Weight Loss Velocity and Patulous Eustachian Tube Chronicity After Bariatric Surgery.

OBJECTIVES: To evaluate the relationship between weight loss velocity following bariatric surgery and the onset and long-term persistence of Patulous Eustachian Tube (PET) symptoms. METHODS: Sixty patients who underwent laparoscopic sleeve gastrectomy were prospectively evaluated. Otological assessments and the Eustachian Tube Dysfunction Questionnaire (ETDQ-7) were completed preoperatively and postoperatively at 1, 3, 6, and 12 months. Patients were categorized into Asymptomatic (Group 1), Recovered (Group 2), and Persistent (Group 3) groups based on 12-month clinical trajectories. Postopera

History-anchored interpretation of discordant JK-05A outputs in Eustachian tube function testing.

PURPOSE: The JK-05A platform performs up to four Eustachian tube (ET) subtests in a single session; however, the outputs often disagree. We evaluated discordant JK-05A outputs in a single-center cohort and developed a practical framework for their interpretation. MATERIALS AND METHODS: We retrospectively analyzed 51 patients who underwent JK-05A testing between September 2025 and April 2026 using explicit reading rules. Inter-test concordance, sonotubometric threshold provenance, and the Japan Otological Society (JOS) criteria informed a history-anchored, functional-domain approach. Patulous

Complications of Otitis Media: Mastoiditis and an Intracranial Abscess Presenting as Behavioral Changes.

An intracranial abscess is a rare but life-threatening complication of otitis media (OM) and mastoiditis that requires rapid diagnosis and intervention. We present the case of a 43-year-old woman who presented with acute altered mental status and aggressive behavior following recent treatment for OM. Physical examination revealed a perforated, purulent right tympanic membrane, and neuroimaging confirmed bilateral mastoiditis and a right temporal lobe brain abscess with midline shift. Following treatment with broad-spectrum intravenous antibiotics, the patient underwent a successful craniotomy

Patulous Eustachian tube dysfunction following rapid weight loss associated with semaglutide use: A case report.

Patulous Eustachian tube dysfunction (PETD) is an uncommon condition characterized by an abnormally patent Eustachian tube, leading to symptoms such as aural fullness and autophony. Rapid weight loss is a recognized risk factor due to loss of peritubal adipose support. With the increasing use of semaglutide and other glucagon-like peptide-1 receptor agonists for obesity management, substantial and rapid weight reduction has become more common, raising the potential for underrecognized otologic complications. A 79-year-old male presented with intermittent bilateral aural fullness and ear "poppi

Diagnostic Value of Tubomanometry in Patulous Eustachian Tube.

BACKGROUND: A patulous Eustachian tube (PET) may be challenging to diagnose in cases where objective tympanic membrane movement is absent during physical examination. OBJECTIVE: The primary aim was to assess the diagnostic usefulness of tubomanometry (TMM) in patients with PET. A secondary aim was to evaluate nasopharyngeal pressure curves as indicators of nasopharyngeal muscle function, given the proposed contribution of muscular dystrophy in PET pathophysiology. METHODS: A retrospective study was conducted at a tertiary referral center on adult patients diagnosed with PET who had undergone

Beyond the Tradition in Tympanoplasty: A Retrospective Cohort Study of a New Modified Palisade Tympanoplasty Technique.

BACKGROUND & AIM: Tympanic membrane perforation is a common cause of conductive hearing loss and otologic morbidities. The most common cause is trauma, which tends to heal spontaneously. Conversely, perforations caused by infections or middle ear pathology require surgical intervention, such as tympanoplasty. While cartilage tympanoplasty provides benefits of durability and resistance to retraction, concerns persist regarding graft rigidity and its adverse effects on hearing outcomes. This study was implemented to assess the anatomical and functional success of an innovative and novel form of

Bariatric Surgery as a Potential Risk Factor for Patulous Eustachian Tube: A Comprehensive Literature Review.

Patulous Eustachian tube (PET) is a condition characterized by abnormally persistent patency of the Eustachian tube, resulting in symptoms such as autophony, aural fullness, and otalgia, with weight loss increasingly reported as a potential risk factor. This review aimed to evaluate the association between bariatric surgery and PET. PubMed, Google Scholar, and Scopus were searched from inception to August 2026 for studies addressing PET, bariatric surgery, weight loss, and Eustachian tube dysfunction. Available evidence, predominantly from small observational studies and case reports, demonstr

Transcanal endoscopic excision of early-stage middle ear paragangliomas.

OBJECTIVE: To describe patient-level hearing and symptom outcomes after exclusive transcanal endoscopic ear surgery (TEES) for early-stage middle ear paragangliomas. METHODS: This retrospective case series included nine consecutive patients with Glasscock-Jackson stage I-II tumors treated between January 2019 and September 2023. Air-conduction (AC) and bone-conduction (BC) pure-tone averages and air-bone gaps (ABGs) were assessed preoperatively and at 6 months. A decrease of at least 10 dB in AC pure-tone average defined hearing improvement. RESULTS: Mean AC pure-tone average decreased from

Sedation-Assisted, Non-sedated, and Escalated Removal of Pediatric ENT Foreign Bodies in Emergency Settings: A Systematic Review.

Pediatric ear, nose, and throat (ENT) foreign bodies are among the most common procedural challenges in emergency medicine. Substantial uncertainty exists regarding the optimal approach to removal, including sedation-assisted versus non-sedated techniques, and the appropriate thresholds for escalation to otolaryngology or the operating room. A systematic literature search was conducted in PubMed, the Cochrane Library, Web of Science, and Scopus (search date: June 2026). Studies were included if they reported outcomes of ENT foreign body removal in pediatric or predominantly pediatric emergency

Systematic Review: Pediatric Patulous Eustachian Tube Dysfunction-An Underestimated Entity in Otolaryngology.

BACKGROUND: Pediatric patulous Eustachian tube dysfunction (PETD) is uncommon in daily practice and is often overshadowed by more prevalent middle ear disorders. Because symptoms may be difficult for children to describe and objective confirmation is inconsistently used, the true frequency and clinical burden of PETD in childhood remain uncertain. This systematic review synthesized the literature on pediatric PETD to clarify reported prevalence, clinical presentation, diagnostic approaches, associated factors, and treatment strategies. METHODS: Following Preferred Reporting Items for Systemat

OTOLens: Development and validation of a mobile application for assessment of tympanic membrane and middle ear pathology.

BACKGROUND/OBJECTIVE: Diagnosing tympanic membrane and middle ear pathology in primary care is challenging due to anatomical complexity, overlapping presentations, and limited access to specialized equipment. We developed and externally validated OTOLens, a smartphone-based mobile application classifying otoscopic images using deep learning. METHODS: In this prospective development and validation study, OTOLens combined three ResNet-based image classifiers (OME, TMP, and MYR) with a parallel gradient-boosted tabular classifier operating on PCA-reduced image features, integrated by a per-condi

Further Study of Craniotomy in Suboccipital Retrosigmoid Keyhole Approach.

BACKGROUND: The suboccipital retrosigmoid approach represents a classic and mainstream surgical corridor for the management of cerebellopontine angle lesions. Our team has previously proposed a novel, precise keypoint localization strategy termed the "one point, two lines, two distances" theory. Nevertheless, the standardized procedural techniques for craniotomy based on this localization system remain undefined and unspecified. OBJECTIVE: On the basis of the aforementioned landmark localization system, this study aimed to elaborate on the standardized surgical techniques for retrosigmoid cra
