Fulminant Idiopathic Intracranial Hypertension Presenting With Bitemporal Hemianopia Mimicking Chiasmal Pathology in Early Pregnancy: A Case Report.

Idiopathic intracranial hypertension (IIH) is characterized by elevated intracranial pressure without an identifiable cause and typically presents with headache, papilledema, and transient visual obscurations (TVOs). Fulminant IIH is a rare, rapidly progressive subtype that can lead to permanent visual loss. Although visual field defects are common in IIH, bitemporal hemianopia is an uncommon and diagnostically challenging presentation that may initially suggest chiasmal compression. A 29-year-old woman with no significant medical history presented with two weeks of progressive peripheral visu

A Case of Nutritional Optic Neuropathy Caused by Vitamin B12 and Folate Deficiency Presenting With Bitemporal Hemianopia-Like Visual Field Defects.

Nutritional optic neuropathy is a rare condition caused by vitamin B12 or folate deficiency. It typically presents with subacute bilateral vision loss and central visual field defects, whereas a presentation resembling bitemporal hemianopia is uncommon. A 70-year-old man living alone presented with gradually worsening bilateral vision loss over four months. His best-corrected visual acuity (BCVA) had decreased to 20/400 in the right eye (OD) and 20/200 in the left eye (OS). Optical coherence tomography showed widespread thinning of the ganglion cell complex, with preservation of the peripapill

May Acute Appendicitis Complicate the Management of Pseudotumor Cerebri in Shunt-Dependent Patients? A Paediatric Case Report.

INTRODUCTION: Shunt-dependent secondary pseudotumor cerebri may remain clinically stable for many years; however, intra-abdominal sepsis such as perforated appendicitis can necessitate abrupt shunt removal, eliminating cerebrospinal fluid (CSF) diversion and precipitating sudden, vision-threatening intracranial pressure (ICP) elevation. CASE PRESENTATION: A 17-year-old girl with a long-standing ventriculoperitoneal shunt for a childhood arachnoid cyst initially developed papilledema that was successfully managed with the addition of a lumboperitoneal shunt. Nine months later, she presented wi

[Migraine aura: differential diagnostics between ophthalmology and neurology].

Migraine aura is a common and diagnostically important phase of migraine. Approximately 25-35 % of individuals with migraine experience fully reversible focal neurological symptoms that typically develop gradually, spread over time, and occur in succession. Visual symptoms are the most common and include scintillating scotomas, fortification spectra, photopsias (flashes of light), and transient visual field defects. Sensory, language, motor, and brainstem symptoms may also occur. This article reviews the clinical spectrum of migraine aura based on the ICHD-3 diagnostic criteria and its charact

Assessment of Sport-Related Brain Injuries with Rapid Objective Perimetry.

Sport-related mild traumatic brain injury (mTBI) or concussion is common and has long-term implications. The lack of diagnostically accurate, rapid, and easy-to-administer tests exacerbates the problem. We evaluated the objectiveFIELD Analyser® (OFA®) for mTBI. This cross-sectional study included athletes who had sustained a concussion, and two groups of controls: the putative control group (pCG) comprising rugby players who claimed they had never had a concussion, and a non-rugby normal control group (nCG). Two OFA tests, the 8 min (OFA30) and the rapid 90 s (OFA30-12), were performed. Discri

Pituitary Apoplexy Without Chiasmal Compression Presenting With Radiographic Oculomotor Nerve Compression in a Prolactinoma: A Case Report.

Pituitary apoplexy is a rare but potentially life-threatening complication of pituitary macroadenomas that classically presents with acute headache, visual field deficits, and signs of optic chiasm compression. However, atypical neuro-ophthalmologic presentations without chiasmal involvement may delay recognition and treatment. This report highlights an unusual presentation of pituitary apoplexy associated with radiographic oculomotor nerve compression secondary to cavernous sinus extension in a patient with prolactinoma. We report the case of a 29-year-old Hispanic woman who presented to the

Optic disc melanocytoma: Predicting visual field defects and recognizing malignant transformation - a review of current evidence.

Optic disc melanocytoma (ODM) is a benign pigmented tumor historically regarded as stationary. ODM can produce visual dysfunction and rarely undergoes malignant transformation. We synthesize evidence on predicting visual field defects and recognizing malignant transformation. Optical coherence tomography angiography (OCTA) studies demonstrate that visual field defects correlate more strongly with absent intratumoral blood vessel networks than with tumor size. A validated threshold of capillary density loss > 0.75% per year identifies patients at increased risk of visual field progression. OCTA

AI to Identify Strain-Sensitive Regions of the Optic Nerve Head Linked to Functional Loss in Glaucoma.

PURPOSE: The purposes of this study were to assess whether optic nerve head (ONH) biomechanics, quantified by tissue strain, improves classification of progressive visual field (VF) loss patterns in glaucoma beyond morphology, and to use saliency maps to identify ONH regions associated with the predictions. METHODS: We recruited 249 patients with glaucoma (mean age 69 ± 5 years, 54% female patients). One eye per subject was imaged under (1) primary gaze and (2) primary gaze with IOP elevated to approximately 35 millimeters of mercury (mm Hg) via ophthalmo-dynamometry. Twelve subjects were exc

Routine Fundoscopy Uncovering Wolfram Syndrome in a Diabetic Patient: A Case Report.

Wolfram syndrome is a rare inherited neurodegenerative disorder, in which early ophthalmologic abnormalities may provide the initial diagnostic clue. In this article, we report the case of a 20-year-old male with early-onset bilateral deafness and insulin-dependent diabetes mellitus who was referred for evaluation of possible diabetic retinopathy. He reported no visual complaints at the time of presentation. Examination showed a visual acuity of 2/10 bilaterally, and optic disc pallor on fundoscopy without diabetic retinopathy. Further complementary testing revealed diffuse visual field defect

Primary Vascular Dysregulation in Normal-Tension Glaucoma: Pathophysiology and Management.

Normal-tension glaucoma (NTG) is a progressive optic neuropathy occurring in the absence of elevated intraocular pressure (IOP), implying that mechanisms beyond mechanical stress drive axonal loss. Primary vascular dysregulation (PVD), a constitutional impairment of vascular tone regulation independent of structural disease, is increasingly recognized as a central pathogenetic contributor to NTG. This review examines the evidence linking PVD to NTG, explores underlying mechanisms, and evaluates current and emerging management strategies targeting vascular dysregulation. A comprehensive narrati

Sequential Contralateral Venous Sinus Stenting for Persistent or Recurrent Symptoms Following Unilateral Stenting: A Single-Center Case Series.

BACKGROUND: Cerebral venous sinus stenting (CVSS) is an established treatment for idiopathic intracranial hypertension (IIH) and pulsatile tinnitus (PT). Despite frequent bilateral venous sinus stenosis, treatment is typically unilateral, and data on sequential contralateral stenting for persistent or recurrent symptoms remain limited. OBJECTIVE: To report safety and clinical outcomes of sequential contralateral CVSS in patients with incomplete or recurrent symptom improvement after technically successful unilateral stenting. METHODS: We conducted a retrospective single-center case series of

Complementary Visual Assessment: Validation of a High-Precision Eye-Tracking Kinetic Perimetry in the Evaluation of Visual Field Deficits in Hemianopia.

Vision is a multicomponential system where different aspects of visual processing rely on distinct neural circuits. This fundamental characteristic has profound implications for how we assess visual field defects. Classical gold standard perimetry focuses primarily on contrast detection. However, the size of the scotoma could be different when measured with kinetic perimetry since motion detection relies on different neural networks. Therefore, we developed and validated a high-precision eye-tracking kinetic perimetry as a complementary tool for evaluating visual field deficits in hemianopia.

Clinical Reasoning: A 60-Year-Old Diabetic Woman With Fluctuating Visual Disturbances.

Visual disturbances are a common presenting complaint in neurology, and the characteristics of these complaints combined with the examination findings are important in localization and then the generation of a complete differential diagnosis. Patients with diabetes, particularly in the setting of hyperglycemia, can have multiple reasons for vision changes localized throughout the visual pathways, including ophthalmic manifestations such as diabetic retinopathy, early development of nuclear sclerotic cataracts, and macular edema, as well as increased risk of ischemic optic neuropathies and retr

Altitudinal Visual Field Defect as the Presenting Sign of a Brain Tumor: A Case Report.

Brain tumors present with symptoms depending on their type, location, and stage. Most cause visual field defects respecting vertical meridians. Compressive optic neuropathy may produce altitudinal defects, mimicking ischemic optic neuropathy. Therefore, brain tumors should be included in the differential diagnosis, as early detection may improve clinical outcomes.
