Haptic-Extended Flange Intrascleral Fixation for Single-Piece Acrylic Intraocular Lenses.

Background/Objective: Whereas conventional flanged intrascleral fixation is limited to 3-piece intraocular lenses (IOLs), we developed and evaluated a novel haptic-extended flange technique for single-piece acrylic IOLs, including presbyopia-correcting and toric lenses. Methods: In this technique, a 6-0 polypropylene suture is secured along the lateral aspect of the single-piece IOL haptic to extend its length, followed by intrascleral flange fixation. Uncorrected visual acuity (UCVA) from far to near, corrected distance visual acuity (CDVA), and refraction were evaluated at 1 month postoperat

Theta burst stimulation of the visual cortex does not alter contrast sensitivity under optically induced blur.

This study investigated whether a single session of repetitive transcranial magnetic stimulation (TMS), specifically theta burst stimulation (TBS), acutely modulates visual function under conditions of induced optical defocus. Twelve adults with normal or corrected-to-normal vision participated in a pre-registered, within-subject, double-blind, sham-controlled study. Participants first adapted to an experimentally induced + 1.50D optical blur for 10 minutes. Following this, stimulation was applied to the primary visual cortex according to a pre-randomised allocation. Stimulation consisted of c

Tonic accommodation as the physiological baseline of the accommodative system.

Understanding tonic accommodation as the physiologic baseline of the accommodative system may help clinicians interpret accommodative symptoms, refractive variability, presbyopic change, and responses to myopia-control and accommodative therapies. Accommodation is traditionally described relative to the optical far point of the eye, implicitly assuming that the distance-focused state represents physiological rest. However, experimental studies performed under conditions of minimal visual stimulation consistently demonstrate that the eye adopts an intermediate focal position known as tonic acco

Mechanical basis of cortical cataract.

PURPOSE: Accommodation, the ability of the human eye to adjust focus, declines with age due to progressive lens stiffening. This study hypothesizes that cycles of attempted accommodation and relaxation in presbyopic eyes (≥ 50 years) induces internal deformations in the equatorial cortex of the lens, contributing to cortical cataract formation. METHODS: Finite element simulations were performed to evaluate internal strains and stresses in the equatorial cortex of lenses of different ages caused by ciliary body movements during accommodation or attempted accommodation. The models incorporated

Visual and Refractive Outcomes of Corneal Refractive Surgery for Hyperopic and Mixed Astigmatism: A Systematic Review and Meta-Analysis.

TOPIC: To evaluate the visual and refractive outcomes of corneal refractive surgery (PRK, LASIK, and SMILE) for hyperopic and mixed astigmatism. The clinical question addressed was: in adults with hyperopic or mixed astigmatism, how effective and safe are modern corneal refractive procedures, without alternative surgical comparators, in reducing refractive cylinder and improving uncorrected distance visual acuity? CLINICAL RELEVANCE: Hyperopic and mixed astigmatism are less predictable to correct than myopic errors due to biomechanical and ablation-profile complexity. Historically, these trea

Binocular Summation in Healthy Presbyopic Adults: Contrast Sensitivity and Defocus Curves.

Background/Objective: Binocular summation, the improvement in visual performance under binocular compared with monocular viewing, depends on the visual task and the degree of optical degradation of the retinal image. Its behavior across the full defocus curve in presbyopia remains poorly characterized. The purpose is to quantify binocular summation in healthy presbyopic adults by assessing contrast sensitivity (CS) at intermediate and near distances and by comparing monocular and binocular defocus curves to evaluate the effect of binocularity on depth of focus, the area under the curve (AUC),

Isolated Bilateral Foveal Hypoplasia Diagnosed in Adulthood Following Evaluation for Presbyopia.

Foveal hypoplasia, also known as fovea plana, is an uncommon congenital anomaly caused by incomplete foveal differentiation during ocular development. Although it is usually diagnosed in childhood, particularly in patients with associated ocular or systemic disorders, isolated forms may remain unrecognized until adulthood. We report the case of a 45-year-old woman who presented for the first time with persistent near-vision difficulty, initially attributed to presbyopia. Despite appropriate refractive correction, her visual symptoms did not fully improve. Ophthalmic examination revealed bilate

Patient-reported outcome measures after multifocal and extended depth of focus intraocular lens implantation in refractive lens exchange and cataract surgery.

Presbyopia-correcting intraocular lenses (IOLs) represent a fast-growing segment of ophthalmic surgery. The evaluation of lens performance has undergone a fundamental shift: best-corrected visual acuity alone is an insufficient measure of the clinical benefit of premium IOLs, whose true value lies in spectacle independence, visual quality, and visual function in daily activities. In that regard, patient-reported outcome measures (PROMs) are a cornerstone of presbyopia-correcting IOL assessment. This review provides an overview of the current landscape of IOLs, PROM instruments and their limita

Demonstration of an optical full adder and subtractor for phase-encoded data using nonlinear wave mixing.

BACKGROUND: Contact lenses are a cornerstone of vision correction for ametropia and can be of particular value for management of irregular astigmatism and corneal ectasia. Beyond objective measures of visual acuity and corneal health, patient-reported outcomes and vision-related quality of life (VR-QoL) have emerged as critical endpoints. This review aimed to synthesize evidence on VR-QoL outcomes with contact lenses. METHODS: We conducted a narrative review of studies published between 2000 and 2025 on the influence of various contact lens modalities including soft conventional lenses, rigid

NAD+-sirtuin-mitochondrial quality control in lens epithelial cells: a candidate modulatory network in crystalline lens aging.

Crystalline lens aging reflects parallel processes: intrinsic chemical modification of exceptionally long-lived fiber-cell proteins, deterioration of lens-wide redox and transport homeostasis, and declining maintenance in metabolically active cellular compartments. Lens epithelial cells (LECs) support antioxidant defense, ion and water balance, stress responses, and the quality of newly formed fibers. Current lens and LEC evidence shows age-associated reductions in mitochondrial oxygen consumption in primary human LECs obtained during cataract surgery; SIRT1-related signaling is linked to epit
