Severe retinal vasculopathy associated with myelin oligodendrocyte glycoprotein antibody optic neuritis in a toddler.

We report the case of a 22-month-old boy with retinal vascular disease caused by optic neuritis due to myelin oligodendrocyte glycoprotein antibody disease. He presented with acute disseminated encephalomyelitis without optic nerve involvement. Despite treatment, he developed optic neuritis and vision loss within days. In addition to the usual findings of optic neuritis, he had evidence of severe retinal venous stasis including intraretinal, subretinal, preretinal, and choroidal hemorrhages that were rapidly progressive. The retinal perfusion did not recover, leading to neovascularization. Thi

Under Pressure: Increased Intracranial Pressure in Infants and Children. Presented at the 2024 AOC/AACO/AAO Sunday Symposium.

Intracranial hypertension (ICH) in children refers to increased pressure within the skull. The skull normally contains the brain, blood vessels, meninges, and cerebrospinal fluid (CSF). Abnormalities in any of these intracranial structures may cause ICH. The differential diagnosis for pediatric ICH includes brain tumors; intracranial hemorrhages; intracranial infections; craniosynostosis; disorders of CSF production, clearance, or abnormal CSF contents; and pseudotumor cerebri syndrome (PTCS), which may be primary (also known as idiopathic intracranial hypertension [IIH]) or secondary. Nasal-Predominant Ganglion Cell-Inner Plexiform Layer Thinning Distinguishes Early-Stage Leber Hereditary Optic Neuropathy From Acute Optic Neuritis.

BACKGROUND: To compare acute-phase macular ganglion cell-inner plexiform layer (GCIPL) thickness on optical coherence tomography (OCT) between Leber hereditary optic neuropathy (LHON) and optic neuritis (ON), and to evaluate the diagnostic performance of nasal GCIPL thickness. METHODS: In this retrospective single-centre study, OCT (CIRRUS HD-OCT; Carl Zeiss Meditec, Dublin) acquired within 45 days of symptom onset was analysed. Sectoral GCIPL thickness was compared between LHON and ON-including double-seronegative (DN-ON), aquaporin-4 antibody-positive (AQP4-ON) and myelin oligodendrocyte gl

A novel limited-dose C5 inhibitor add-on as rescue therapy for refractory NMOSD attacks: a practical alternative to conventional multi-dose regimens.

BACKGROUND: Neuromyelitis optica spectrum disorder (NMOSD) patients with refractory attacks, despite standard therapy with high-dose intravenous methylprednisolone (IVMP) and plasma exchange (PLEX), often experience significant disability. While complement C5 inhibition has established efficacy in relapse prevention, its role in acute-phase management remains unclear. METHODS: In this single-center, retrospective case series, eight patients (6 AQP4-IgG seropositive, 2 seronegative) with acute NMOSD and suboptimal responses to IVMP ± PLEX were treated with 1-4 weekly doses of ECU (900 mg per d

Resolution of disc oedema in nonarteritic anterior ischaemic optic neuropathy versus optic neuritis: a retrospective, cross-sectional study.

BACKGROUND: The common causes of unilateral disc oedema include nonarteritic anterior ischaemic optic neuropathy (NAION) and optic neuritis (ON). As prolonged disc oedema may affect visual function, early differentiation is crucial. Therefore, this study compared the resolution time and rate of disc oedema between NAION and ON cases and identified correlated clinical factors. METHODS: In this retrospective, cross-sectional comparison study, unilateral NAION or ON cases, in which disc oedema was confirmed through fundus photography, were reviewed. Optical coherence tomography data, such as str

An unusual association of acute acquired comitant esotropia and bilateral idiopathic optic neuritis in two boys: a case series.

INTRODUCTION: Acute acquired comitant esotropia is characterized by sudden-onset, large-angle comitant esotropia occurring in adults and older children. The condition differs from infantile esotropia in its acute presentation, typically occurring after the age of 6 months, when binocular vision has developed. While many cases of AACE are idiopathic or attributed to functional factors such as excessive near work with digital devices, neurological etiologies can account for up to 10% of cases and must be investigated to rule them out. Here we are reporting an unusual association of acute acquire

When Routine Care Changes a Life.

On a rare day off from my duties as an ophthalmology resident, I wandered into a neighborhood bookstore and was recognized by a woman I had treated for optic neuritis in the emergency department. I didn't remember her, but she remembered everything. In that gap-between my routine and her life-altering night-I heard what the emergency department often muffles: how ordinary work for me can be extraordinary to the person living it. The piece follows our brief conversation to ask a simple question: how can I carry less and notice more, honoring meaning without slowing the work that must go on?

Precision management of autoimmune ocular complications: Th17 mechanisms and therapeutic innovations.

Precision management of ocular complications in systemic autoimmune diseases, such as Sjögren's syndrome (SS), systemic lupus erythematosus (SLE), Behçet's disease, and thyroid-associated ophthalmopathy (TAO), requires integration of immune mechanisms with clinical translation. These disorders manifest as dry eye, uveitis, retinal vasculitis, optic neuritis, and orbital fibroinflammatory disease, posing a substantial risk of irreversible visual loss. Recent studies highlight the T-helper 17 (Th17)/interleukin-17 (IL-17) axis, tumor necrosis factor-alpha (TNF-α), complement component 5a (C5a),

Atypical retrobulbar optic neuropathy after semaglutide escalation.

Semaglutide, a glucagon-like peptide-1 receptor agonist (GLP-1RA), is widely used to manage type 2 diabetes and obesity. Recent pharmacovigilance signals have reported an increased incidence of nonarteritic anterior ischemic optic neuropathy (NAION) among semaglutide users, although the absolute risk remains low. We report a man in his early 30 seconds with class III obesity, obstructive sleep apnea, and prediabetes who developed acute, painless, asymmetric bilateral visual dysfunction four weeks after semaglutide escalation to 1 mg per week. Evaluation revealed retrobulbar optic neuropathy wi

Development and validation of a novel and graded colour saturation threshold test, the LSS-8.

Colour vision changes are a key clinical feature of many acquired retinal and optic nerve disorders. Current clinical tests are often labour-intensive or designed for congenital defects. We introduce the novel LSS-8 test, which measures colour saturation thresholds across eight hues. Three cohorts with mixed ocular health underwent testing in a diagnostic crossover study. The reliability cohort was tested with LSS-4 on different days (n = 30 eyes). The screen independence cohort used LSS-8 with 11-inch versus 13-inch screens in random order (n = 27 eyes). Brightness independence testing used L

Radiologists' recommendations following MRI for optic neuritis: influence of report wording and patient accessibility in a tertiary neuro-ophthalmology clinic.

OBJECTIVE: Optic neuritis is often managed by neuro-ophthalmologists. Neuro-imaging plays an important role in diagnosis. Radiologists can influence care by communicating pertinent findings and recommending next steps. Increasingly, patients have direct access to their reports, which may affect their understanding, anxiety, and interactions with clinicians. This study aimed to characterize the frequency and nature of radiologists' recommendations in magnetic resonance imaging (MRI) reports for optic neuritis. DESIGN: A retrospective chart review. PARTICIPANTS: Patients referred to a tertiary

Case Report: Presenting as optic neuritis-a biopsy-proven IgG4 anti-NF155-positive combined central and peripheral demyelination syndrome.

Combined central and peripheral demyelination (CCPD) is a condition characterized by concurrent demyelination in both the central and peripheral nervous systems. To date, cases of CCPD presenting with isolated optic neuritis as the initial manifestation and positive for anti-neurofascin 155 (anti-NF155) antibodies are exceedingly rare. We herein report a case of a young male positive for anti-NF155 antibodies who developed severe optic neuritis and was subsequently diagnosed with CCPD via histopathological examination. This case highlights that CCPD may develop in patients presenting with opti
