Observations on mastoid versus ear canal recorded cochlear microphonic in newborns and adults.

The cochlear microphonic (CM) may play an important role in the diagnosis of auditory neuropathy (AN) in newborns. However, since the CM tends to mirror the waveform of the acoustic stimulus, conscientious recording methodology must be applied to separate true response from artifact. The difficulty in achieving this separation has limited the clinical usefulness of the CM. In an effort to call attention to the importance of recording protocol when measuring the CM, the present study was designed to optimize CM recordings in humans by investigating the following parameters: (1) secondary minus

Development of mastoid air cell system in children treated with ventilation tubes for early-onset otitis media: a prospective radiographic 5-year follow-up study.

OBJECTIVES/HYPOTHESIS: Although most studies have agreed that small mastoid air cell systems correlate with long-standing otitis media, the extent to which the environmental factors affect the development of MACS remains undetermined. We investigated the radiographic development of mastoid air cell systems in children with recurrent acute otitis media or otitis media with effusion who were treated with ventilation tubes early in life. STUDY DESIGN: Prospective follow-up. METHODS: Of 305 consecutive patients aged less than 17 months with recurrent acute otitis media or otitis media with effus

Chronic suppurative otitis media and related complications at the University Clinic of Kinshasa.

During the five-year period from 1998 to 2002, the ENT department of the University Clinic of Kinshasa treated 343 patients suffering from chronic suppurative otitis media (CSOM) and complications. Two hundred and seven patients had simple, 28 had cholesteatomatous form of CSOM, and 108 patients presented with intratemporal or intracranial complications. Roughly half of the patient group consisted of children younger than 10 years. The simple form of CSOM affects men and women equally, but the cholesteatomatous form was more prevalent among men. Otorrhoea and hearing loss were the major presen

Pulsatile tinnitus: a review of the literature and an unusual case of iatrogenic pneumocephalus causing pulsatile tinnitus.

BACKGROUND: Pulsatile tinnitus is frequently attributed to identifiable and treatable causes, in contrast to the more common subjective non-pulsatile tinnitus. It usually originates from vascular structures as a result of either increased blood flow or lumen stenosis; atherosclerotic carotid or subclavian artery disease; arterial, venous, or arteriovenous malformations, fistulas, or dissection; and paragangliomas. Other causes have also been reported, with often unclear pathophysiology. OBJECTIVE: The aim of this paper is to present a case of pulsatile tinnitus secondary to iatrogenic pneumoc

[P50 component of auditory evoked potentials in persons with schizophrenia and their first degree relatives].

UNLABELLED: A pair of auditory stimuli presented in a 0.5 second interval cause a reduction of the P50 auditory evoked potential (P50 AEP) amplitude after the second stimuli in the pair. In some, but not all studies, a clear deficit of such P50 AEP gating was observed in schizophrenic patients. It was present constantly irrespective of the clinical state. This deficit was also found in first degree relatives of schizophrenic patients. However the results of the studies, as well as their interpretation remain controversial. AIM: To differentiate between P50 AEP in groups of schizophrenic patie

Van der Woude syndrome with sensorineural hearing loss, large craniofacial sinuses, dental pulp stones, and minor limb anomalies: report of a four-generation Thai family.

A four-generation Thai family affected with Van der Woude syndrome is reported. The disorder appeared to be originally inherited from a person who was half Thai and half Pakistani. The lip lesions found in this family were varied and did not appear to be related to other phenotypes. There were some clinical manifestations possibly specific for the condition in this family. They included sensorineural hearing loss, prominent frontal bone, large frontal/sphenoidal/maxillary sinuses with increased mastoid air cells, long tooth roots, dental pulp stones, ankyloglossia, brachydactyly of hands, brac

Brain responses to number sequences with and without active task requirement.

OBJECTIVE: To examine the effect of the active task involvement versus the passive knowledge of the structure of the stimulation on event-related potentials (ERPs) to violation in number sequences. This should contribute to future development of an ERP test for neurological patients. METHODS: ERPs were recorded in subjects who listened to 4-item sequences of numbers incrementing by one. Half of the sequences had an appropriate end item, whereas the other half ended inappropriately. The role of active versus passive condition was investigated in a between-subject design. RESULTS: Incorrectly

Chronic suppurative otitis media.

Restoring precise muscular control in the poststroke wrist/hand (W/H) demands sensorimotor integration to correct compensatory neuroplasticity. However, current rehabilitation robots inadequately modulate ascending somatosensory pathways from specific muscles. This study developed an electromyography (EMG)-driven soft robot with electro-vibro-feedback (EVF-robot) for targeted somatosensory priming in W/H muscles. This system integrates (a) focal vibratory stimulation and neuromuscular electrical stimulation for recruiting the somatosensory pathways of the targeted W/H flexors and extensors; (b

Tumor necrosis factor-alpha and transforming growth factor-beta reflect severity of liver damage in primary biliary cirrhosis.

BACKGROUND AND AIMS: The pathogenesis of primary biliary cirrhosis (PBC) is unknown. The role of cytokines such as tumor necrosis factor-alpha (TNF-alpha) and transforming growth factor-beta (TGF-beta), and the effect of ursodeoxycholic acid (UDCA) in modifying the cytokine environment in patients with PBC has remained largely unstudied. Our aims were to determine: (i) the relationship between serum levels of TNF-alpha and TGF-beta and the severity of PBC; and (ii) the effects of UDCA therapy on TNF-alpha and TGF-beta levels in patients with PBC. METHODS: We studied 90 patients who had been t

The development of the middle ear in neonatal chinchillas II. Two weeks to adulthood.

Studies of auditory function in the human neonate indicate adult-like hearing sensitivity, mature cochlear function and well-developed responses in the auditory pathway. Paradoxically, measurements of middle ear function are characterized by responses that would be interpreted as abnormal in older subjects. Consequently, there is not an accepted clinical test for middle ear disease in the newborn population. Like human neonates, chinchillas have normal hearing sensitivity at birth, but middle ear function tested by multifrequency tympanometry is abnormal compared to the adult. A previous study

Intracranial extension of acquired aural cholesteatoma.

OBJECTIVE: Cholesteatoma of the petrous bone extending into the intracranial region is an unusual occurrence. Most cases have been attributed to secondary extension of a primary epidermal blastomatous malformation of the temporal bone into the middle or posterior fossae. Within the past two and a half decades, intracranial extension of acquired aural cholesteatoma has been recognized as a likely alternative to this mechanism. Recent literature has rejoined this observation by considering both primary and secondary cholesteatoma of the petrous bone as a single group, petrosal cholesteatoma. The

[The presence of air in epidural hematoma due to congenital bone defect in the mastoid tegmen--a case report].

Presence of air in acute epidural hematomas has been described by several authors. Although the air trapped in the epidural space is thought to be derived from the air-containing structure, there are some cases in which there is an absence of any demonstrable fracture involving air-containing structure. We report a very rare case of air in an epidural hematoma which was not related to fracture involving air-containing structure. The patient was a 30-year-old man whose left temporal bone was struck during a fall. CT scan revealed acute epidural hematoma with a large amount of air in the epidura

INTRODUCTION: Facial palsy can be a consequence of untreated chronic suppurative otitis media. This disease can last for many years manifesting as partial deafness and ear effusion resulting in palsy after spreading of pathological process in the surrounding cranial structures. The patient with facial palsy cannot rise his forehead, nor close his eye on the involved side, while his cheek and lips just fall. MATERIAL AND METHODS: This retrospective research included patients operated for chronic suppurative otitis media with facial palsy during the last ten years. We have examined data on dura
