Recurrent Dedifferentiated Clival Chondrosarcoma Presenting 32 Years after Initial Treatment: A Case Report.

Background Skull base chondrosarcomas are rare and locally aggressive tumors with heterogeneous biology accounting for 0.1-0.2% of all intracranial tumors. Dedifferentiated chondrosarcoma is an exceptionally uncommon and aggressive variant, seldom seen at the skull base. We report a recurrent clival chondrosarcoma with a small, dedifferentiated component, and an unusually long latency after initial treatment. Case Report A 66-year-old woman was referred with a recurrent progressive clival lesion, initially suspected to be a chordoma. She had undergone subtotal resection followed by single-frac

Diagnostic Diplopia: Isolated Oculomotor Nerve Palsy With Normal Imaging.

A 51-year-old man presented with progressive, subacute diplopia and ophthalmoplegia. Examination by the hospitalist demonstrated right ophthalmoplegia on left lateral gaze. No neurologist or neuro-ophthalmologist was available. Imaging and laboratory work were largely unremarkable. Although a microvascular etiology was suspected, the unremarkable workup expanded the differential diagnosis, highlighting the diagnostic and management challenges of cranial nerve III palsy when imaging is unremarkable in a community hospital with limited resources.

Microscope-Assisted Excision of CT-Occult Inferior Rectus Adhesions After Pediatric Orbital Floor Trapdoor Fracture Repair: A Case Report.

We report the case of a 12-year-old competitive athlete who presented with persistent ocular motility restriction and diplopia following surgical repair of an orbital floor trapdoor fracture. Although the initial surgery was performed at another institution on the day following the injury and postoperative CT confirmed the successful release of tissue entrapment, her clinical symptoms persisted (elevation restriction grade -3). Revision orbital surgery was performed 53 days after the initial injury. Pre-incision forced duction testing (FDT) confirmed mechanical tethering. Under microscopic gui

[Parinaud syndrome : a diplopia unlike any other].

Sudden-onset diplopia warrants an emergency management in ophthalmology. A binocular, vertical and persistent diplopia, should raise suspicion of a central lesion, particularly within the brainstem. We reporte the case of a 53-year-old man, who presented with acute vertical binocular diplopia. Ophthalmological examination revealed a bilateral elevation limitation, characteristic of Parinaud's syndrome. Brain magnetic resonance imaging identified a right-sided ischemic midbrain infarction, and further cardiovascular work-up demonstrated a patent foramen ovale causing a right-to-left shunt.

Infected Frontal Sinus Mucocele Mimicking a Cutaneous Abscess.

Frontal sinus mucoceles are benign expansile lesions that may progressively erode the surrounding bone and extend toward the orbit or intracranial compartment. When expansion occurs anteriorly, however, they may present as an apparently superficial forehead swelling and be mistaken for a cutaneous abscess. A 77-year-old woman with chronic sinusitis developed recurrent left paramedian frontal swelling over several months, with only transient improvement following antibiotic treatment. The lesion was initially interpreted as a cutaneous abscess and incised in an outpatient setting. Clear, nonpur

Orbital Fracture Repair Materials: Research Hotspots, Clinical Applications, and Emerging Technologies.

Orbital fractures, particularly of the medial and inferior walls, often require precise reconstruction to restore orbital volume and prevent enophthalmos, diplopia, and facial deformity. The selection of repair material, ranging from titanium meshes and porous polyethylene to resorbable polymers and tissue-engineered scaffolds, is therefore central to surgical success, yet a clinically oriented synthesis of the available options remains lacking. This study combines a bibliometric mapping with a focused review of repair materials to chart how the field has evolved and to clarify the relationshi

Epstein-Barr Virus-Positive Nonkeratinizing Squamous Cell Carcinoma (Lymphoepithelioma-Like Carcinoma) of the Medial Periorbital Area: A Case Report.

Epstein-Barr virus (EBV)-positive nonkeratinizing squamous cell carcinoma (NKSCC), formerly referred to as lymphoepithelioma or lymphoepithelioma-like carcinoma, most commonly arises in the nasopharynx. However, it has been reported in multiple extra-nasopharyngeal sites. Orbital involvement is exceedingly rare, with most reported cases occurring in the ocular adnexa. We present the case of a 67-year-old Asian male with EBV-positive NKSCC of the medial periorbital area who developed right-sided diplopia and periocular edema. Recognition of this rare entity and its diagnostic features is essent

Outcomes of endoscopic sinus surgery (ESS) on sino-orbital osteomas: ophthalmic manifestations and visual outcomes from a 12-year multicenter experience and literature review.

BACKGROUND: Osteomas are the most common benign craniofacial bone tumors that may extend into the orbital cavity. While the majority of them remain asymptomatic, orbital involvement can lead to clinically significant ophthalmologic manifestations. Although endoscopic sinus surgery (ESS) has been increasingly utilized, data focusing specifically on ophthalmologic outcomes following ESS in sino-orbital osteomas are limited. METHODS: This 12-year multicenter descriptive cohort evaluated 12 patients with symptomatic sino-orbital osteomas managed with ESS, including advanced approaches such as Dra

Stimulated single-fiber electromyography in the diagnostic workup of diplopia and ptosis.

OBJECTIVE: Ptosis and diplopia are common neuro-ophthalmological presentations with a broad differential diagnosis. In clinical practice, stimulated single-fiber electromyography (SFEMG) is often requested when ocular myasthenia gravis (MG) remains a diagnostic consideration after initial evaluation. This study evaluated the diagnostic contribution of stimulated SFEMG in a real-world cohort of patients referred with ptosis and/or diplopia. METHODS: Patients presenting with ptosis, diplopia or both were prospectively recruited and underwent comprehensive serological, radiological, and electrop

Impact of water on the fracture behavior of an advanced lithium disilicate.

OBJECTIVE: To investigate the influence of water on the monotonic and fatigue strength of a lithium disilicate glass-ceramic. METHODS: Ninety bar-shaped specimens (1.0 × 1.0 × 12.0 mm) were prepared from Advanced Lithium Disilicate (CEREC Tessera; Dentsply Sirona). Half of the specimens were stored in deionized water (W) at 37 °C for 30 days, while the other specimens stayed dry (D). A three-point bending test was carried out in a dry/wet environment (subgroup d/w) to determine monotonic strength. A stepwise fatigue test was conducted for dry-stored specimens in a dry environment (Dd) and wet
