Transcriptomic response to Borrelia afzelii infection in the skin of wild bank voles.

Bank voles are one of the main reservoirs of tick-transmitted spirochete Borrelia afzelii, a causative agent of Lyme disease in humans in Europe. How the immune system deals with infection at the site of entry, that is, the skin, has not been explored in this species. Here, we used RNA sequencing to explore the transcriptomic response in the ear skin of wild bank voles infected with B. afzelii. We identified 54 differentially expressed genes, of which 37 showed upregulation, and 17 showed downregulation in infected voles compared to uninfected ones. Weighted gene co-expression network analysis

Lyme Carditis Identified by a Novel Syndromic Surveillance Definition, New York State, 2017-2021.

OBJECTIVES: Lyme carditis is a rare, potentially fatal manifestation of Lyme disease. Although Lyme disease is nationally notifiable, data on clinical manifestations are not collected systematically in the United States. We developed a syndromic surveillance definition to identify patients with Lyme carditis in New York State during 2017-2021. METHODS: We developed a definition of Lyme carditis by using diagnostic codes and keywords to identify emergency department visits related to Lyme carditis through the National Syndromic Surveillance Program's BioSense Platform. We systematically abstra

Ectoparasite- and Vector-Borne-Related Dermatoses: A Single-Centre Study with Practical Diagnostic and Management Insights in a One Health Perspective.

Background: Parasitic skin-related conditions represent a frequent and evolving challenge in human dermatology, as they often mimic other dermatoses, and are increasingly complicated by therapeutic resistance. With this study, we aimed to provide a practical, clinician-oriented overview of our experience, contextualising it within the current literature. Materials and Methods: We conducted a retrospective, single-centre observational study, reporting a case series of 88 patients diagnosed with parasitic or arthropod-related skin infestations at the San Raffaele Hospital Dermatology Unit (Milan

A lethal combination: fulminant liver failure due to Babesia microti and Borrelia burgdorferi co-infection.

Babesiosis and Lyme disease are tick-borne infections endemic to the northeastern and upper Midwestern United States. Although both diseases can involve multiple organ systems, acute liver failure (ALF) is exceedingly uncommon. Co-infection with Babesia microti and Borrelia burgdorferi may exacerbate disease severity, create diagnostic uncertainty, and delay appropriate therapy. We describe a 77-year-old man with type II diabetes mellitus, stage IV chronic kidney disease, and heart failure with preserved ejection fraction who presented with dyspnea, cough, and peripheral edema. Initial evaluat

The Erosion of a Pillar of Public Health: ACIP's Role and the Future of US Vaccine Policy Post-June 2025.

Since June 2025, the restructuring of the US Advisory Committee on Immunization Practices (ACIP)-including the abrupt replacement of all voting members and leadership-marked a significant departure from its longstanding norms of continuity and governance. Subsequent deviations from evidence-based policymaking raise concerns about the independence, transparency, and scientific rigor of vaccine policies under the new Department of Health and Human Services administration. The politicization of deliberations, selective use of questionable evidence, and disregard for established methodological sta

Automated Enrichment of DNA Biomarkers from Large-Volume Samples: Detection of B. burgdorferi Cell-Free DNA in Urine.

Liquid biopsies enable rapid and noninvasive analysis for disease biomarkers. However, in the early stages of a disease, biomarkers are often present in extremely dilute concentrations, making them difficult to isolate and quantify. We have developed an Automated Magnetically and Thermally Induced Conical Separator (Auto-MATICS), which enables purification and enrichment of trace biomarkers from large volume biological samples (up to 15 mL) using magnetic beads and thermally responsive alkane partitions (TRAPs). Here, we quantify the enrichment capabilities of the Auto-MATICS and demonstrate a

Serological evidence of tick-borne encephalitis virus in limousin, france.

BACKGROUND: Ticks transmit multiple pathogens, including B. burgdorferi, the agent of Lyme disease, and tick-borne encephalitis virus (TBEV), which can cause severe neurological disease and death despite frequent pauci- or asymptomatic infections. The Limousin region is considered at risk for TBEV because of dense tick (Ixodes) populations, although no clinical TBE cases have yet been reported there. METHODS: This study assessed TBEV seroprevalence in 880 patients tested for Lyme disease at Limoges University Hospital between January 2020 and January 2023. Presence of neutralizing antibodies

Retrospective Case Series of Ocular Lyme Disease, 1988-2025.

Reports of ocular manifestations of Lyme disease (LD) are uncommon, and signs and symptoms may be overlooked by physicians. We conducted a retrospective case series of ocular LD reported during 1988–2025. Among 27 published reports in PubMed, we noted that, in 38 cases, the most common ocular manifestation was uveitis, representing 45% of cases, followed by optic neuritis and cranial nerve palsies (including trochlear and abducens). Not all cases met Centers for Disease Control and Prevention surveillance guidelines for LD, given that some case reports were published before the current guideli

Protective properties of a candidate C-terminal domain OspA vaccine for prevention of Lyme disease.

Lyme disease is a growing public health concern that is geographically focused in regions where ticks that carry the causative bacteria, Borrelia burgdorferi sensu lato (s.l.), are endemic. Outer surface protein A (OspA) is expressed by B. burgdorferi s.l. spirochetes during the tick phase and OspA antibodies introduced during tick feeding can block transmission and prevent B. burgdorferi infection. Candidate Lyme disease vaccine VLA15 is comprised of the C-terminal domains of the six B. burgdorferi s.l. OspA serotypes (ST) prevalent in North America and Europe. We report herein that non-human

Interacting effects of mean temperature and temperature variability on Ixodes scapularis (Acari: Ixodidae) overwinter survival and energy use.

Within recent decades, the northeastern United States has experienced milder winters, supporting an increase in blacklegged tick (Ixodes scapularis) densities and concomitant increase in reported cases of tick-borne disease. While numerous laboratory and field studies have investigated impacts of cold temperatures on tick overwinter survival, temperature variability also may contribute to tick longevity and remains largely underexplored. In this study, we conducted three experiments. First, we carried out a laboratory experiment in which blacklegged tick nymphs were exposed to different low te

Geographic variation in risk of blacklegged tick-borne coinfections in the eastern United States.

Blacklegged tick (Ixodes scapularis)-associated diseases are increasing in incidence and geographic range in the United States. Pathogen range expansion may increase the risk of coinfections, which could complicate accurate diagnosis and treatment of tick-borne diseases. To identify regions of coinfection risk across the eastern United States, we used a national database of tick-borne pathogen occurrence to identify areas of co-occurrence between four I. scapularis-borne human pathogens: Anaplasma phagocytophilum human-active variant (Apha; anaplasmosis), Babesia microti (Bam; babesiosis), Bor

A Lyme Neuroborreliosis Case Presenting With Ophthalmoplegia, Dysarthria, and Spastic Quadriparesis.

Lyme neuroborreliosis (LNB) represents the neurological manifestation of Lyme disease and is the most common form of disseminated Lyme infection in Europe. Central nervous system involvement in LNB includes cranial neuritis, meningitis, radiculoneuritis, encephalitis, and encephalomyelitis with quadriparesis. We report a patient presenting with complex ophthalmoplegia, speech impairment, and spastic quadriparesis. The neurological weakness initially followed an ascending pattern suggestive of Guillain-Barré syndrome (GBS). Cerebrospinal fluid (CSF) analysis revealed lymphocytic pleocytosis, an

Distinct Clinico-pathogenic Subgroups in Pediatric Lyme Neuroborreliosis.

BACKGROUND: Lyme neuroborreliosis (LNB) is a common manifestation of Lyme disease in children. It is caused by the bacterium Borrelia burgdorferi and can affect both the peripheral nervous system (PNS) and the central nervous system (CNS). This study aimed to describe clinical and immunological features of LNB in children. METHODS: We performed a large retrospective cohort study of children diagnosed with LNB at the University Children's Hospital Zurich from 1 January 2006 to 31 December 2020. RESULTS: A total of 190 children diagnosed with LNB were included (median age, 7.6 years). Meningit

Streamlining Borrelia burgdorferi cultivation using quantitative PCR screening.

Introduction. Direct detection of Borrelia burgdorferi by culture is considered the gold standard for confirming Lyme disease (LD). However, B. burgdorferi culture is not routinely used in clinical practice or research due to its lengthy protocol and low success rate. This study aimed to streamline the process by integrating a specific quantitative PCR (qPCR) screening early into the B. burgdorferi culture workflow for identification of cultures that are likely to yield viable spirochetes.Methods. Thirty-two blood plasma and 11 cerebrospinal fluid (CSF) samples were collected from 32 children

Telomere-to-telomere assembly detects genomic diversity in Canadian strains of Borrelia burgdorferi.

Borrelia burgdorferi, the bacteria causing Lyme disease, has a complex genome comprising a linear chromosome and multiple linear and circular plasmids. The atypical hairpin telomeres and the highly paralogous plasmids complicate genome assembly. We develop a genome assembly pipeline using both long and short reads to overcome these challenges. Using long reads, we assemble the hairpin telomeres of the linear replicons, an lp28-1a plasmid subtype, and circular plasmids of nine B. burgdorferi strains from five regions across Northwest Ontario and Manitoba, Canada. Although similar across the cor

Sex and menopause-based differences in presentation of early Lyme disease: A prospective cohort study.

Although prior research has established sex and menopausal status-based differences in immune response, susceptibility, and severity to a variety of pathogens, their relevance in early Lyme disease is understudied. We examined the clinical and serologic presentation of patients with early Lyme disease, stratified first by sex then by menopausal status. We also explored the hypothesis that males would present with more severe early Lyme disease. In this prospective cohort study from the Mid-Atlantic US, 243 adult, antibiotic-naïve patients were enrolled with a diagnostic erythema migrans rash p

Chronic Tonsillitis as a Focal Infection: A Decade-Long Case Involving Severe Systemic Symptoms.

Unlike acute tonsillitis, which is readily recognized as infectious, chronic tonsillitis, tonsilloliths, and tonsillar detritus are often considered non-infectious and benign, despite their potential to act as focal infections causing systemic inflammatory symptoms that are frequently overlooked when tonsillar compression is not performed. We report the case of a 29-year-old female with a decade-long history of progressive musculoskeletal pain, episodic low-grade fever, headaches, and exercise-induced inflammatory arthralgia affecting the feet, ankles, wrists, and spine. Structure and ligand specificity of Borrelia burgdorferi BmpA.

BmpA is a putative substrate-binding protein from Borrelia burgdorferi , the causative agent of Lyme disease, an organism with limited metabolic capacity that relies on salvage pathways rather than de novo nucleotide biosynthesis. Here, we determine the crystal structure of BmpA to a resolution of 2.6 Å, revealing a conserved substrate-binding protein fold with a deeply buried nucleoside-binding pocket. Using microscale thermophoresis, we show that BmpA binds thymidine with high affinity followed by cytidine and adenosine, whereas binding to ribose, guanosine, inosine, and uridine was not dete

Borrelia mayonii induces carditis but not arthritis in Lyme-susceptible mice.

BACKGROUND: Borrelia mayonii, a Borrelia burgdorferi sensu lato (Bbsl) genospecies, is a cause of Lyme disease in the upper midwestern United States. B. mayonii infection can present with some atypical clinical manifestations, including unusually high spirochetemia. Previous studies have demonstrated the infectivity of B. mayonii in laboratory mice and found evidence of widespread dissemination to internal tissues, however, did not report evidence of high spirochetemia. In the current study, we sought to directly compare spirochetemia as well as typical Lyme disease pathology including arthrit
