Novel Chronic Rhinosinusitis (CRS) Subtypes Based on Clinical Features and Response to Treatment.

BACKGROUND: Current classification of chronic rhinosinusitis (CRS) is based primarily on the absence or presence of nasal polyposis. OBJECTIVE: To (1) examine novel CRS classifiers based on clinical characteristics and (2) examine whether these classifiers predict response to treatment. METHODS: Between 2004 and 2020, patients seen initially at Massachusetts General Hospital meeting the CRS definition (N = 942 cohort) were analyzed retrospectively for clinical characteristics. Patients seen through 2016 to 2020 (N = 312 cohort) were analyzed longitudinally for their response to treatment, in

First reported case of an intradural otic polyp arising from a cranial nerve with concomitant middle ear cholesteatoma.

BACKGROUND: Otic (aural) polyps are benign inflammatory lesions typically arising from chronic middle ear inflammation and commonly associated with cholesteatomas. While generally confined to the external auditory canal or middle ear, their occurrence within the cranial cavity is exceptionally rare. CASE DESCRIPTION: We present a case of a 69-year-old female who presented with a 2-week history of right House- Brackmann equivalent grade 5 facial nerve palsy and profound right sensorineural hearing loss. Magnetic resonance imaging brain demonstrated a middle ear cholesteatoma extending into the

Fibroepithelial Polyp of the External Auditory Canal Extending to the Eustachian Tube: A Rare Clinical Entity.

Fibroepithelial polyps are benign mesodermal lesions that are an uncommon occurrence within the external auditory canal and the middle ear. The present case involves a 39-year-old male patient who has been experiencing hearing loss and intermittent ear discharge for a period of 10 years. Otoscopic examination revealed a smooth mass that completely filled the external auditory canal. A thorough analysis of the computed tomography results revealed the presence of soft-tissue density extending to the middle ear and Eustachian tube, with no evidence of bone destruction. Noncontrast magnetic resona

PD98059 inhibit the proliferation and differentiation of osteoblasts in the formation of tympanosclerosis via ERK1/2-MAPK signaling pathway.

Tympanosclerosis (TS) has become a common pathological condition of the middle ear, but the underlying mechanism of it is still ambiguous. It was found that osteoprotegerin/receptor activator of nuclear factor kappa B ligand (OPG/RANKL) axis played an important role in the development of TS and was regulated by the extracellular signal regulated kinase 1/2-mitogen activated protein kinase (ERK1/2-MAPK) pathway. However, whether ERK1/2-MAPK pathway mediates the occurrence of TS by regulating OPG/RANKL axis has not been reported. In this study, MAPK and calcium pathway were found significantly a

The role of matrix metalloproteinase expression levels in the pathogenesis of tympanosclerosis.

PURPOSE: Tympanosclerosis is a disease characterized by the accumulation of hyaline calcareous deposits in the tympanic membrane, tympanic cavity, ossicular chain, ligaments, and rarely, in the mastoid. Matrix metalloproteinases, which are involved in physiological and pathologic processes, are also likely to play a role in tympanosclerosis. In our study, we aimed to investigate the presence of MMP 2 and 9 in patients with chronic otitis media with tympanosclerosis and its possible effect on tympanosclerosis severity and prognosis. METHODS: In our study, 45 patients with myringosclerosis or m

Engineering the human tympanic membrane: lessons from mechanics, modelling, and materials.

The tympanic membrane (TM) plays a pivotal role in auditory transduction by converting acoustic signals into mechanical energy. Damage to the TM, whether from chronic perforations, infections, trauma, or pathological changes such as tympanosclerosis, can significantly impair its biomechanical function and lead to conductive hearing loss. Tympanoplasty remains the standard intervention, traditionally utilising autologous grafts such as temporalis fascia or cartilage. However, these biological materials present limitations, including donor site morbidity, variable mechanical properties, and pote

Otoscleroma of the Middle Ear and Mastoid Cavity with Facial Palsy: A Case Report.

INTRODUCTION: Scleroma is a chronic, specific granulomatous disease that affects the head and neck mucosa. Its common sites are the nose and larynx; however, it might affect other areas. One of the rare sites to be affected is the middle ear and mastoid cavity, for which the term otoscleroma was coined. We present such a rare case in this report. CASE REPORT: A 47-year-old patient with a history of both old laryngoscleroma and recent Rhinoscleroma. He presented with symptoms of ear discharge and facial palsy. Examination revealed complete facial nerve palsy and an external auditory canal poly

Choanal Polyps With Osseous Metaplasia From the Nasal Septum: A Case Report and the Literature Review.

Choanal polyps belong to a special type of nasal polyps, which are quite uncommon if originating from the nasal septum, especially those with osseous metaplasia. In this article, we report the case of a 63-year-old male patient with persistent nasal obstruction on the right side. An irregular light yellow lobulated mass with smooth surface could be visualized in the nasal cavity through nasal endoscopy, arising from the right nasal septum and extending to the nasopharynx. Computed tomography scan showed a large soft tissue shadow of the nasal meatus, with ossified structure in the center. Hist
