ATP synthase (complex V) catalyzes ATP synthesis and is composed of the F1 catalytic sector and the F0 proton-conducting sector. The e subunit of the F0 sector, encoded by ATP5ME, is essential for complex V dimerization and cristae organization; however, genetic variants in ATP5ME have not yet been implicated in human disease. We ascertained a 4-year-old male, born of a consanguineous marriage, who presented with neuroregression, feeding difficulty, spasticity, encephalopathy, bilateral sensorineural hearing loss and optic atrophy. Exome sequencing identified a biallelic 62 bp deletion, c.-48_

BACKGROUND: Lung adenocarcinoma (LUAD) remains a prevalent malignant tumor characterized by a dismal prognosis. This study aimed to reveal potential mitochondria-related biomarkers and explore possible mechanisms.
METHODS: Mitochondria-related differentially expressed genes (mitoDEGs) were identified by intersecting LUAD-associated DEGs with mitochondria-associated genes. Hub mitoDEGs were determined via protein-protein interaction (PPI) network analysis and machine learning. Aldehyde dehydrogenase 18 family member A1 (ALDH18A1) was silenced in LUAD cells to investigate its effects on prolife

BACKGROUND: Endophthalmitis is a blinding complication following any invasive ocular procedure, and historical treatment guidelines do exist but only for cataract surgery cases. To the best of our knowledge, no randomised controlled trial evidence exists to answer the question of early vitrectomy surgery for the treatment of endophthalmitis following any eye surgery.
METHODS: We conducted a multicentre, national prospective randomized controlled clinical (RCT) trial aimed to assess the feasibility of randomizing patients to an early vitrectomy (Arm-A) intervention within 48-96 hours compared

We present a case and literature review regarding posterior ischemic optic neuropathy (PION) in the context of orbital hemorrhage following cosmetic blepharoplasty. A 49-year-old male patient, who underwent resection of residual fat from a previous blepharoplasty, developed acute orbital hemorrhage six hours postoperatively, with sudden visual loss, painful proptosis, and a relative afferent pupillary defect. Urgent decompressive surgery was performed. Ultimately, the patient did not experience visual improvement, and optic atrophy was evident one month later, confirming the diagnosis of PION.

Wolfram syndrome is a rare inherited neurodegenerative disorder, in which early ophthalmologic abnormalities may provide the initial diagnostic clue. In this article, we report the case of a 20-year-old male with early-onset bilateral deafness and insulin-dependent diabetes mellitus who was referred for evaluation of possible diabetic retinopathy. He reported no visual complaints at the time of presentation. Examination showed a visual acuity of 2/10 bilaterally, and optic disc pallor on fundoscopy without diabetic retinopathy. Further complementary testing revealed diffuse visual field defect

Global plastic production has led to widespread contamination by micro- and nanoplastics, with polystyrene nanoplastics (PSNPs) increasingly being detected in human biological samples, including blood and cardiac tissue. Given the critical role of mitochondria in cardiac energy metabolism, this study investigated whether 100 nm PSNPs interact with mitochondria and affect mitochondrial function in H9c2 cardiomyoblasts. Cellular uptake and intracellular distribution were examined, followed by an evaluation of mitochondrial ultrastructure, intracellular and mitochondrial reactive oxygen species (

We report a very rare autosomal recessive metabolic disorder in monozygotic twin sisters caused by the steroid 5a-reductase type 3 (SRD5A3) gene defect, a subtype of congenital disorder of glycosylation (CDG). SRD5A3 activity is required for N-glycosylation of proteins. This step is important for the protein to gain its function. The condition is characterized by severe neurodevelopmental delay, cerebellar atrophy or hypoplasia, ocular abnormalities, and ichthyotic skin changes. We describe 20-month-old female monozygotic twins born to non-consanguineous South Asian parents. Notably, both twin

PURPOSE: Dominant optic atrophy (DOA) is a rare disease characterized by the chronic loss of retinal ganglion cells that transduce the visual information from the retina to the brain. Dominant optic atrophy shows interfamilial and intrafamilial phenotypic variations and a restricted 35% molecular diagnosis, with half presenting a pathogenic variant in optic atrophy 1 (OPA1), encoding a large mitochondrial GTPase. Here, we describe the largest ever identified DOA family, with 64 nonsyndromic individuals harboring a novel 10-kb OPA1 deletion.
DESIGN: Retrospective, longitudinal cohort study of

AIM: To identify early molecular diagnostic biomarkers for optic atrophy (OPA) and explore potential mechanisms mediated by proteins.
METHODS: Gene expression data was sourced from eQTLGen (31 684 samples; 19 960 genes). The OPA discovery cohort came from FinnGen (629 cases; 496 621 controls); the validation cohort came from the genome-wide association studies (GWAS) catalog (58 cases; 496 621 controls). Protein data for mediation analysis was obtained from the deCODE Genetics consortium (35 559 samples; 4907 proteins). Causal estimates were derived using two-sample Mendelian randomization (M

Leber hereditary optic neuropathy (LHON) is the most common mitochondrial disorder, typically causing substantial, often permanent, central vision loss in young adults. It manifests as a subacute optic neuropathy, frequently progressing sequentially in both eyes, due to selective degeneration of retinal ganglion cells (RGCs). The condition is primarily associated with three mitochondrial DNA (mtDNA) point mutations-m.11778G>A, m.14484T>C, and m.3460G>A-located in complex I of the mitochondrial respiratory chain. These mutations impair oxidative phosphorylation, elevate reactive oxygen species

Isolated cone dystrophy is a rare retinal degenerative disorder characterized by selective dysfunction of cone photoreceptors, with relatively preserved rod function. Clinical presentation can be variable and, in rare cases, may mimic retinal vascular injuries or optic neuropathies, leading to diagnostic uncertainty. We report the case of a 32-year-old male patient with gradual, painless, asymmetric vision loss predominantly affecting the left eye. Fundus examination revealed optic disc pallor, arteriolar narrowing, macular pigmentary changes, and absence of the foveal reflex, raising initial

Leber's hereditary optic neuropathy (LHON) is a genetically inherited disease of the eye triggered by mtDNA mutations, leading to degeneration of RGCs. We previously reported that the mitochondrial tRNAThr (MT-TT) 15927G > A homoplasmic mutation disrupted the base pairing (28C-42G) conserved in the anticodon stem of tRNAThr, impairing t6A modification, aminoacylation, and steady-state tRNAThr levels, ultimately resulting in mitochondrial dysfunction. However, the absence of suitable animal and cell models for LHON has delayed efforts to elucidate disease pathophysiology, particularly tissue-sp

Wolfram syndrome is a rare childhood neurodegenerative disease characterized by diabetes followed by severe and rapid optic atrophy leading to blindness before the age of 20. Patients often develop other symptoms, such as deafness and neurological dysfunction. Wolfram syndrome is caused by mutations in the WFS1 gene, which encodes wolframin protein. Despite decades of intensive research, the complex mechanisms of optic neuropathy are not fully understood, and there are currently no therapies to prevent vision loss in Wolfram patients. Here, we showed that the Wfs1 knockout mice produced by the

Peripheral sympathetic neurodegeneration drives cardiac dysfunction in dominant optic atrophy, revealing a critical neuro-cardiac link. Optic atrophy factor-1 haploinsufficiency disrupts mitochondrial dynamics and neurotrophic signaling, causing targeted sympathetic denervation and arrhythmias. Restoring nerve growth factor transport and mitochondrial health in sympathetic neurons represents a promising therapeutic avenue for cardiac autonomic disorders. Future research must unravel mechanisms of neurocardiac crosstalk to develop precise interventions against neurogenic cardiac disease progres

BACKGROUND: Mitochondrial diseases comprise a genetically and clinically heterogeneous group of disorders primarily resulting from defects in oxidative phosphorylation, leading to impaired ATP synthesis and cellular energy deficiency. The functional competence of mitochondria critically depends on the ultrastructural integrity of the inner mitochondrial membrane, notably the cristae, which serve as the platform for respiratory chain complexes. The mitochondrial contact site and cristae organizing system complex (MICOS) is essential for maintaining cristae junction architecture and inner membra

Most patients with a rare movement disorder (MD) do not receive a molecular diagnosis, and the underlying genetic variants and mediating genes remain elusive. Here, we evaluate the diagnostic accuracy of conventional and next-generation sequencing-based genetic testing strategies in a cohort of 2,811 individuals with ataxia, spastic paraplegia and dystonia. Exome sequencing establishes genetic diagnoses in 19.3% of cases, and specificity of phenotypic features and age at testing are positive predictors. Genome analysis 'beyond the exome' increases the diagnostic yield by 7.5%, mostly due to th

OBJECTIVE: Complex I (CI) deficiency, the most common biochemical defect in pediatric mitochondrial diseases, presents with diverse phenotypes, including cardiomyopathy, myopathy, Leigh syndrome, and mitochondrial leukoencephalopathy (ML). No curative therapies exist. Riboflavin, a precursor of CI cofactors FMN and FAD, is a potential treatment, but evidence is heterogeneous and formal guidelines are lacking.
METHODS: We retrospectively analyzed two patients with genetically confirmed CI deficiency due to NDUFS1 and NDUFV2 variants, treated with high-dose riboflavin with long-term clinical, b

We report the case of a 22-month-old boy with retinal vascular disease caused by optic neuritis due to myelin oligodendrocyte glycoprotein antibody disease. He presented with acute disseminated encephalomyelitis without optic nerve involvement. Despite treatment, he developed optic neuritis and vision loss within days. In addition to the usual findings of optic neuritis, he had evidence of severe retinal venous stasis including intraretinal, subretinal, preretinal, and choroidal hemorrhages that were rapidly progressive. The retinal perfusion did not recover, leading to neovascularization. Thi
