Concomitant dominant optic atrophy and juvenile glaucoma in two siblings with a novel OPA1 splicing variant.

PURPOSE: We report the clinical history of two siblings, initially diagnosed with juvenile glaucoma (JG), who were subsequently found to harbor a novel pathogenic OPA1 splicing variant consistent with dominant optic atrophy (DOA). METHODS AND RESULTS: The male proband presented with elevated intraocular pressure (IOP) at age 11, while his sister had normal IOP values at age 16. Both developed bilateral temporal optic nerve pallor, central visual field defects, and reduced color vision. Optical coherence tomography (OCT) confirmed thinning of the retinal nerve fiber and ganglion cell layers. W

IL-15 Links Muscle-Kidney Crosstalk to Preserving Podocyte Mitochondrial Fusion and Attenuating Diabetic Nephropathy.

OBJECTIVES: High glucose induces mitochondrial dysfunction in podocytes, contributing to the development of diabetic nephropathy (DN). There is increasing evidence that muscles play a protective role by secreting myokines into the kidneys. Here, we investigated how skeletal muscle influences podocyte health via muscle-kidney crosstalk. METHODS: To increase myokine release, we overexpressed PGC-1α specifically in skeletal muscle (mPGC-1α) and crossed these mice with db/m mice to generate diabetic mPGC-1α:db/db mice. In parallel, db/db mice were treated intraperitoneally with recombinant murine

Optic Atrophy Associated With a Mitochondrial G8363A Mutation in a Family.

We report a rare family of optic atrophy diseases associated with mitochondrial DNA G8363A transfer ribonucleic acid (RNA) cleavage mutations. This family does not exhibit the characteristic symptoms of myoclonic epilepsy or cutaneous lipomas, but instead presents with bilateral optic atrophy. The clinical presentation in this family is highly atypical compared to previous reports.

Oxidative phosphorylation and mitochondrial dynamics are regulated by Sestrin2 to maintain cellular function.

The stress-inducible protein Sestrin2 (SESN2) has recently emerged as an orchestrator of mitochondrial signaling. The regulation of mitochondria-related pathways, such as aerobic respiration, is thought to be mediated by SESN2, but the underlying mechanisms are not fully understood. Here, we characterized mitochondria in Sesn2-knockdown myoblasts under physiological conditions using oxygen consumption rate measurements, fluorescence microscopy, and protein content analysis. We discovered that SESN2 is essential for sustaining oxidative phosphorylation and maintaining the mitochondrial network

Znhit3 regulates p53/p21 signaling and governs cerebellar granule cell development.

Mutations in ZNHIT3 are strongly associated with progressive encephalopathy with edema, hypsarrhythmia and optic atrophy (PEHO syndrome), characterized by severe cerebellar atrophy and profound intellectual disability; however, their role in cerebellar development remains unknown. By developing spatiotemporally-regulated conditional Znhit3 knockout mice, we discovered that Znhit3 is essential for granule cell progenitor survival, proliferation, differentiation, and migration. Knockout of Znhit3 caused loss of granule cell progenitors due to apoptosis, premature cell-cycle exit, and migration a

Neuroinflammation-Associated Optic Structural-Functional Degeneration in Early Diabetic Optic Neuropathy.

BACKGROUND: Diabetic optic neuropathy (DON) is characterized by optic nerve degenerative changes, which may progress to optic atrophy and even blindness. Although DON is relatively common, its early diagnosis remains challenging. This study employed a noninvasive in vivo imaging combined histopathological assessment system to investigate changes in optic structural-functional degeneration in DON models. METHODS: Animals were divided into experimental group (8 db/db mice) and control group (8 db/m mice). Blood glucose, body weight, intraocular pressure, peripapillary retinal nerve fiber layer

A multistage cost-effective strategy for the molecular diagnosis of unexplained vision loss patients: practice in inherited ocular fundus disease.

Inherited ocular fundus diseases are the most common causes of blindness with high heterogeneity. We established a tiered strategy for the molecular diagnosis of unexplained vision loss patients. Patients were screened with ophthalmological examinations followed by a tiered genetic diagnosis, including mitochondrial genome sequencing, multigene panel and whole exome sequencing. A total of 146 individuals with unexplained vision loss were enrolled, including 103 individuals with abnormal pattern visual evoked potential and 43 individuals with abnormal optic coherence tomography. Evaluation of a Child With Optic Atrophy.

Optic atrophy is an optic neuropathy that results from permanent damage to the axons and retinal ganglion cells of the optic nerve, causing irreversible vision loss. Optic atrophy is a major cause of vision loss in children worldwide and has many etiologies. Herein, we gather and explain the common etiologies of pediatric optic atrophy and provide insights on history-taking, examination, workup, and clinical decision-making for the general ophthalmologist. We highlight the importance of a comprehensive approach to evaluation and coordination of care for vision services in children with irrever

Expanding the AFG3L2 Spectrum: A Link to Axonal Neuropathy.

OBJECTIVES: The AFG3L2 gene encodes a mitochondrial AAA-protease involved in inner mitochondrial membrane (IMM) proteostasis. Heterozygous variants in the AFG3L2 proteolytic domain cause Spinocerebellar Ataxia type 28, heterozygous variants in the AFG3L2 ATPase domain cause Optic Atrophy type 12, while biallelic variants lead to recessive Spastic Ataxia type 5. In this study, we aimed to investigate the link between AFG3L2 haploinsufficiency and Charcot-Marie-Tooth (CMT) phenotypes. METHODS: We performed clinical evaluation, genetic analyses, electrophysiology, and functional studies in 1 pat

Using Optical Coherence Tomography in Pediatric Neuro-ophthalmology.

Optical coherence tomography (OCT) has transformed adult neuro-ophthalmology, but pediatric adoption has lagged because of perceived technical, financial, and practical barriers. This chapter provides a pragmatic framework for integrating OCT into pediatric and pediatric neuro-ophthalmology clinics, emphasizing how to obtain reliable images in awake children and how to interpret them. We first review technical aspects of tabletop and handheld OCT in children, including positioning, fixation strategies, and when to prioritize single-line macular or optic nerve head B-scans over full segmented p

Early-Onset Hyperkinetic Movement Disorders Define the Most Severe Presentation of the ATP8A2-Related Phenotypic Spectrum.

Variants in ATP8A2 gene have been traditionally associated with cerebellar ataxia, mental retardation and disequilibrium syndrome type 4 (CAMRQ4). However, this nomenclature fails to capture the predominantly extrapyramidal and encephalopathic nature of the most severe presentation of the ATP8A2-related phenotypic spectrum. We report two siblings with a novel homozygous ATP8A2 frameshift variant (p.Ser839Glyfs*21) presenting with a complex neurodevelopmental encephalopathy. Their phenotype was characterized by very early-onset hyperkinetic movement disorders, including chorea, dystonia and myo

Synaptic Alterations Are Preceding the Axonal Loss in Optic Atrophy of Wolfram Syndrome Mouse Model.

Wolfram syndrome is a rare autosomal recessive disorder characterized by antibody-negative early-onset diabetes mellitus, optic atrophy, sensorineural hearing loss, arginine-vasopressin deficiency, and progressive neurodegeneration of the brainstem and cerebellum. It is caused primarily by pathogenic variants in the WFS1 gene, which encodes a transmembrane endoplasmic reticulum-resident protein involved in the unfolded protein response and cellular calcium homeostasis. Although multiple rodent models of Wolfram syndrome have been developed and shown to exhibit visual defects, some studies have

Genotype-Based Severity Scoring System in Wolfram Syndrome: Correlation with Onset of Cardinal Symptoms and WFS1 Gene Variant Types.

Wolfram syndrome is a rare genetic disorder characterized by antibody-negative early-onset atypical diabetes mellitus, optic nerve atrophy, sensorineural hearing loss, diabetes insipidus (arginine vasopressin deficiency), and progressive neurodegeneration, with significant variability in disease severity. We assessed the accuracy of a genotype-based severity scoring system to predict the onset of cardinal symptoms in Wolfram syndrome. This system is based on the type of WFS1 variants (in-frame or out-of-frame) and their location relative to transmembrane domains. Severity scores were assigned

Viral vector-mediated SLC9A6 gene replacement reduces cerebellar motor and molecular abnormalities in the shaker rat model of Christianson syndrome.

BACKGROUND: Christianson syndrome is an x-linked recessive neurodevelopmental and neurodegenerative condition caused by mutations to the SLC9A6 gene encoding NHE6, a sodium-hydrogen exchanger critical for regulating endosomal pH. Using an adeno-associated viral (AAV) vector targeting Purkinje cells (PHP.eB-L7-Slc9a6-GFP), we recently performed functional complementation studies to demonstrate that mutation to the rat Slc9a6 gene causes a Christianson syndrome-relevant cerebellar phenotype in the shaker rat. METHODS: We carried out a longitudinal study evaluating the impact of gene replacement
