Louse-borne relapsing fever in Ethiopia: an urgent call for WHO recognition as a neglected tropical disease.

Louse-borne relapsing fever (LBRF), caused by Borrelia recurrentis and transmitted by the human body louse, remains a persistent health emergency in Ethiopia, resulting in preventable mortality among young adults. Its continued endemicity reflects systemic failures and amplifies the syndromic effects of poverty, displacement and social stigma-locally termed 'Qmalam'-which deters care-seeking. Clinical management is further complicated by the potentially fatal Jarisch-Herxheimer reaction following antibiotic treatment. Despite these severe consequences, LBRF remains off the official WHO list of

Blood smear examination matters: an incidental case of tick-borne relapsing fever in a traveller returning from Senegal.

Relapsing fever is caused by various species of Borrelia bacteria and is traditionally categorized into louse-borne relapsing fever (LBRF) and tick-borne relapsing fever (TBRF). The epidemiological patterns differ according to the vector: LBRF is typically linked to epidemic outbreaks, while TBRF usually presents with an endemic distribution. Even though TBRF is endemic also in the Mediterranean Basin, diagnosis is mainly incidental in migrants or visiting friends and relatives travelling from sub-Saharan Africa. We report a case of TBRF with aseptic meningitis caused by Borrelia crocidurae/du

Complement inhibition by a unique cluster of immunomodulatory outer surface proteins of Borrelia recurrentis.

Borrelia recurrentis, the agent of louse-borne relapsing fever, causes a poverty-associated, infectious disease of high mortality. Here, we identified and characterized five Complement targeting and Host Interacting proteins, ChiA to ChiE displaying immunomodulatory functions. Almost all Chi homologs inhibit complement activation by direct binding of key components, block membrane attack complex formation, and interact with plasminogen. Borrelia proteins protect susceptible spirochetes from complement-mediated killing and ChiB, ChiC, and ChiD facilitate serum resistance. X-ray structures of Ch

First cluster of relapsing fever compatible with louse-borne transmission in southern Saudi Arabia: An eight-case series from Najran.

Louse-borne relapsing fever (LBRF), caused by Borrelia recurrentis, is a neglected vector-borne infection associated with overcrowding, migration, and poor hygiene conditions. We describe eight spatiotemporally linked cases of Borrelia spp. relapsing fever compatible with louse-borne transmission identified in Najran, southern Saudi Arabia. This retrospective case series included eight patients diagnosed between March 2013 and August 2013, identified during routine malaria smear examination. Seven patients were undocumented East African migrants who had recently entered through the Yemeni bord

Ancient Borrelia genomes document the evolutionary history of louse-borne relapsing fever.

Several bacterial pathogens have transitioned from tick-borne to louse-borne transmission, which often involves genome reduction and increasing virulence. However, the timing of such transitions remains unclear. We sequenced four ancient Borrelia recurrentis genomes, the agent of louse-borne relapsing fever, dating from 2300 to 600 years ago. We estimated the divergence from its closest tick-borne relative to 6000 to 4000 years ago, which suggests an emergence coinciding with human lifestyle changes such as the advent of wool-based textiles. Pan-genome analysis indicated that much of the evolu

Unusual presentation of complicated relapsing fever with spontaneous hemoperitoneum mimicking surgical acute abdomen: a case report.

BACKGROUND: Louse-borne relapsing fever (LBRF) is a prevalent disease in Ethiopia, affecting malnourished and impoverished populations. Historically fatal, mortality has decreased to less than 5% with antibiotics. Symptoms include high fever, rigors, myalgia, hepatosplenomegaly, jaundice, and petechial rash. Diagnosis is challenging due to Giemsa-stained blood films and PCR in resource-limited settings. CASE PRESENTATION: A 29-year-old Ethiopian patient experienced abdominal pain, vomiting, diarrhea, and high fever for 3 days. Physical examination revealed low blood pressure, oxygen saturation

Prevalence and Susceptibility Status of Body Louse (Pediculus humanus humanus) (Anoplura: Pediculidae) to Deltamethrin in Urmia City, Iran (2024).

BACKGROUND: The body louse spreads diseases such as epidemic typhus and louse-borne relapsing fever and has shown resistance to various insecticides. While deltamethrin is used to treat infestations in some countries, research on its effectiveness against body lice is limited. This study assessed the susceptibility of body lice to deltamethrin using a bioassay. METHODS: Body lice specimens were collected from an addiction treatment camp in Urmia City, West Azerbaijan Province, Iran. They were treated in 5 cm Petri dishes lined with 1 ml of various concentrations of deltamethrin (0.01, 0.02, 0

Serological evidence of louse-borne relapsing fever in northern Kenya.

BACKGROUND: Tick- and louse-borne relapsing fever are highly-neglected, vector-borne diseases caused by diverse Borrelia species. Presently, there are no data available on the endemicity of tick- and louse-borne relapsing fever spirochetes in Kenya. Here, we present data of a retrospective study on the seroprevalence of louse-borne relapsing fever (LBRF) in northern Kenya. METHODS: A novel immunoassay, recently established for the diagnosis of LBRF was utilized to screen 2005 blood samples collected from individuals with fever without a source in Turkana County, Kenya between May 2009 and Nov

Comparative Analysis of Tick-Borne Relapsing Fever Spirochaetes from Ethiopia and Nigeria.

Despite increasing reports of tick-borne diseases in Africa, remarkably, reports of tick-borne relapsing fever (TBRF) in Nigeria are lacking. Ornithodoros savignyi from Nigeria have been reported with the relapsing fever Candidatus Borrelia kalaharica. Conversely, in Ethiopia, the agent of relapsing fever is the louse-borne relapsing fever (LBRF) spirochaete Borrelia recurrentis with no TBRF reported to occur. A total of 389 Ornithodoros ticks, Ethiopia (N = 312) and Nigeria (N = 77), were sampled, together with 350 cattle, and 200 goat sera were collected from Nigeria. Samples were screened f

[Progress in research of epidemiology of relapsing fever and prevention and control measures].

Relapsing fever, caused by Borreliae of the relapsing fever groups, is an infectious disease, which would cause spirochaetaemia and repeated fever in human. To comprehensively understand the classification and distribution of relapsing fever, as well as correlated factors, this paper summarizes the progress in research of epidemiology of relapsing fever in the world, and suggests prevention and control measures. The disease is heterogenous and can be divided into three groups according to vectors, i.e. tick-borne relapsing fever, louse-borne relapsing fever and the avian relapsing fever. Tick

Travel- and migration-associated epidermal parasitic skin diseases. A review.

Epidermal parasitic skin diseases are a family of parasitic diseases which occur globally or are frequent in special settings. Epidermal parasitic skin diseases occur in travelers and migrants, although epidemiology and/or clinical manifestations differ between these groups. The objective of this study is to summarize the existing knowledge concerning tungiasis, hookworm-related cutaneous larva migrans, scabies, pediculosis capitis and pediculosis corporis in the context of travel and migration, and to identify risk factors for infection. The PUBMED database was searched using the terms tungia

Emerging bacterial infectious diseases/pathogens vectored by human lice.

Human lice have always been a major public health concern due to their vector capacity for louse-borne infectious diseases, like trench fever, louse-borne relapsing fever, and epidemic fever, which are caused by Bartonella quintana, Borrelia recurrentis, and Rickettsia prowazekii, respectively. Those diseases are currently re-emerging in the regions of poor hygiene, social poverty, or wars with life-threatening consequences. These louse-borne diseases have also caused outbreaks among populations in jails and refugee camps. In addition, antibodies and DNAs to those pathogens have been steadily

[Cardiac implantable electronic device infections; risk factors, prevention and treatment].

The infection rate associated with cardiac implantable electronic devices has increased over the past decades. A recent study found the prevalence of infection after cardiac resynchronization therapy to be more than tenfold higher than reported to the national registry. Risk factors for infection can be host-, procedure-, or device-related, the most important being recent manipulation of the device. Perioperative contamination is the most frequent source, but leads can also be secondarily infected from bacteremia, most commonly with staphylococci. Patients with pocket infection often present w

Tick borne relapsing fever - a systematic review and analysis of the literature.

Tick borne relapsing fever (TBRF) is a zoonosis caused by various Borrelia species transmitted to humans by both soft-bodied and (more recently recognized) hard-bodied ticks. In recent years, molecular diagnostic techniques have allowed to extend our knowledge on the global epidemiological picture of this neglected disease. Nevertheless, due to the patchy occurrence of the disease and the lack of large clinical studies, the knowledge on several clinical aspects of the disease remains limited. In order to shed light on some of these aspects, we have systematically reviewed the literature on TBR

A Comparative Study of Body Lice and Bed Bugs Reveals Factors Potentially Involved in Differential Vector Competence for the Relapsing Fever Spirochete Borrelia recurrentis.

Borrelia recurrentis is the causative agent of louse-borne relapsing fever and the only Borrelia species transmitted by an insect rather than a tick vector. While bed bugs (Cimex lectularius L.) are not established vectors of any human pathogens, a recent study reported that they may be competent vectors of B. recurrentis. However, many aspects of infection and transmission remain unclear in this possible secondary vector. Here, we carried out several quantitative laboratory studies to gain a better understanding of the host suitability of bed bugs relative to the established body louse vector

Historical overview and update on relapsing fever group Borrelia in Latin America.

Relapsing fever group Borrelia (RFGB) are motile spirochetes transmitted to mammalian or avian hosts through the bite of hematophagous arthropods, such as soft ticks (Argasidae), hard ticks (Ixodidae) and the human clothing lice. RFGB can infect pets such as dogs and cats, as well as birds, cattle and humans. Borrelia recurrentis, B. anserina and B. theileri are considered to have worldwide distribution, affecting humans, domestic birds and ruminants, respectively. Borrelia spp. associated with soft ticks are transmitted mainly by Ornithodoros ticks and thrive in endemic foci in tropical and s
