Fungal keratitis caused by a dematiaceous Pleosporales member following ocular trauma with a wooden fragment: a case report.

BACKGROUND: Fungal keratitis is a vision-threatening infection. Keratitis caused by dematiaceous fungi belonging to or related to the order Pleosporales is rare and diagnostically challenging because culture growth may be slow, morphology may be nonsporulating, and species-level identification may require molecular methods. CASE PRESENTATION: A 44-year-old male forestry worker developed right ocular pain and a corneal ulcer after ocular trauma with a wooden fragment. He was referred to our hospital approximately one month after the injury; this first visit was defined as Day 0 (202X/11/27). A

A report of herpetic necrotizing keratitis following coronavirus disease 2019 infection.

Herpetic necrotizing keratitis is a severe ocular manifestation of herpes simplex virus (HSV) infection, often leading to significant visual impairment. We report a case of a 65-year-old female who presented with ocular pain and reduced acuity in the left eye, 2 weeks after a coronavirus disease 2019 (COVID-19) infection, with a relevant history of a herpetic keratitis three decades prior. Examination revealed necrotizing keratitis with stromal necrosis, dendritic epithelial defect, corneal neovascularization, and diffuse conjunctival injection. This was managed concurrently with topical and s

Impact of Chemotherapy and Radiotherapy on Iris Metastases: A Case Report.

INTRODUCTION: Iris metastases are a rare initial presentation of a systemic malignancy, managed with chemotherapy and radiotherapy in most cases. These cases provide visual documentation of iris metastasis resolution following chemotherapy and radiotherapy and illustrate potential adverse effects associated with treatment response, including intraocular pressure variations and development of iris synechiae. CASE PRESENTATION: Two cases presented to metropolitan hospitals with ocular pain, which on ophthalmic review, revealed the presence of vascularised iris lesions. One case had a history of

Ocular Causes for Undesired Behaviors in the Horse.

Numerous ocular diseases cause visual impairment and induce ocular pain that may be associated with undesired behaviors in horses. Horses with ocular pain, indicated by the presence of blepharospasm, blepharoedema, ocular discharge or rubbing the eye, should be rested from work to avoid worsening the ocular disease and to avoid undesired behaviors associated with pain or vision impairment. Horses with ocular disease are frequently head shy. Some stoic horses may not have obvious outward clinical signs despite significant visual impairment, and as such, undesired behaviors such as spooking, bal

Necrotizing scleritis resistant to biologics successfully managed with the JAK inhibitor upadacitinib: a case report.

BACKGROUND: Necrotizing scleritis is a rare but severe ocular inflammatory disorder, frequently associated with systemic autoimmune diseases such as rheumatoid arthritis (RA). Although biologic agents targeting cytokines such as TNF-α and IL-6 have improved disease outcomes, some patients remain refractory to these treatments. We describe a case of necrotizing scleritis resistant to multiple biologic agents that responded favorably to the Janus kinase (JAK) inhibitor upadacitinib. CASE PRESENTATION: A 43-year-old woman with rheumatoid arthritis presented with bilateral conjunctival injection a

Multilayered amniotic membrane transplantation for tectonic stabilization and symptomatic relief in pediatric post-traumatic corneal thinning: a case report.

BACKGROUND: Amniotic membrane transplantation (AMT) is a recognized technique in ocular surface reconstruction. Its application in pediatric patients remains less extensively documented across a range of ocular-surface indications. There is a literature gap regarding its use for rehabilitating symptomatic, post-traumatic corneal thinning with surface irregularity, where the goal shifts from visual recovery to achieving a painless, stable ocular surface amenable to prosthetic fitting. We detail the surgical strategy aimed at achieving rapid surface stabilization and highlight the considerations

Three case reports of optic perineuritis complicated by ocular scleritis and hypertrophic pachymeningitis.

Optic perineuritis (OPN) is an uncommon form of the orbital inflammatory disease targeting the optic nerve sheath, which is distinct from optic neuritis. It causes reduced visual acuity and ocular pain and is linked to infection or autoimmunity, including myelin oligodendrocyte glycoprotein-associated disease, but 30-50% are idiopathic. We report three cases of OPN with ocular scleritis and hypertrophic pachymeningitis occurring together or sequentially; in one, pachymeningitis extended from the optic sheath, in two, it was separate. Case 1: 54-year-old man with a history of ocular scleritis d

Neuropathic Corneal Pain and Blepharospasm: A Case Series.

Background and Clinical Significanc: Neuropathic corneal pain is a debilitating condition characterized by ocular pain disproportionate to clinical signs, often resulting from peripheral and central sensitization of the corneal somatosensory pathway. Emerging evidence suggests that chronic involuntary muscle contraction in blepharospasm may lead to irritation of trigeminal afferents and corneal neurogenic inflammation, potentially predisposing patients to neuropathic corneal pain. Given its debilitating nature, early recognition can prevent the progression of neuropathic sequelae. This study e

First-in-human Porcine-to-human Corneal Endothelial Xenotransplantation for Bullous Keratopathy.

BACKGROUND: Corneal blindness remains a major cause of preventable vision loss worldwide; however, the global shortage of donor corneas severely limits transplantation. Xenotransplantation using porcine corneas represents a potential solution. Here, we report the first-in-human transplantation of porcine corneal endothelial grafts. METHODS: Descemet's stripping automated endothelial keratoplasty (DSAEK) was performed using pathogen-free, nongenetically modified porcine grafts in 2 patients with painful bullous keratopathy and irreversible vision loss. One patient received topical corticostero

Ibuprofen-Induced Pemphigus Vulgaris: A Case Report.

Pemphigus Vulgaris (PV) is a chronic autoimmune blistering disease. Although pharmacological agents, particularly those containing thiol groups, have been identified as precipitating factors, reports of non-thiol drugs, such as nonsteroidal anti-inflammatory drugs (NSAIDs), causing PV remain limited. We report a case of a 41-year-old Myanmar female who developed generalized bullous eruptions, oral ulcers, and ocular pain after the concurrent use of ibuprofen and penicillin V. She presented with extensive cutaneous erosions complicated by sepsis and malnutrition. Although penicillin V was initi

Binswanger's Disease: A Diagnostic Challenge in a Middle-aged Male.

Binswanger's disease, a rare form of subcortical vascular dementia, presents with a progressive decline in cognitive and motor functions due to chronic small vessel ischemic changes in the brain. Its clinical presentation often overlaps with other neurological conditions, particularly Wernicke's encephalopathy, leading to potential diagnostic delays. We report the case of a 45-year-old male who presented with altered mental status, gait abnormalities, memory impairment and ocular pain. Given his significant history of alcohol consumption, an imperial diagnosis of Wernicke's encephalopathy was

Bilateral simultaneous acute angle closure glaucoma following anticholinergic therapy for vestibular neuritis.

Attacks of bilateral simultaneous acute angle closure glaucoma are rare. A 59-year-old man developed vertigo and vomiting, with nystagmus in primary gaze, left-beating nystagmus in left gaze and a positive head impulse test. He was diagnosed with acute vestibular neuritis and received standard therapy, using four drugs that have anticholinergic effects. He subsequently developed bilaterally red eyes, initially diagnosed as bacterial conjunctivitis, and then blurred vision with ocular pain. An ophthalmological assessment identified mid-dilated, non-reactive pupils with corneal oedema and marked

Traditional Eye Medicine in Rural Somalia: Culturally Safe Harm Reduction to Prevent Corneal Blindness.

Traditional eye medicine (TEM) remains widely practiced across rural Somalia, where distance, poverty, limited eye care workforce, and low health literacy constrain access to timely biomedical services. Commonly reported practices include instillation of aloe vera liquid or gel, expressed human breast milk, honey, and plant derived sap or rubbed tree material to relieve redness, photophobia, and ocular pain. While these practices are culturally embedded and may be perceived as effective, the ocular surface is uniquely vulnerable to toxicity, microbial contamination, and delayed presentation, f

Multidrug-resistant Acanthamoeba keratitis caused by the T4 Genotype: A case report.

This report describes the case of a 28-year-old woman, who presented with a two-week history of severe ocular pain and photophobia in her right eye. She was a soft contact lens wearer with no history of ocular trauma or systemic illness. Ophthalmic examination revealed conjunctival hyperemia, epithelial irregularities, and a developing ring-shaped stromal infiltrate. In vivo confocal microscopy demonstrated double-walled hyperreflective cysts, and culture followed by PCR and sequencing confirmed the presence of Acanthamoeba T4 genotype. Despite intensive treatment with topical polyhexamethylen

Retention of the foetal membranes (RFM) and post-partal endometritis are common problems in dairy cows. Among other things, the disease is characterized by a bacterial endometritis with aerobic as well as anaerobic bacteria. From an endocrine perspective, cows with RFM have high levels of 15-ketodihydro-PGF(2 alpha) (PG-metabolite) immediately after parturition but these levels fall rapidly within 2 weeks post-partum (early PG-metabolite elevation). After this decline, the PG-metabolite levels increase again and the levels (at this time of a lower magnitude) remain elevated during the period o
