[Case series: anterior and posterior manifestations of scleritis].

Scleritis is a potentially sight-threatening inflammation with a broad spectrum of anterior and posterior manifestations. In addition to characteristic pain and the risk of scleral thinning, posterior segment involvement is of particular clinical importance. It may cause rapid visual deterioration and present with findings suggestive of other inflammatory, vascular, or neuro-ophthalmic disorders, including venous stasis and neuroretinitis. This case series illustrates anterior and posterior manifestations of scleritis, with particular emphasis on retinal and optic disc involvement. These cases

Bilateral Painless Vision Loss From Probable Leptomeningeal Metastasis During Systemic Response in Esophageal Adenocarcinoma.

Leptomeningeal metastasis (LM) is a rare complication of esophageal adenocarcinoma, with reported cases typically presenting with meningitic, encephalopathic, or multifocal neurologic symptoms. We report a 70-year-old man with metastatic esophageal adenocarcinoma with signet-ring cell features who developed subacute, painless, bilateral visual loss with bilateral optic disc edema as the initial manifestation of probable LM. Symptoms arose four months into first-line folinic acid, fluorouracil, and oxaliplatin (FOLFOX) plus pembrolizumab while all measurable extracranial disease was responding

Human T-lymphotropic Virus Type 1 (HTLV-1)-Associated Uveitis Presenting With Frosted Retinal Vasculitis.

Retinal vasculitis is a manifestation observed in various forms of uveitis. However, frosted retinal vasculitis, a severe type of retinal vasculitis, is seen in only a limited number of cases, typically associated with severe infections or autoimmune inflammatory conditions. A 65-year-old woman presented to our hospital after noticing a decline in vision in both eyes over the preceding month. Examination revealed vitritis and frosted retinal vasculitis in both eyes. Fluorescein angiography showed marked leakage from the optic disc and retinal vessels. Optical coherence tomography (OCT) reveale

Recurrent Left Abducens Nerve Palsy and Idiopathic Intracranial Hypertension During Pregnancy: A Diagnostic Challenge.

Idiopathic intracranial hypertension (IIH) is characterized by elevated intracranial pressure without an identifiable mass lesion, hydrocephalus, cerebral venous sinus thrombosis, or abnormal cerebrospinal fluid composition. Diagnosis during pregnancy can be challenging because headache and visual symptoms overlap with several neurologic and obstetric disorders. We report a previously healthy 20-year-old primigravida with a body mass index of approximately 33 kg/m² who presented at 17 weeks' gestation with intermittent headache, photophobia, blurred vision, horizontal diplopia, nasal deviation

A spatial multi-omics atlas of immunosenescence reveals germinal-center B cell dysfunction in human lymph nodes.

PURPOSE: While it has been well established that advanced glaucoma is associated with a large cup-to-disc ratio (CDR), it is not known if the shape of the cup, particularly excavated cups with expanded lower portions (bean-pot cups), play any additional role in glaucomatous damage. We investigated this among individuals of African ancestry, a population that is more vulnerable to glaucoma than any other ethnic group. DESIGN: Case-control study. METHODS: Setting:: Institutional (University of Pennsylvania)Subjects:: 3,255 eyes from 1,734 glaucoma cases from the Primary Open-Angle African Amer

Prediction of Optic Disc Edema Progression During Spaceflight.

IMPORTANCE: There are limited means to predict which crewmembers are at risk for developing optic disc edema (ODE) during spaceflight. Identifying crewmembers who have a low probability of developing ODE will allow for reduced monitoring and countermeasure resources, thereby enabling alternate mission activities. OBJECTIVE: To determine if magnitude of ODE early in flight is associated with magnitude of ODE later in flight. DESIGN, SETTING, AND PARTICIPANTS: This cohort study involved crewmembers on board the International Space Station. Participants were astronauts who had optical coherence

Corticosteroid-responsive optic neuritis associated with acute retinal necrosis due to Herpes simplex virus type 2 in a pediatric patient.

PURPOSE: Optic neuritis (ON) associated with acute retinal necrosis (ARN) has traditionally been considered a complication resulting from direct viral infiltration of the optic nerve. Although it is currently treated with intravenous acyclovir, the appropriate dosage and duration of treatment in the pediatric population remain unclear, and no standardized effective treatment has been established in clinical guidelines. METHODS: A 14-year-old boy was referred for panuveitis in the right eye (RE), with a visual acuity (VA) of hand motions. Fundus examination revealed intense vitritis, and PCR t

How to diagnose papilloedema.

Papilloedema is optic disc swelling due to raised intracranial pressure (ICP). Typically, this occurs bilaterally, and visual function is usually relatively preserved unless the disease is moderately severe or chronic. The implications of a new finding of papilloedema are potentially serious; however, overdiagnosis of papilloedema can lead to iatrogenic harm from unnecessary investigation or treatment. It is important to consider the differential diagnosis for swollen optic disc appearances to ensure patients are investigated appropriately. We describe an approach to the clinical assessment of

Orbital and Meningeal Metastases Secondary to Breast Adenocarcinoma: A Case Report.

Orbital metastasis and meningeal carcinomatosis (MC) are uncommon complications of breast cancer and typically indicate advanced disease with poor prognosis. While breast cancer is the most frequent primary tumor associated with orbital metastases in women, the simultaneous occurrence of orbital metastasis and MC is exceedingly rare. We present the case of a 51-year-old female patient diagnosed with stage IV breast adenocarcinoma, previously diagnosed with controlled HIV infection, who presented with posterior cervical pain, progressive bilateral visual loss, and left blepharoptosis. Normative optical coherence tomography-based measurements and classification of optic nerve head parameters in adult Congolese subjects.

BACKGROUND: Normative data on optic nerve head (ONH) topographic measurements are scarce among black people residing in Africa. Such measurements obtained with imaging modalities such as optical coherence tomography (OCT) are crucial for the diagnosis and management of optic nerve diseases, particularly glaucoma. They assist clinicians in identifying deviations that may indicate disease or abnormalities. The aim of this study was to determine normal reference values of ONH topographic measurements in a Congolese adult population using OCT. METHODS: Hospital-based cross-sectional observational

AI to Identify Strain-Sensitive Regions of the Optic Nerve Head Linked to Functional Loss in Glaucoma.

PURPOSE: The purposes of this study were to assess whether optic nerve head (ONH) biomechanics, quantified by tissue strain, improves classification of progressive visual field (VF) loss patterns in glaucoma beyond morphology, and to use saliency maps to identify ONH regions associated with the predictions. METHODS: We recruited 249 patients with glaucoma (mean age 69 ± 5 years, 54% female patients). One eye per subject was imaged under (1) primary gaze and (2) primary gaze with IOP elevated to approximately 35 millimeters of mercury (mm Hg) via ophthalmo-dynamometry. Twelve subjects were exc

Routine Fundoscopy Uncovering Wolfram Syndrome in a Diabetic Patient: A Case Report.

Wolfram syndrome is a rare inherited neurodegenerative disorder, in which early ophthalmologic abnormalities may provide the initial diagnostic clue. In this article, we report the case of a 20-year-old male with early-onset bilateral deafness and insulin-dependent diabetes mellitus who was referred for evaluation of possible diabetic retinopathy. He reported no visual complaints at the time of presentation. Examination showed a visual acuity of 2/10 bilaterally, and optic disc pallor on fundoscopy without diabetic retinopathy. Further complementary testing revealed diffuse visual field defect

Detecting glaucoma progression through optic nerve head hemoglobin concentration using automated colorimetric analysis.

PurposeTo evaluate whether an algorithm that quantifies changes in hemoglobin (Hb) concentration in the optic nerve head (ONH) can detect progression in glaucomatous eyes with progressive visual field changes.MethodsThis retrospective cohort study involved eyes with primary open-angle glaucoma (POAG). Eyes were required to have at least five Standard Automated Perimetry (SAP) tests and three ONH photos during follow-up. ONH photos were processed using the Laguna ONhE software (Retinalyze, Spain). The software estimates Hb concentration at the ONH using automated colorimetric analysis. Visual f

Neuro-Ophthalmic Presentation of Steroid 5a-Reductase Type 3 Congenital Disorder of Glycosylation: A Case of Monozygotic Twins.

We report a very rare autosomal recessive metabolic disorder in monozygotic twin sisters caused by the steroid 5a-reductase type 3 (SRD5A3) gene defect, a subtype of congenital disorder of glycosylation (CDG). SRD5A3 activity is required for N-glycosylation of proteins. This step is important for the protein to gain its function. The condition is characterized by severe neurodevelopmental delay, cerebellar atrophy or hypoplasia, ocular abnormalities, and ichthyotic skin changes. We describe 20-month-old female monozygotic twins born to non-consanguineous South Asian parents. Notably, both twin
