Endoscopic posterior transchoroidal approach for biopsy and fenestration of lesions of the posterior tentorial incisura: a case series and review of the literature.

Objective. To describe the anatomical rationale, surgical technique, and outcomes of a purely endoscopic posterior transchoroidal approach (EPTCA) to lesions of the tentorial incisura, and to contextualise these results within the existing literature on endoscopic transchoroidal procedures. Methods. We retrospectively reviewed all patients who underwent EPTCA for lesions of the posterior tentorial incisura at our institution between 2012 and 2024. All procedures were performed via a single frontal burr hole, combining ETV when indicated with a posterior transchoroidal opening at the body–atriu

Bimedial rectus plication in Residual exotropia.

Introduction: Residual exotropia refers to a constant outward deviation of eye noted after strabismus surgery for exotropia once postoperative healing is complete (≥6-8 weeks). Management of residual exodeviation necessitates careful evaluation and meticulous surgical planning to achieve optimal outcomes. Methods: A 28-year-old male presented with a 40 prism-diopter residual left exotropia associated with poor convergence. He had a history of childhood strabismus surgery in the left eye; however, the operative records were not available. Preoperative anterior segment optical coherence tomograp

Isolated fourth cranial nerve palsy in a patient treated with pembrolizumab: a case report.

Introduction: Pembrolizumab is widely used in melanoma treatment and may rarely be associated with neuro-ophthalmic immune-related adverse events. Isolated ocular motor nerve palsies are exceptional. Methods: We report a 76-year-old man who presented with sudden vertical binocular diplopia six weeks after starting adjuvant pembrolizumab following resection of cutaneous melanoma. A comprehensive neurological, ophthalmological, laboratory, and neuroimaging assessment was performed. Results: Ocular motility examination revealed an isolated right superior oblique palsy. Brain and orbital MRI showe

Neural responses to binocular in-phase and anti-phase stimuli.

Binocular vision fuses compatible inputs from the two eyes into a single percept, whereas incompatible inputs can produce rivalry, lustre, or diplopia. We measured neural responses to binocular stimuli with different phase relationships to test predictions from contemporary binocular combination models. Steady-State Visually Evoked Potentials (SSVEPs) were recorded from 15 observers in response to monocular and binocular stimulation at 3 Hz, using either On/Off or counterphase flicker with varied spatial and temporal phase relationships. On/Off flicker elicited responses at the fundamental fre

The association between diplopia and clinical phenotypes in spinocerebellar ataxia type 3.

BACKGROUND: Spinocerebellar ataxia type 3 (SCA3) is a rare monogenic hereditary neurodegenerative disease. It is the most common form of spinocerebellar ataxia worldwide, with diplopia being one of its most frequent symptoms. Diplopia has been reported to be associated with clinical phenotypes and daily living activities in various neurodegenerative diseases. OBJECTIVES: Our objective is to investigate the association between diplopia and the clinical phenotype of SCA3. METHODS: We conducted a retrospective analysis of 182 patients with SCA3. Participants were categorized into two groups based

Ophthalmic manifestations of Chiari malformation: a systematic review.

OBJECTIVE: Chiari malformation (CM) comprises congenital or acquired hindbrain herniations (>5-mm cerebellar tonsillar descent below the foramen magnum) that disrupt cerebrospinal fluid flow and compress neural structures. While neurological symptoms are well recognized, ophthalmic manifestations, such as papilledema, diplopia, and nystagmus, which are uncommon in the overall CM population but may serve as important diagnostic clues when present, remain undersynthesized. This systematic review summarizes the ophthalmic manifestations of CM to identify symptom patterns, mechanisms, and implicat

Navigating the delicate balance of the transorbital approach.

The transorbital approach (TOA) is a significant advance in minimally invasive neurosurgery, eliminating the need for the extensive craniotomy and brain retraction typically associated with traditional methods. By accessing deep-brain and skull base lesions through a small periorbital incision, TOA significantly reduces surgical trauma, accelerates recovery, and offers superior esthetics. The well-equipped instruments and navigation system provide a magnified, meticulous view, proving effective for complex lesions near the orbital-cranial junction. However, the technique is challenged by the "

Bony realignment after surgical removal of orbital implantation cysts: A case series.

Orbital cyst formation following previous orbital fracture surgery is an uncommon complication, yet it poses a significant concern related to permanent implant use. This complication may result in a pronounced mass effect that leads to deformation of the orbital bony walls. We present the three cases of orbital implantation cysts associated with orbital wall deformity, where bony alignment normalized after complete cyst removal. Patients exhibited ocular symptoms, including proptosis, diplopia, and hyperglobus. Surgical management involved total resection of the orbital cyst, removal of the pr

Anti-GQ1b-Positive Miller Fisher Syndrome Following Pfizer Bivalent COVID-19 Vaccination.

Miller Fisher syndrome (MFS) is a rare, immune-mediated variant of Guillain-Barré syndrome characterized by acute onset of ataxia, areflexia, and ophthalmoplegia. It typically develops following an antecedent event, such as an infection or, in rare instances, a vaccination, which triggers the production of anti-ganglioside antibodies. While cases have been anecdotally linked to COVID-19 vaccination, serologically confirmed reports remain uncommon. We report a case of a 51-year-old, previously independent male who presented with acute neurological deterioration, including ascending numbness, se

Rib-Origin Ewing Sarcoma With Cranial Nerve Involvement From Orbital Metastasis: A Rare Case Report.

Ewing sarcoma (ES) is an aggressive tumor of the bone in young adults, commonly originating from the pelvis and long bones. It is characterized by chromosomal translocation t(11;22) (q24;q12), resulting in the EWSR1-FLI1 fusion gene. It presents with metastasis commonly to the lungs and other soft tissues. While the metastasis to lungs is common, orbital involvement with cranial involvement is exceptionally rare. The case highlights the need for the early detection and aggressive treatment in order to reduce mortality in patients ES. A 23-year-old South-Asian male presented with a 2 month hist

Intravascular large B-cell lymphoma mimicking an orbital mass lesion and bilateral cavernous sinus syndrome: A case report.

Intravascular large B-cell lymphoma (IVLBCL) is a rare entity that can present with atypical extranodal manifestations, often mimicking other orbital or neurological conditions. A 69-year-old woman presented with rapidly progressive left ptosis and diplopia, which progressed to complete bilateral ophthalmoplegia within several weeks. Magnetic resonance imaging revealed a previously unreported right orbital mass accompanied by bilateral cavernous sinus (CS) enhancement, representing a novel imaging feature of IVLBCL. The diagnosis was established by sphenoid sinus biopsy. The patient achieved n

Characterization of Inferior Rectus Muscle Action in Normal Subjects Using Real-Time Magnetic Resonance Imaging of the Orbit.

Orbital floor fractures may cause long-term functional and esthetic impairments. Diplopia due to impaired function of the inferior rectus muscle is frequently an indication for surgical repair, but some cases, such as those where the diagnosis has been delayed or a previous attempt at repair has been made, may not always be amenable to surgical correction. It is advantageous for the surgeon to know whether the proper function of the inferior rectus muscle can be restored for the purposes of surgical planning and prognostication. The authors hypothesized that real-time MRI could be used to char
