Aberrant Internal Carotid Artery: A Rare Cause of Unfitness Revealed During Admission Audiometry.

INTRODUCTION: An aberrant tympanic segment of the internal carotid artery is a rare vascular anomaly. Clinical manifestations are variable and nonspecific, including hearing loss, tinnitus, vertigo, headache, and aural fullness. In military settings, it may remain undiagnosed, particularly when candidates are asymptomatic or do not report minor symptoms. This condition can render a candidate unfit for enlistment because of the risk of ear trauma during training or procedures. The French Armed Forces require comprehensive medical evaluations before enlistment, including audiometric testing and

Beyond the Air-Bone Gap: The Role of Bone Conduction Thresholds in Predicting Functional Outcomes and Guiding Surgical Decision-Making in Active Middle Ear and Bone Conduction Implants.

Introduction: In patients with conductive and mixed hearing loss, implantable hearing devices such as active middle ear implants (AMEIs) and bone conduction implants (BCIs) are established alternatives when conventional hearing aids fail. Although bone conduction (BC) thresholds are routinely used as eligibility criteria, their role as frequency-specific predictors of postoperative functional outcomes remains poorly defined. This study aimed to evaluate the influence of preoperative BC thresholds across the audiometric spectrum on postoperative speech recognition outcomes after implantation wi

Establishment of High Channel-Count Packaging in Active Implantable Medical Devices for Neuroprosthesis.

Neuroprostheses have contributed extensively to humankind's capacity to treat disease and injury. Hearing loss is routinely overcome; pain is controlled; and myriad neurological, sensory, motor, and psychological disorders are managed through interventions involving neuromodulation. Fabrication of active implantable medical devices (AIMDs) often relies on highly skilled operators, leading to high costs, reliability challenges, and limitations to further device miniaturization and manufacturing output. Extending AIMD benefits to other disorders may also require a substantial increase in the num

The "C-Bending-Spring-Tympanoplasty" for Reconstruction of Anterior Tympanic Membrane Defects: A Prospective Randomized Clinical Trial.

INTRODUCTION: Anterior tympanic membrane (TM) defects present a surgical challenge due to the lack of support for the graft, resulting in reduced success rates. Established anchoring tympanoplasty (EAT) techniques usually aim to anchor the graft to the anterior rim of the TM. The "cartilaginous bending spring tympanoplasty" (CBST) secures the graft to the TM by supporting it with a U-shaped bending spring cartilage placed underneath in the middle ear. Initial experimental studies have already shown the feasibility of CBST without impeding sound transmission. This prospective clinical study eva

Design and technical validation of a low-cost, titanium sheet metal TORP (SM-T) for manufacturing and use in LMICs.

OBJECTIVES: Total ossicular replacement prostheses are commonly used to restore hearing in the conductive hearing impaired. However, total ossicular replacement prostheses are expensive and scarcely available in low- and middle-income countries. Therefore, the aim was to develop and validate an open-source design of a total ossicular replacement prosthesis that can be produced locally for low- and middle-income countries hospitals. METHODS: Based on input from literature and clinicians a design was made and verified. Accuracy, re-bending safety, cadaver ear fitting, sound transmission and pro

Temporal bone thickness in Japanese pediatric patients: Implications for the timing of bone conduction device surgery.

OBJECTIVE: Western guidelines for bone conduction devices (BCDs) recommend implantation at age ≥5 years, but their applicability to Japanese children remains unclear due to potential racial differences in skeletal development. This study measured temporal bone thickness in Japanese children using computed tomography (CT) to determine the anatomically appropriate surgical age. METHODS: We analyzed head CT images from 217 Japanese children (0-15 years) without temporal bone abnormalities. Bone thickness was measured 1 cm posterior to the sigmoid sinus at two points: the "upper level" (superior

Conductive Hearing Loss is Associated With Dementia in the All of Us Research Program.

OBJECTIVES: Sensorineural hearing loss (SNHL) has been associated with cognitive impairment and dementia. However, because SNHL contains a neural component, it is difficult to completely exclude reverse causation whereby dementia causes worse SNHL. Conductive hearing loss (CHL), a purely peripheral phenomenon, would not have this mechanistic connection. We investigate the association between CHL and dementia in a large national cohort. METHODS: Cross-sectional study of ≥ 18-year-old (n = 338,048) voluntary participants from the NIH All of Us Research Program. The exposure was bilateral CHL de

Intraoperative ABR in Vibrant Soundbridge Surgery: Influence of Bone-Conduction Thresholds.

BACKGROUND: Vibrant Soundbridge (VSB) has shown effective hearing rehabilitation for patients with conductive, mixed, or sensorineural hearing loss. However, optimal floating mass transducer (FMT) coupling is critical for device performance. This study investigates the utility of intraoperative auditory brainstem response (ABR) for assessing coupling efficiency and guiding surgical decision-making, particularly in patients with normal bone conduction. METHODS: This retrospective cohort study included VSB patients with complete audiometric data. Coupling efficiency was assessed by comparing po

Silent obstruction: The hidden role of aberrant internal carotid artery in middle ear pathology.

PURPOSE: Aberrant Internal carotid artery (ICA) within the middle ear is a rare congenital vascular anomaly, occurring in approximately 1% of the population [1]. It often remains clinically silent or presents with nonspecific otological symptoms, making preoperative diagnosis challenging. This study aims to highlight the importance of early identification and safe management of aberrant ICA through a clinical case and literature review. METHODS: We report a case of 36 year old female presenting with bilateral chronic ear discharge and tympanic membrane retraction. Detailed otoscopic examinatio
