Split-GFP Reporter-Based Detection of Chlamydia trachomatis Inclusion Damage.

Chlamydia trachomatis, an obligate intracellular pathogen responsible for trachoma and widespread urogenital infections, replicates exclusively within the confines of a membrane-bound vacuole known as the inclusion. This compartment shields the bacterium from host cellular immune detection; thus, clarifying how inclusion integrity is maintained and protected from vacuole-destabilizing host attacks may identify new therapeutic strategies. To enable such insights, we recently developed split-GFP-based microscopic reporters that can detect early forms of membrane damage and track individual bacte

Serologic Assessment of Public Health Action Needed for Trachoma, Coastal Ecuador, 2021-2024.

We evaluated the probability of needing public health actions against trachoma in Esmeraldas Province, Ecuador, by estimating seroconversion rates of Chlamydia trachomatis plasma gene protein 3 IgG in children. Seroconversion rates suggest high probability of the need for public health interventions in rural communities and motivate further assessment.

Design considerations for incorporating serological monitoring into trachoma prevalence surveys.

Serology is increasingly used to monitor infection transmission and disease elimination. Embedding dried blood spot collection in trachoma surveys allows transmission intensity inference through population-level seroconversion rate (SCR) estimation, but no formal assessment of required sample size has been published. Using serology data from 40 trachoma prevalence surveys, we estimated seroprevalence intra-cluster correlation coefficient, a key design parameter, and assessed survey design considerations for incorporating serological monitoring. Design scenarios focused on proposed operational

Acquired Distichiasis in Dermatomyositis: A Possible Delayed Sequela of Chronic Periocular Inflammation.

Acquired distichiasis is an uncommon eyelid disorder characterized by an accessory row of eyelashes arising from the meibomian gland orifices, most often in the setting of chronic inflammatory or cicatricial disease. We report a case of a 65-year-old woman with longstanding dermatomyositis, recurrent periocular inflammation, and episodes of blepharitis who developed bilateral upper-eyelid distichiasis. Review of clinical photographs from 2020 and from a severe periocular flare in 2023 showed no clearly visible accessory row. At follow-up in January 2026, clinical and dermoscopic examination de

Identification of secondary microglial formation centers in the human fetal brain.

BACKGROUND: Trachoma is the leading infectious cause of irreversible blindness. In areas where trachoma is endemic, active trachoma is common among preschool-aged children, with varying magnitude. There is a dearth of information on the prevalence of active trachoma among preschool-aged children (the most affected segment of the population). PURPOSE: The study aimed to assess the prevalence of clinically active trachoma and its associated risk factors among preschool-aged children in Arba Minch Health and Demographic surveillance site, Southern Ethiopia. PATIENTS AND METHODS: A community-bas

Therapeutic Potential of Desert Truffles in the Management of Eye Infections: Demystifying the Fact.

Truffle, an ascomycetous, hypogeous macrofungi, has long been recognized and valued for its therapeutic and dietary properties. Of late, a range of medicinal compounds, such as ergosterol, tuberoside anandamide, polysaccharides, and phenolics exhibiting anti-inflammatory, immunomodulatory, anticancer, antibacterial, and aphrodisiac properties have been identified in truffles. This review provides an update on contemporary truffle research with a focus on antimicrobial potentials and aims to draw the attention of researchers to exploit the therapeutic potential of truffles in the management of

Trachoma prevalence surveys in 15 indigenous and non-indigenous evaluation units in Brazil, 2018-2023.

BACKGROUND: To provide the groundwork for a future declaration of elimination of trachoma as a public health problem in Brazil, we conducted house-to-house surveys following WHO methodological guidance. METHODS: An observational cross-sectional study was conducted in 10 non-indigenous and five indigenous evaluation units (EUs) from 2018 to 2023; data on six EUs are reported here for the first time. Two-stage cluster sampling was used: 30 clusters per EU, and 30 households per cluster. We estimated the prevalence of trachomatous inflammation-follicular (TF) in 1-9-y-olds and trachomatous trich

Does persistent active trachoma mandate antibiotic mass drug administration? A comparison of prevalence of trachomatous inflammation-follicular with that of conjunctival infection and anti-chlamydial antibodies, Western Province, Zambia.

BACKGROUND: The evaluation unit comprising Kaoma, Luampa, and Nkeyema districts, Western Province, Zambia, has persistent active trachoma. In 2023, we sought to compare the evaluation unit-level prevalence of the active trachoma sign, trachomatous inflammation-follicular (TF), to that of conjunctival Chlamydia trachomatis (Ct) infection and anti-Chlamydia trachomatis (Ct) seropositivity. METHODS: We conducted a cluster-sampled cross-sectional survey. In selected households, we examined all consenting residents ≥1 y of age for trachoma. We collected dried blood spots (DBSs) by finger-prick fro

Developing strategies to address persistent and recrudescent trachoma in Ethiopia using the Delphi techniques.

BACKGROUND: Despite significant progress, trachoma persistence and recrudescence in Ethiopia pose a challenge to global elimination of trachoma as a public health problem. Developing evidence-based, expert-informed strategies is crucial for achieving global elimination targets by 2030. Our aim was to develop strategies to address persistent and recrudescent trachoma using Delphi techniques. METHODS: The study reviewed literature and studies from districts in southern Ethiopia where trachoma prevalence remains above the elimination threshold. A modified Delphi technique was employed to achieve

Osteo-odonto-keratoprosthesis: a surgical procedure for treating advanced corneal diseases.

Corneal blindness remains a significant challenge in modern ophthalmology, accounting for 12% of global blindness cases. While corneal transplantation is often an effective treatment, its success is limited in patients with severe ocular surface diseases such as Stevens-Johnson syndrome, trachoma, or autoimmune-related dry eye disease. For these patients, osteo-odonto-keratoprosthesis (OOKP) has emerged as a highly specialized surgical solution, utilizing a patient's tooth as a scaffold for an artificial cornea, providing long-term visual restoration. This article traces the historical evoluti

A Randomised Trial of Three Face-Washing Methods for the Removal of Chlamydia trachomatis From the Faces of Children With Severe Active Trachoma.

OBJECTIVES: A three-arm, open, parallel-group randomised trial compared three face-washing methods for cleaning Chlamydia trachomatis from the faces of children with severe active trachoma. The impact of face-washing on Chlamydia trachomatis on the hands of children and their caregivers and Chlamydia trachomatis duration on faces and hands were investigated as secondary objectives. METHODS: Children aged 1-7 years in Oromia, Ethiopia, were screened for active trachoma. We aimed to recruit 470 children with severe conjunctival inflammation; 141 were expected to have concurrent conjunctival inf

Evaluation of fluorometholone as adjunctive medical therapy for trachomatous trichiasis surgery (FLAME): a parallel, double-blind, randomised controlled field trial in the Jimma Zone, Ethiopia.

BACKGROUND: In trachoma, trachomatous trichiasis mediates visual impairment. Trachomatous trichiasis surgery has an unacceptably high relapse incidence. We hypothesised that anti-inflammatory therapy with fluorometholone 0·1% suspension (hereafter fluorometholone) eyedrops perioperatively twice daily for 28 days would safely, efficaciously, and cost-effectively reduce postoperative trachomatous trichiasis relapse (PTT). METHODS: In this randomised, parallel, placebo-controlled trial, we compared placebo (artificial tears) with fluorometholone for eyes undergoing trachomatous trichiasis surger

The Effect of a Water, Sanitation, and Hygiene Intervention on Clinical Trachoma: A Cluster-Randomised Trial.

OBJECTIVES: Few randomised trials have been done to assess the impact of hygiene interventions on trachoma even though hygiene improvements feature prominently in the strategy for global elimination of trachoma. This study's aim was to determine if implementing an integrated water, sanitation, and hygiene (WASH) intervention in a hyperendemic region of Ethiopia would reduce clinical signs of trachoma. METHODS: In a parallel-group cluster-randomised trial, 40 Ethiopian communities were randomised in a 1:1 ratio to a WASH intervention or to delayed intervention (i.e., control) (ClinicalTrials.g

Age-Specific Prevalence of Trachoma in Remote Australian Communities.

PURPOSE: Trachoma is endemic in Australia, affecting remote Indigenous communities in northern, central and western Australia. The World Health Organization (WHO) defines a key requirement of elimination as a public health problem being the prevalence of active trachoma (characterised as trachomatis inflammation - follicular) below 5% in children aged 1-9 years. In Australia, screening is based in school settings and focuses on children aged 5-9 years. There is international evidence that active trachoma may be more common in children under five, meaning true Australian prevalence may be under

The Cost of Annual and More Frequent Than Annual Mass Drug Administration for Trachoma in Two Districts in Amhara, Ethiopia.

Ethiopia accounts for 59% of the global trachoma burden. To eliminate trachoma as a public health problem by 2030, modified mass drug administration (MDA) strategies have been proposed, including more frequent than annual MDA. In the present study, the cost of the "child MDA" (CMDA) strategy, defined as an initial community-wide MDA treatment followed by another treatment targeting children aged 6 months to 9 years, was estimated in the Lasta and Wadilla districts, Amhara, Ethiopia. A micro-costing analysis was conducted from a payer perspective, documenting the total financial and economic co

Monitoring of Chlamydia trachomatis infection and antibodies in low-prevalence districts of Amhara, Ethiopia: Insights from a hyper-endemic trachoma region.

BACKGROUND: Trachoma, an infectious eye disease caused by Chlamydia trachomatis (Ct), remains endemic in Ethiopia's Amhara region despite implementation of the Surgery, Antibiotic, Facial Cleanliness, and Environmental Improvement (SAFE) strategy since 2007. Once a formerly endemic district has reached the trachoma elimination threshold of a trachomatous inflammation-follicular (TF) prevalence of <5% among children ages 1-9 years and mass drug administration has been halted, there is concern that the prevalence could recrudesce back above 5% over time. Formerly endemic districts that are adjac

Australian Trachoma Surveillance Report 2023.

Trachoma is the world's leading infectious cause of preventable blindness and is linked to poor living conditions. Australia has remained the only high-income country where trachoma is endemic, primarily in remote Indigenous communities in the Northern Territory, South Australia and Western Australia. The Australian Government funds the National Trachoma Surveillance and Reporting Unit to analyse surveillance data annually to assess progress against World Health Organization (WHO) criteria for the elimination of trachoma as a public health problem. These criteria include (i) prevalence of trac

Trachomatous trichiasis surgeons appreciate using HEAD START for extended training during periods of low surgical activity: A preliminary study.

BACKGROUND: Trachomatous trichiasis (TT) surgeons routinely experience periods of low surgical activity, which may contribute to low or variable surgical skill. Surgeons need strategies for maintaining skill during these periods. METHODOLOGY/PRINCIPAL FINDINGS: We recruited 28 newly-trained TT surgeons for this pilot study in southern Ethiopia; we randomized 15 TT surgeons to receive extended surgical practice (Extended HEAD START or EHS group) and feedback using the HEAD START simulation device and 13 TT surgeons to follow standard practice (Standard HEAD START or SHS group) during a 5-month

Use of serology to assess the probability of public health action needed for trachoma in coastal Ecuador.

We evaluated the probability of need for public health action against trachoma in Esmeraldas province, Ecuador. Compared to global references, seroconversion rates to Chlamydia trachomatis Pgp3 in children suggest high probability of action needed in rural villages (91%) and lower in more urban areas (32%), motivating further trachoma assessment.
