a pragmatic framework for integrating OCT into pediatric and pediatric neuro-ophthalmology clinics, emphasizing how to obtain reliable images in awake children and how to interpret them. We first review technical aspects of tabletop and handheld OCT in children, including positioning, fixation strategies, and when to prioritize single-line macular or optic nerve head B-scans over full segmented p

Paraneoplastic neuromyelitis optica spectrum disorder with dual AQP4-IgG and CRMP5 antibodies following thymectomy: a case report.

We present a rare case of paraneoplastic neuromyelitis optica spectrum disorder (NMOSD) in a 50-year-old woman with a history of B3 thymoma, marked by dual positivity for AQP4-IgG and CV2/CRMP5 antibodies. The patient developed sequential bilateral optic neuritis, with orbital MRI revealing enhancement of the left optic nerve. Serological analysis showed a high AQP4-IgG titer (1:320) and positive CV2/CRMP5 antibodies, while other paraneoplastic markers were absent. Treatment with glucocorticoids and mycophenolate mofetil led to substantial visual recovery. This case underscores the association
