Audiologic findings in Saethre-Chotzen syndrome.

BACKGROUND: Hearing loss has been described in Apert syndrome but is poorly documented in other craniosynostosis disorders. METHODS: The authors retrospectively reviewed the audiologic and otologic records of patients with Saethre-Chotzen syndrome to define the incidence, type, and extent of hearing loss. Only patients with documented audiologic examinations were included. Hearing loss was categorized by American Speech-Language-Hearing Association guidelines (i.e., mild, 26 to 40 dB; moderate, 41 to 55 dB; moderate/severe, 56 to 70 dB; severe, 71 to 90 dB; and profound, >90 dB). RESULTS: Tw

Improvement in autophony symptoms after superior canal dehiscence repair.

OBJECTIVE: Autophony, or the unusually loud or disturbing sound of a patient's own voice, can be a prominent and disabling symptom of superior canal dehiscence (SCD) syndrome. The current study measures autophony symptoms before and after SCD plugging to quantify the benefits of surgery. STUDY DESIGN: Patients undergoing SCD plugging between September 2007 and October 2008 completed a questionnaire before and 3 months after surgery. The questionnaire consisted of 26 statements to assess the disability caused by the sound of the patient's own voice. Patients graded each item on a scale from 0

Roaring in the ears: patulous Eustachian tube in bulbar amyotrophic lateral sclerosis.

We report a 64-year-old female with predominantly bulbar amyotrophic lateral sclerosis who experienced 'roaring' in her ears. She was diagnosed with patulous Eustachian tube, which is presumed to occur secondary to bulbar weakness. The roaring adversely affected the management of her ALS by impairing her ability to tolerate BiPaP. A review of the relevant literature is reported.

Autologous cartilage injection for the patulous eustachian tube.

The eustachian tube (ET) is normally closed, but it opens temporarily during swallowing. Patients with a patulous ET (PET) have various aural symptoms such as aural fullness, autophony, and hearing their own breathing. These symptoms are caused by abnormal transmission of sound from the pharynx to the middle ear via an open ET with little attenuation. We introduce a novel injection technique for the treatment of PET using autologous cartilage. This procedure is minimally invasive and has been successfully used to treat PET in 2 patients.

Simple mass loading of the tympanic membrane to alleviate symptoms of patulous eustachian tube.

BACKGROUND: Patulous eustachian tube (PET) has a major impact on a patient's quality of life. The purpose of this study was to understand mechanisms behind the symptoms, develop treatments based on these, and develop and use a questionnaire to measure changes in PET symptoms with a novel intervention. Our hypothesis is that PET symptoms can be addressed at the level of the eardrum more easily than at the level of the eustachian tube. METHODS: In a population of 14 PET subjects and 6 fresh temporal bones, several investigations were performed. Nasal audiometry was used to measure frequencies p

Patulous eustachian tube in bariatric surgery patients.

OBJECTIVE: Patulous eustachian tube (PET) is associated with several factors, including weight loss. Rapid and significant weight loss in bariatric surgery patients may be risk factors for developing PET. The aim of this study is to provide information about the prevalence of PET after bariatric surgery and its association to weight loss. STUDY DESIGN: Case series with planned data collection. SETTING: Tertiary center. SUBJECTS AND METHODS: We recruited 163 patients who had undergone bariatric surgery at the Clinical Hospital of the University of Chile between 2006 and 2009. One hundred for

Use of calcium hydroxylapatite for management of recalcitrant otorrhea due to a patulous eustachian tube.

We describe a medically complex 4-year-old child with a history of chronic unilateral myringotomy tube otorrhea and laryngopharyngeal reflux who was diagnosed with a patulous eustachian tube. The diagnosis was confirmed by nasopharyngoscopy, retrieval of food from the affected ear after oral challenges with green dye, and computed tomography. The child was effectively treated with an endoscopic transoral injection of calcium hydroxylapatite (Radiesse Voice) in the lateral pharyngeal wall and torus tubarius. She initially received a test injection with a temporary gel of synthetic polymers (Rad

Masked patulous Eustachian tube: an important diagnostic precaution before middle ear surgery.

The Eustachian tube is normally closed, but it opens upon swallowing for only less than one second to equalize the middle ear pressure with the atmospheric pressure, and immediately closes again. Patients with patulous Eustachian tube (PET) suffer from annoying symptoms, such as aural fullness (sensation of fullness in the ear), autophonia (abnormally loud audition of own voice), audition of breathing sound, and fluctuating sensation of the tympanic membrane upon respiration. The diagnosis of PET is not difficult when patients complain of such typical symptoms. However, there is an unexpected

Three-dimensional computed tomography imaging of the eustachian tube lumen in patients with patulous eustachian tube.

BACKGROUND: The eustachian tube (ET) has a complicated anatomy, which is related to middle ear pathology. The anatomy of the ET has been well investigated in cadavers, but may not accurately reflect that in living subjects. Three-dimensional (3D) computed tomography (CT) was used to examine the ET in patients with patulous ET. METHOD: The subjects were 35 patients (17 males, aged 59 +/- 16 years, and 18 females, aged 44 +/- 15 years) with patulous ET which could be continuously traced from the pharyngeal orifice to the tympanic orifice (53 ears). CT was performed in the sitting position at re

The effects of mastoid aeration on autophony in patients with patulous eustachian tube.

The effect of the mastoid pneumatization on sound transmission from the nasopharyngeal cavity to the inner ear, which is one of the important factors to indicate the severity of the patulous eustachian tube (ET), are investigated in patients with patulous ET. Twenty ears of 13 subjects with patulous ET were retrospectively analyzed. Sound transmission from the nasopharyngeal cavity to the inner ear was evaluated by observing the masking effects of noise presented in the nasopharyngeal cavity on the auditory threshold (nasal noise masking audiometry). The relationship between this masking effec

Patulous Eustachian tube associated with hemodialysis.

The objective of the present study is to demonstrate that hemodialysis (HD) is a possible causal factor for patulous Eustachian tube (ET). We present a representative case report and results of a mass examination in the HD unit. The present patient (a 55-year-old woman) reported aural fullness and autophony were often experienced after HD. The findings of eardrum and ET function tests revealed that her patulous ET condition was observed soon after HD and then disappeared until the next HD. A survey in the HD unit was carried out to identify patients who noticed patulous symptoms after HD. Pati

Patulous Eustachian tube in spontaneous intracranial hypotension syndrome.

A 48-year-old slender woman presented with a rare case of spontaneous intracranial hypotension syndrome manifesting as patulous Eustachian tube. The patient complained of sudden onset of ear fullness and nasal voice as well as typical orthostatic headache. Patulous Eustachian tube was identified by observation of synchronous movement of the tympanic membrane with respiration and swallowing. The diagnosis of spontaneous intracranial hypotension was confirmed by negative cerebrospinal fluid pressure, and typical magnetic resonance imaging and radioisotope cisternography findings. All symptoms co
