[Cochlear implantation in Ménière's disease].

Menière's disease is a chronic inner ear disorder characterized by recurrent vertigo attacks, tinnitus, and fluctuating hearing loss, which may progress to severe sensorineural hearing impairment in a subset of patients. Since 2015, international consensus criteria distinguish between definite and probable disease. Current therapeutic strategies follow a stepwise approach, ranging from non-ablative to ablative interventions. Cochlear implantation (CI) has emerged as an effective option for auditory rehabilitation and has also shown beneficial effects on vertigo and tinnitus. Combined strategie

Genetically predicted plasma metabolites mediate the relation between inflammatory factors and Meniere's disease.

OBJECTIVE: Emerging evidence suggests inflammation contributes to Meniere's Disease (MD), a chronic vestibular disorder characterized by vertigo, hearing loss, and tinnitus. However, the causal role of Inflammatory Factors (IFs) and the mediating effects of Plasma Metabolites (PMs) in MD remain unclear. This study investigated bidirectional causality between IFs and MD and evaluated PMs as potential mediators. METHODS: Using Genome-Wide Association Study (GWAS) summary data, we performed bidirectional Mendelian Randomization (MR) analyses to assess causal links between 91 IFs, 1,400 PMs, and

Prospective MR Evaluation of Endolymphatic Hydrops Using Half-Dose Gadopiclenol.

BACKGROUND AND PURPOSE: Gadopiclenol is a next-generation macrocyclic gadolinium-based contrast agent (GBCA) distinguished by its high T1 relaxivity and kinetic stability. It was developed to address the clinical need for reduced gadolinium dosing while maintaining high diagnostic accuracy, thereby minimizing potential long-term risks associated with gadolinium retention. Although various neuroradiology applications have been explored, the potential benefits of gadopiclenol's increased T1 relaxivity have not been investigated for the purpose of evaluating endolymphatic hydrops (EH) using delay

A Stress-Neuroendocrine-Myeloid Inflammation Axis Is Associated with the Progression of Ménière's Disease.

BACKGROUND: Ménière disease (MD) is a chronic inner ear disorder of unknown etiology. Although an immune-inflammatory link is suspected, the upstream triggers and cellular mechanisms connecting psychosocial stress to inner ear pathology remain poorly defined. This study aimed to investigate the role of stress-related, myeloid cell-derived inflammation in the progression of MD. METHODS: This multi-cohort study involved 384 MD patients (62.5% female); and 138 healthy controls (HCs) (46.4% female). Perceived stress was evaluated in 110 MD patients and 65 HCs. Transcriptional profiles were charac

Association of body mass index and cardiovascular risk profile with ménière's disease: A case-control study.

This case-control study investigated the association between Body Mass Index (BMI), cardiovascular risk factors, and Meniere’s disease (MD). We enrolled 100 patients with definite MD and 100 age- and sex-matched controls. We assessed BMI categories, cardiovascular risk factors, and medication use. Hearing loss was evaluated via pure-tone audiometry. Multivariable logistic regression, adjusted for confounders, identified independent associations. BMI distribution differed significantly. Overweight was more prevalent in MD patients (73% vs. 22%), while normal weight was less common (19% vs. 72%;

Imaging meets function: radiological and vestibular predictors of long-term cochlear implant outcomes in patients with Ménière disease.

INTRODUCTION: Although the proportion of Ménière disease (MD) patients who meet candidacy for CI remains relatively low, this subgroup has garnered increasing clinical interest. The main objective of this work is to evaluate the influence of preoperative vestibular function and radiological markers of endolymphatic hydrops (EH) on long-term auditory outcomes after cochlear implantation (CI) in patients with definite Ménière’s disease (MD). METHODS: All patients underwent CI and were assessed using a dedicated 3T MRI hydrops protocol. Audiological outcomes were measured using pure-tone audiomet

The Effect of Etiology, Array Type and Duration of Deafness on Focused Detection Thresholds in Cochlear Implant Listeners.

Cochlear implants (CIs) are considered effective at restoring some hearing abilities to individuals with severe hearing loss. However, there is considerable variability in performance outcomes among adults with CIs. One likely source of variability is the quality of the electrode-neuron interface (ENI), which describes how well implanted electrodes stimulate nearby spiral-ganglion neurons (SGNs) in the cochlea. Absolute thresholds, measured via focused electrical stimulation, have been proposed as a behavioral estimate of ENI. In this retrospective study, focused-stimulation threshold patterns

Case Report: Recurrent vestibular episodes in a dog resembling Meniere's disease.

Ménière's disease, characterized by episodic vertigo, sensorineural hearing loss, and tinnitus, is well-documented in humans but rarely reported in veterinary medicine. This case report describes a 15-year-old male mixed-breed dog with recurrent vestibular episodes lasting 30 min to several hours. Clinical signs included ataxia, head tilt, and nystagmus. MRI and diagnostic workups ruled out common structural causes of vestibular signs and ear diseases. Audiometric testing revealed bilateral sensorineural hearing loss with preserved high-frequency hearing in the right ear, a pattern resembling

Cochlear neural degeneration is key to the variation in word scores among people with similar thresholds.

Patients with Ménière's disease show lower word scores than those with other types of sensorineural hearing loss [Grant et al., 2022, Sci. Reports 12(1):8929]. We hypothesized that cochlear neural degeneration is a key driver of these differences among patients with similar audiograms. Here, we quantified histopathology in human cochleas with hearing loss from several etiologies and assessed the contribution of each cell type to word scores in a multivariable regression. We studied Ménière's cases (n = 44), contralateral (unaffected) ears of Ménière's cases (n = 10), age-matched normal-aging c

Circulating Clues in Ménière's Disease: Elevated Cell-Free DNA and a Pro-Inflammatory Signature in Patients' Blood.

Ménière's disease (MD) is thought to involve dysfunction of the blood-labyrinth barrier, but circulating mechanisms of endothelial injury remain poorly understood. The present study investigated whether cell-free DNA (cfDNA) and inflammatory mediators in plasma contribute to vascular stress and barrier disruption in MD. cfDNA levels were significantly elevated in plasma from patients compared with plasma from healthy controls. Exposure of primary human stria vascularis endothelial cell monolayers to plasma from MD patients led to decreased transepithelial electrical resistance and a significan

Longitudinal study of Emotional Intelligence Scale items associated with improvement in Meniere's disease.

BACKGROUND: Meniere's disease is a stress-related inner ear disorder characterized by recurrent vertigo and auditory symptoms. Evidence linking stress-coping abilities to clinical outcomes remains limited. Emotional intelligence (EI), particularly the situation-management domain of the Emotional Intelligence Scale (EQS), may influence symptom improvement through adaptability and behavioral flexibility. OBJECTIVE: To examine whether EI assessed by the EQS is associated with improvement in Meniere's disease. METHODS: This retrospective study included 93 patients aged 23-82 years who had baseli

Age-adjusted analysis of cVEMP frequency tuning using spline approximation for diagnosis of endolymphatic hydrops.

BACKGROUND: Although the cervical vestibular-evoked myogenic potential (cVEMP) frequency response has diagnostic value in Ménière's disease (MD), a method that accounts for age-related changes is needed. AIMS/OBJECTIVE: To develop an age-independent diagnostic approach for MD using cVEMP. SUBJECTS AND METHODS: cVEMP frequency responses were recorded at six stimulus frequencies (250-2000 Hz) in 35 definite unilateral MD ears and 55 control ears. The discrete peak frequencies were converted into a continuous variable by spline approximation. This spline peak was normalized age-adjusted Z (AAZ)

Low-frequency transient-evoked otoacoustic emission characteristics as potential indicators of hearing improvement in acute Ménière's disease: an exploratory analysis.

PURPOSE: Fluctuating hearing loss in Ménière’s disease (MD) presents a key clinical challenge due to lack of reliable prognostic markers. The clinical value of transient-evoked otoacoustic emissions (TEOAEs) low-frequency components remains underexplored due to technical difficulty in signal extraction. This study profiled the group delay (GD) and magnitude of low-frequency TEOAEs and observed the association with hearing improvement after acute MD episode. METHODS: 31 patients with unilateral MD were prospectively enrolled during the acute phase of cochleovestibular symptoms. TEOAE and pure-t
