Cholesterol granuloma is a rare, benign lesion of the mastoid air cells, often associated with glue ear and typically presenting unilaterally. Histopathologically, it comprises of granulation tissue with cholesterol crystals, multinucleated giant cells, and hemosiderin deposits from erythrocyte breakdown. While commonly found in the petrous apex, tympanomastoid cholesterol granulomas are rare, with very few cases reported in literature. It can mimic other otological conditions, leading to diagnostic challenges. We report a rare case of cholesterol granuloma presenting as acute mastoiditis in a

INTRODUCTION: Acute mastoiditis, a complication of acute otitis media (AOM), remains a concern in children despite vaccines and antibiotics. Serious complications like cerebral venous sinus thrombosis (CVST) can be life-threatening.
CASE PRESENTATION: A 4-year-old girl with fever, vomiting, and ear pain worsened despite amoxicillin treatment, developing headache, neck stiffness, and drowsiness. An ENT examination showed thickened tympanic membranes. Neurological symptoms, including headache, drowsiness, and neck stiffness, prompted lumbar puncture and initiation of ceftriaxone, with elevated

UNLABELLED: Acute mastoiditis (AM) is the most common intratemporal complication of acute otitis media in children. This multicenter study aimed to describe the epidemiological, clinical, microbiological, and radiological features of pediatric AM in four Italian regions and to identify factors associated with radiologic complications. This retrospective, observational cohort study included all patients under 18 years hospitalized with AM in the pediatric departments of Bari, Forlì, Macerata, and Pescara between January 2022 and May 2025. Demographic, clinical, microbiological, imaging, and man

Background: Ongoing healthcare and medical education reforms in Kazakhstan have been accompanied by persistent workforce shortages and reduced inpatient capacity in pediatric care. Therefore, this study aimed to assess and forecast selected healthcare system indicators using acute mastoiditis (AM) as a sentinel condition while also describing its clinical and epidemiological characteristics. Materials and Methods: This study combined an analysis of national healthcare and demographic statistics in Kazakhstan from 1998 to 2024 with a retrospective review of pediatric AM patients treated at a te

AIM: The aim of this report is to present a rare case of acute mastoiditis caused by Klebsiella (K.) pneumoniae carbapenemase (KPC) producing bacteria.
METHODS: The following report describes a 78-year-old immunosuppressed female who was hospitalized due to left-sided acute otitis media complicated by tympanic membrane rupture and acute mastoiditis. Fluid-discharge culture revealed KPC. The patient successfully received four weeks of conservative treatment with intravenous colistin (based on antibiotic susceptibilities).
DISCUSSION: Acute mastoiditis is a rare complication of acute otitis me

OBJECTIVES: Otitic hydrocephalus is defined as increased intracranial pressure secondary to acute mastoiditis with cerebral venous thrombosis, leading to visual morbidity from papilledema. As no standardized ophthalmological treatment for papilledema exists, this study aims to evaluate the impact of corticosteroids on visual and clinical outcomes.
METHODS: This retrospective chart review includes children ≤18 years old, with otitic hydrocephalus and papilledema. Patients were evaluated by ophthalmology between July 2022 and July 2024 at a quaternary children's hospital. Data from ophthalmolog

Otogenic infections remain a potentially under-recognized cause of pneumococcal meningoencephalitis in adults. In elderly patients, acute mastoiditis may present without overt otological findings ("masked mastoiditis"), delaying diagnosis and increasing the risk of intracranial complications. An 86-year-old unvaccinated male patient with multiple comorbidities initially presented to the emergency department with non-specific upper respiratory symptoms and normal otoscopy and was discharged with presumed viral infection. Four days later, he returned with acute neurological deterioration and hem

OBJECTIVE: Acute mastoiditis (AM) is a serious complication of acute otitis media. While historically managed with mastoidectomy, recent trends support conservative treatment, including intravenous (IV) antibiotics and pressure equalization tube (PET) placement. International studies demonstrate high success with these approaches, but U.S. pediatric data remains limited.
METHODS: Pediatric patients diagnosed with AM identified via Current Procedural Terminology (CPT®) coding and International Classification of Diseases (ICD-10) between January 2010 and October 2021 were reviewed using PearlDi

BACKGROUND: Acute mastoiditis is a complication of acute otitis media in children that often requires surgical intervention. The increasing availability of procedural sedation in pediatric emergency departments (PED) means that some surgical procedures can be performed in the PED rather than in the operating room (OR), but the clinical outcomes of this practice remain unclear.
OBJECTIVE: To compare the outcomes of Tympanostomy Tube (TT) insertion for acute mastoiditis performed in the PED vs. in the OR in terms of complications, readmissions, and effectiveness.
METHODS: A retrospective obser

OBJECTIVES: To evaluate the association between prior community-based antibiotic treatment for acute otitis media (AOM) and the incidence and severity of complications in children hospitalized with acute mastoiditis (AM), noting that this design does not address whether prior antibiotic treatment prevents AM development.
METHODS: This retrospective cohort study analyzed medical records of 522 children (age < 15 years) diagnosed with AM at a tertiary medical center between 2015 and 2025. Children were categorized by prior antibiotic exposure: those receiving antibiotics for > 48 h before hospi

BACKGROUND: There has been a recent increase in the incidence of paediatric acute mastoiditis (AM) with a global trend towards more conservative management strategies. Our aim was to evaluate the contemporary management of paediatric AM in Aotearoa New Zealand.
METHODOLOGY: A retrospective observational study including AM patients aged <18 years treated in five tertiary hospitals across Aotearoa New Zealand between 2014 and 2022.
RESULTS: There were 212 cases of AM with 104 cases (49%) of uncomplicated AM and 108 cases (51%) of complicated AM. Complications included 75 cases (35%) of sub-per

Complicated infections of the middle ear and paranasal sinuses remain a relevant clinical challenge. Although acute otitis media and acute sinusitis are generally self-limiting, a small but clinically significant proportion of cases progress to severe local, orbital, or intracranial complications. These include acute mastoiditis, labyrinthitis, facial nerve palsy, orbital complications, frontal bone osteomyelitis, intracranial abscess, meningitis, and cerebral venous sinus thrombosis, all of which are associated with considerable morbidity and potential mortality. The current review provides a

Acute mastoiditis is a recognized intratemporal complication of acute otitis media in children, but progression to frank temporal bone osteomyelitis with extensive cortical necrosis is exceptionally rare in infancy. This report presents a 1-year-old infant with refractory acute mastoiditis complicated by destructive temporal bone osteomyelitis. High-resolution computed tomography with 3D reconstruction demonstrated extensive cortical loss. Emergency subtotal mastoidectomy and radical debridement were performed. Deep intraoperative culture yielded multidrug-resistant Staphylococcus hominis, and

Background/Objectives: Recurrent acute otitis media (AOM) in children is known to cause cumulative cochlear and vestibular injury. Whether a single fulminant episode severe enough to require surgical intervention produces an analogous long-term audiovestibular signature, and whether infection severity contributes to outcome independently of cumulative episode count, is unclear. The present study addressed this gap. Methods: In this single-centre retrospective cohort study, 65 paediatric patients who had undergone surgical treatment for acute mastoiditis-the fulminant form of AOM-between July 2

BACKGROUND: Granulomatosis with Polyangiitis (GPA) is an antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis that predominantly affects the upper or lower respiratory tracts and the kidney. While otologic involvement is relatively common in GPA, isolated destructive mastoiditis as the initial manifestation is exceedingly rare, often leading to misdiagnosis.
CASE PRESENTATION: A 69-year-old female presented with a 3-months history of progressive bilateral hearing loss and otorrhea. Based on clinical and radiological findings, she was initially diagnosed with acute mastoiditis but

Subdural empyema, mastoiditis, and sinus venous thrombosis are rare infections that may lead to serious and potentially life-threatening complications. A 2-year-old male patient with no known underlying disease presented with a 2-day history of ear pain, sore throat, and fever. Physical examination revealed no significant pathology other than hyperemia of the left tympanic membrane. Intravenous ceftriaxone was initiated, and the patient was hospitalized because of persistent fever and poor oral intake. However, despite 3 days of treatment, the fever persisted, and an increase in acute phase re
